Health Disparities and Disrupted Puberty in Males.

Disrupted puberty in males may result from self-limiting constitutional delay of growth and puberty (CDGP) or abiding hypogonadism (ie, congenital hypogonadotropic hypogonadism, CHH). This article provides an overview of normal puberty, CDGP, and CHH and synthesizes current evidence on health and wellbeing outcomes. Particular attention is given to the drivers of disparities in outcomes and discussion of future directions to advance the care of males with disrupted puberty.

Peri-operative care of elective adult surgical patients with a learning disability.

People with a learning disability can experience significant problems in accessing healthcare and this may be partly reflected in worse health outcomes compared with the general population, including a shorter life expectancy. The Equality Act (2010) requires that organisations and individuals make changes to the way services are provided for all disabled people to mitigate, as far as possible, any disadvantage they may face in accessing these services. These changes are termed 'reasonable adjustments'. This article describes the reasonable adjustments that can be made to facilitate the admiss

Artificial Intelligence and Health Equity.

Artificial intelligence (AI) is transforming biomedical research, public health, and clinical care by offering opportunities to improve outcomes and advance health equity. Its promise stems from its ability to analyze complex health data to inform prevention, diagnosis, and treatment. However, concerns are growing that AI may worsen disparities, especially for racial and ethnic minorities, individuals with disabilities, and low-income populations, who often face a higher disease burden. Biased or incomplete data can result in inequitable outcomes. This review highlights strategies for equitabl

Decolonizing pain and health-related research to advance research and optimize health.

Pain and healthcare inequities persist as consequences of early colonial practices, which plague pain and health-related research today. Colonialism negatively contributes to known inequities among vulnerable marginalized communities. Scientists control overall study design and who participates in research, continuing colonial power practices. Past ethical shortcomings have fostered mistrust in communities. The model proposed advocates for decolonizing pain and health-related research to create shared understandings and collaborations to restore trust. By acknowledging the past, scientists can

Inclusion oral health: Advancing a theoretical framework for policy, research and practice.

In response to headlines about the oral health of persons experiencing social exclusion resonating in high-income countries, and research demonstrating the need for urgent action, a symposium entitled 'International Perspectives on Socially Inclusive Dentistry: A Call to Action' was organized for the IADR International Meeting of 2018. The aim of the symposium was to initiate an international dialogue on barriers to care, multidisciplinary action and examples of best practice for service delivery for people experiencing social exclusion; in other words, to develop the idea of inclusion oral he

Disparities in Huntington's disease care and research.

PURPOSE OF REVIEW: Disparities in care and outcomes are well described in common neurologic disorders; however, less is known about disparities in rare diseases such as Huntington's disease (HD). This review summarizes HD epidemiology in the US and globally while highlighting disparities in HD diagnosis and outcomes across various HD groups. The review also discusses how the lack of diversity in HD research threatens the validity of clinical trials and raises concerns about global accessibility to emerging therapies. RECENT FINDINGS: Global HD prevalence and incidence vary worldwide, though t

Progress for all: addressing disparities in sarcoidosis.

PURPOSE OF REVIEW: The impact of healthcare disparities in the treatment, care, and outcomes of patients with sarcoidosis has been described. There is paucity of literature on ways to address these disparities with a goal to improving health outcomes for patients with sarcoidosis. RECENT FINDINGS: Recent findings in other respiratory and systemic diseases suggest that multifaceted interventions directed at improving care at various levels including individual, family, and larger societal levels have been successful in dismantling some of the social and structural barriers to care and conseque

The Enduring Plague: How Tuberculosis in Canadian Indigenous Communities is Emblematic of a Greater Failure in Healthcare Equality.

Despite global strides made in prevention and treatment, tuberculosis (TB) remains an acute problem for Indigenous people in Canada. TB affects Indigenous communities at significantly higher rates than the general Canadian population, for whom it is a disease of the past. This paper suggests how colonialism and its history of violence have shaped the face of TB in Canada, and thus how TB is a telling point of analysis for considering the lack of equity and equality in healthcare delivery in Canada.

Health Equity: Foundation of Serious Illness Care.

Health equity is foundational to the delivery of high-quality serious illness care. Despite advances in palliative care, substantial disparities persist in symptom burden, quality of life, caregiver distress, access to specialty services, and end-of-life outcomes across racial, ethnic, socioeconomic, geographic, and other structurally marginalized populations. These inequities are driven not by individual patient characteristics alone, but by structural, institutional, and social factors including discrimination, language discordance, disability, transportation barriers, housing instability, f

Navigating asthma in pregnancy: Immunologic changes, barriers, and disparities in management.

Background: Asthma is the most prevalent chronic condition in pregnancy. It is associated with an increased risk of maternal and perinatal complications, including low birth weight, preeclampsia, and preterm delivery. In women with asthma, pregnancy-induced immunologic changes are dysregulated, including a high T helper type 17 (Th17) to regulatory T cell ratio, a shift from Th1 to Th2 immunity, and an increase in proinflammatory cytokines. Multiple barriers exist that prevent optimal asthma management during pregnancy. This review examines the multifaceted relationship between asthma and preg

Methods for studying health disparities in U.S. nursing homes: a scoping review.

BACKGROUND: Health disparities exist across healthcare settings, including nursing homes, contributing to preventable differences in care quality. Health disparities are a global issue, yet most studies on nursing home health disparities have been conducted in the United States. In this scoping review, our objective was to synthesize methods used in U.S. nursing home disparities research to gain insights to inform similar research in other countries. Specifically, we summarized different approaches for conceptualizing and measuring health disparities, available data sources, study designs, and

Multiplicative Impact of Adverse Social Determinants of Health on Outcomes in Lupus Nephritis: A Meta-analysis and Systematic Review.

OBJECTIVE: Social determinants of health (SDoH) likely contribute to outcome disparities in lupus nephritis (LN). Understanding the overall burden and contribution of each domain could guide future health equity-focused interventions to improve outcomes and reduce disparities in LN. Objectives of this meta-analysis were to 1) determine the association of overall SDoH and specific SDoH domains on LN outcomes and 2) develop a framework for the multidimensional impact of SDoH on LN outcomes. METHODS: We performed a comprehensive search of studies measuring associations between SDoH and LN outcom

Autoimmune Health Crisis: An Inclusive Approach to Addressing Disparities in Women in the United States.

Autoimmune diseases are identified by the chronic inflammation and tissue damage resulting from unregulated immune responses throughout the body. Systemic lupus erythematosus, type 1 diabetes mellitus, and Hashimoto's thyroiditis are among the 80+ characterized conditions, 80% of which are diagnosed in women. The compounded effects of biological sex and hormones; social identities, such as age, race, and gender; and other determinants on health highlight a pressing need for an inclusive approach to address disparities for women living with autoimmune diseases. Such an approach must recognize a

Extracranial-Intracranial Bypass for Moyamoya Disease: The Influence of Racial and Socioeconomic Disparities on Outcomes - A National Inpatient Sample Analysis.

BACKGROUND: Extracranial-intracranial (EC-IC) bypass is an established therapeutic option for Moyamoya disease (MMD). However, little is known about the effects of racial and ethnic disparities on outcomes. This study assessed trends in EC-IC bypass outcomes among MMD patients stratified by race and ethnicity. METHODS: Utilizing the US National Inpatient Sample, we identified MMD patients undergoing EC-IC bypass between 2002 and 2020. Demographic and hospital-level data were collected. Multivariable analysis was conducted to identify independent factors associated with outcomes. Trend analysi
