Severe complications of mediastinal pancreatic pseudocyst: report of esophagobronchial fistula and hemothorax.

We report two patients with alcoholic pancreatic pseudocyst which communicated to the mediastinal space through the aortic hiatus, in one patient resulting in hypotensive shock due to hemothorax, and in the other, resulting in esophagobronchial fistula via the mediastinal cyst. The first patient was successfully treated by radical resection of the pancreatic body and tail, and the spleen, with an ultrasonic scalpel, although inflammatory changes caused by pancreatitis were so prominent that the splenic vein was occluded. The second patient could not be treated surgically, because the superior

Massive Hemothorax from Bronchial Artery Bleeding after Endobronchial Ultrasound Transbronchial Needle Aspiration.

Endobronchial Ultrasound (EBUS) with Transbronchial Needle Aspiration (TBNA) is performed with a very low complication rate. We present a unique case of massive life-threatening hemothorax from bronchial artery bleeding after EBUS-TBNA, presenting in a delayed fashion and requiring operative intervention. Although exceedingly rare, along with the unusual delayed timing of the presentation, practitioners should be aware of this complication. It may be helpful to perform EBUS with color doppler examination of the subcarinal space to identify and avoid bronchial arteries prior to TBNA of subcarin

Massive hemothorax after removal of subclavian vein catheter: a very unusual complication.

A case of massive hemothorax developing a few minutes after removal of a central venous catheter is described in a 51-yr-old woman, who had undergone a renal transplant. The patient had an arteriovenous fistula for hemodialysis on the same side as the central catheter. The mechanism of the onset of this complication is discussed. We recommend avoiding positioning a central catheter on the same side as an arm arteriovenous fistula. Furthermore, we think it is necessary to monitor patients after removal, as is usually done after positioning, to detect this potentially fatal complication.

Hemothorax as an infrequent complication of aortic dissection in a young patient: a case report.

BACKGROUND: Acute aortic dissection (AAD) is a life-threatening condition with high mortality and a broad spectrum of clinical presentations. Hemothorax is an infrequent but catastrophic complication, particularly uncommon in young patients. CASE DESCRIPTION: We report the case of a 39-year-old man with treated systemic arterial hypertension and obesity who presented with acute chest pain of approximately 50 minutes duration prior to hospital arrival. The initial clinical presentation was suggestive of acute coronary syndrome, leading to early management in that direction. Non-contrast chest

Back to the drawing board: Exploring the composition and physical properties of traumatic hemothorax.

BACKGROUND: Despite a wide array of management options for traumatic hemothorax (HTX), rates of secondary intervention remain high. Exploring the composition and physical properties of HTX thrombi may inform improved management. We hypothesized that the composition and physical properties of HTX thrombi vary based upon time postinjury. METHODS: Hemothorax samples were collected during resuscitative thoracotomy or thoracic surgical procedures for rib plating or retained HTX evacuation. Hemothorax thrombi were grouped according to the timing of collection (acute <72 hours postinjury vs. subacut

Efficacy of a novel steerable chest tube system in a swine model of hemothorax.

Thoracostomy tubes (chest tubes) are the standard of care for thoracic trauma, including hemothorax. However, traditional chest tubes are susceptible to occlusion and lack the ability to be repositioned once inserted. This can lead to further life-threatening issues, such as retained hemothorax. Here we describe the investigation of a novel steerable chest tube, the SAM repositionable chest tube (RCT), which is designed with two-axis manipulation for effective drainage of the pleural space while maintaining sterility. We hypothesized that the treatment of hemothorax with RCT would result in an

Risk factors for delayed hemothorax in patients with rib fracture in the emergency department.

OBJECTIVES: Although rib fractures are a risk factor, not all rib fracture patients will develop delayed hemothorax. This study aimed to evaluate risk factors which can identify rib fracture patients in the emergency department who may develop delayed hemothorax. METHODS: Adult patients seen in the emergency room between January 2016 and February 2021 with rib fractures caused by blunt chest trauma were included in this retrospective observational study. Patients who underwent chest tube insertion within 2 days and those without follow-up chest radiographs within 2-30 days were excluded. We u

Cystic Atypical Carcinoid in the Thymus Causing Mediastinal Bleeding Followed by Hemothorax.

We report a case of cystic atypical carcinoid in the thymus that caused mediastinal bleeding. Atypical thymic carcinoid is a relatively rare disease, and few cases of cystic atypical carcinoid have been reported. There are some reports of anterior mediastinal bleeding caused by thymic tumors or cysts; however, reports on atypical carcinoid-associated bleeding are unavailable. Even if the mediastinal tumor is dominated by a cystic lesion, a more detailed examination should be considered. In addition, physicians should keep in mind that spontaneous bleeding can occur in cystic lesions of the ant

Recurrent Massive Hemothorax of Unknown Etiology in an 85-Year-Old Man.

An 85-year-old Japanese man, who was taking aspirin and edoxaban for previous myocardial infarction and atrial fibrillation, came to our hospital with a chief complaint of dyspnea for 3 weeks. Chest radiography showed a massive left pleural effusion (Fig 1A). Analysis of pleural fluid showed an elevated hematocrit level at 32.8% (blood hematocrit level, 32.0%), and he was diagnosed with hemothorax. However, he had neither coagulation disorder nor thrombocytopenia, and the pleural effusion was negative for atypical cells. These findings suggested that the antithrombotic and anticoagulant medica

Rib spur causing a hemothorax, pneumothorax, and diaphragmatic injury in a pediatric patient.

Pleural effusion is a relatively common condition encountered in the pediatric emergency department. Evaluation of pleural effusion in the emergency department typically includes advanced imaging such as computer tomography or ultrasound, as well as diagnostic thoracocentesis. We report a case of a 10-year-old female with a rib spur at the anterolateral left sixth rib that caused a hemothorax, pneumothorax, and diaphragmatic injury. The patient underwent video-assisted thoracoscopic surgery and resection of the rib spur. The procedure was well-tolerated without any complications.

The Volume of Thoracic Irrigation Is Associated With Length of Stay in Patients With Traumatic Hemothorax.

INTRODUCTION: Irrigation of the thoracic cavity at tube thoracostomy (TT) placement may decrease the rate of a retained hemothorax (RHTX); however, other resource utilization outcomes have not yet been quantified. This study evaluated the association of thoracic irrigation during TT with the length of stay and outcomes in patients with traumatic hemothorax (HTX). METHODS: A retrospective chart review was performed of adult patients receiving a TT for HTX at a single, urban Level 1 Trauma Center from January 2019 to December 2020. Those who underwent irrigation during TT at the discretion of t

Incidence and Predictive Factors of Massive Hemothorax Due to Thoracic Vertebral Fractures.

INTRODUCTION: Massive hemothorax due to thoracic vertebral fractures (MHTVF) is a potentially lethal condition; however, its epidemiological and clinical data have been rarely described. Thus, in this study, we aimed to evaluate the incidence, predictive factors, and clinical features of MHTVF. METHODS: This retrospective cohort study enrolled 202 consecutive patients (136 male and 66 female patients) with thoracic vertebral fractures treated at our institute between January 2009 and December 2019. Their mean age was 60.7 (range, 17-90) years. Unstable fractures accounted for 57.4% (n=116) of

[Tension Hemothorax Associated with Spontaneous Rupture of the Thoracic Aorta;Report of a Case].

A 94-year-old woman was admitted to our hospital with sudden onset of chest pain without any episode of trauma. Computed tomography(CT) revealed left massive pleural effusion and extravasation of contrast medium from the side chest wall to the back. Neither aortic aneurysm nor dissection was evident. During the investigation, the patient went into hypovolemic shock. Hemothorax due to bleeding from an intercostal artery was diagnosed, and emergency surgery was performed. A 1-mm hole was detected in the descending aorta, and closed by a single suture. The final diagnosis was spontaneous rupture

Successful treatment of a pancreatic pseudocyst accompanied by massive hemothorax: a case report.

BACKGROUND: It is rare to encounter massive hemothorax as a complication of pancreatic pseudocyst. In addition, as no obvious hypotension and abdominal discomfort were noted, it was difficult to consider gastrointestinal lesion a possibility. CASE PRESENTATION: A 54-year-old Taiwanese man had tightness on the left side of his chest and shortness of breath for 3 days. He had a history of acute pancreatitis 3 months ago. After history taking and a series of examinations including thoracocentesis and computed tomography of his abdomen and chest, the diagnosis was finally confirmed based on the h

[Hemothorax complicated with celiac artery compression syndrome (CACS); report of a case].

We report a case of hemothorax complicated with celiac artery compression syndrome (CACS). A 43-year-old man presented with a sudden onset left back pain. Computed tomography (CT) showed its hemothorax, esophageal artery aneurysm and severe stenosis of the celiac truncus with its anterior compression by median accurate ligament, and a diagnosis of CACS associated with rupture of the aneurysm was made. Emergent transcatheter arterial embolization of the aneurysm resulted in a technical failure, although the patient's condition was stable and performed esophageal artery ligation through video-as

Hemothorax as a complication of subclavian vein cannulation with haemodialysis catheter - case report.

We present the case of a 39 year-old male patient admitted to ICU with symptoms of acute metabolic acidosis. He was investigated for the presence of methanol and glycol. Conservative treatment was initially started, followed by haemodialysis. During insertion of a temporary haemodialysis catheter in a location of Haapaniemi and Slatis, the patient was conscious but restless; therefore sedation was required to continue the procedure. After three hours of haemodialysis, the patient's general condition suddenly deteriorated. Hypovolemic shock and acute respiratory distress led to hypothesis of ri
