The use of hormone replacement therapy; results of a community survey.

Hormone replacement therapy is used for both menopausal symptoms and in prevention, but for the latter to be effective there may be a need to promote its use. Suitable strategies need to be informed by current practice. A postal questionnaire was therefore sent to 1649 women aged 20-69 years in Stockton-on-Tees to assess which women consider and take hormone replacement therapy. The response rate was 74%. Therapy had been considered by 346 (28%) women of whom 164 (47%) were premenopausal. It was taken by 20% of women aged 45-65 years. Users were more likely to have taken the contraceptive pill

Compliance with long-term oral hormonal replacement therapy.

OBJECTIVE: To study long-term compliance with oral hormonal replacement therapy (HRT). METHODS: A total of 151 early postmenopausal women were initially randomly allocated to three groups in a double-blind, placebo controlled trial. Fifty women received continuous combined therapy, another 50 were placed on sequential therapy, while the remaining 51 women were given placebo for 2 years with no subsequent therapy. After 2 years of the trial, the women were followed in an open investigation for a total of 8 years. RESULTS: After 8 years, 112 (85%) women were interviewed. None of the women in t

[Ocular complications of hormonal treatments: oral contraception and menopausal hormonal replacement therapy].

Numerous reports have described ocular complications of sex hormone preparations, particularly after the appearance of the oral contraceptive pill. The most serious complications are vascular occlusions such as central retinal vein or artery occlusion and acute ischemic optic neuropathy. In these cases, permanent visual loss may occur. Combined oral contraceptives have been reported to increase the incidence of these complications but it remains very low. It seems to lessen with the decrease in the estrogen dosage and the use of third-generation progestins. Conversely, post-menopausal hormone

Hormone replacement therapy in gynecologic cancer: oncologic safety and alternative therapies.

Recent advances in surgery and new therapies have greatly improved outcomes for gynecologic cancers, leading to more long-term survivors and a stronger focus on quality of life. Treatments such as surgery, radiation, and chemotherapy often cause iatrogenic menopause, leading to reduced quality of life and long-term health consequences. Hormone replacement therapy is the most effective option for managing menopausal symptoms. However, its use in patients with gynecologic cancer remains controversial because of concerns regarding oncologic safety. We conducted a comprehensive literature review e

Cardiac functions in children with growth hormone deficiency before and during growth hormone-replacement therapy.

Childhood growth hormone deficiency (GHD) decreases left-ventricular (LV) mass, but impairment of cardiac function has never been documented. The objective of this study was to assess the cardiac effects of GHD and recombinant human growth hormone (rhGH) treatment using conventional echocardiography and tissue Doppler imaging. Complete two-dimensional, M-mode, pulse-wave Doppler echocardiography and pulse-wave tissue Doppler imaging were performed in 12 children (6 male and 6 female patients) with GHD at baseline and at 5.86 ± 1.61 months after rhGH therapy. Recombinant human growth hormone tr

[Hormone replacement therapy and cancer of the breast. 1. Does replacement therapy increase the risk of cancer of the breast?].

It has been extensively demonstrated that hormonal replacement therapy (HRT) constitutes an effective treatment of menopausal symptoms. There is also substantial epidemiological evidence suggesting that this treatment protects women against cardiovascular disease and osteoporosis. The possible increase in breast cancer risk appears as its principal disadvantage and is often invoked as the reason why both doctors and patients decline this therapy. In this paper we review the current knowledge on HRT and breast cancer risk. A computerised bibliographical search (MEDLINE) of literature in the Eng

Is hormone replacement therapy cardioprotective? Decision-making after the heart and estrogen/progestin replacement study.

Coronary artery disease (CAD) is the major cause of death in women. Postmenopausal status is associated with a twofold increase of risk for cardiovascular events, in part because of detrimental changes in plasma lipoproteins and endothelial function. Hormone replacement therapy (HRT) has beneficial effects on plasma lipids, flow-mediated vasodilation and fibrinolysis that could be expected to have positive effects on the incidence of cardiovascular events in many postmenopausal women. However, oral HRT also has procoagulant effects and clearly increases the risk of venous thromboembolism. The

Associations of surgical menopause and hormone replacement therapy with meningioma development.

PURPOSE: Bilateral salpingo-oophorectomy (BSO) and hormone replacement therapy (HRT) exert opposing effects on systemic sex hormone exposure. Although HRT is frequently prescribed following BSO to mitigate surgical menopause, their combined impact on meningioma risk remains unclear. We evaluated meningioma incidence following BSO, HRT, or both. METHODS: Using the TriNetX database, we identified female patients for the following three categories: BSO only, HRT only, or BSO with subsequent HRT (BSO + HRT). Propensity score matching adjusted for demographics and meningioma risk factors. Outcomes

Patient-reported Impact of Menopause and Hormone Replacement Therapy on Psoriasis.

Menopause's impact on psoriasis remains unclear. This study's aim was to gather information on patient perception of the impact of menopause and hormone replacement therapy (HRT) on psoriasis. This survey-based study analysed 139 postmenopausal women with psoriasis from the USA and South Korea. In the combined cohort, most women reported menopause either had no effect on their psoriasis (41.7%) or worsened their psoriasis (33.1%). In the combined cohort, a majority of women (73.4%) reported that there was no change in their psoriasis treatment with menopause. Of the women receiving HRT (n = 29

Optimal Hormone Replacement Therapy in Hypothyroidism - A Model Predictive Control Approach.

In this paper, we address the problem of optimal thyroid hormone replacement strategy development for hypothyroid patients. This is challenging for the following reasons. First, it is difficult to determine the correct dosage leading to normalized serum thyroid hormone concentrations of a patient. Second, it remains unclear whether a levothyroxine L-T4) monotherapy or a liothyronine/levothyroxine (L-T3/L-T4) combined therapy is more suitable to treat hypothyroidism. Third, the optimal intake frequency of L-T3/L-T4 is unclear. We address these issues by extending a mathematical model of the pit

Growth hormone deficiency and replacement therapy in adults: Impact on survival.

In a seminal paper from 1990, Rosen and Bengtsson suggested that hypopituitary patients with a presumed growth hormone (GH) deficiency (GHD) have an excess mortality. Later studies have confirmed this finding but have also shown that the cause of the increased risk of death in these patients is multifactorial, including unreplaced GHD as well as non-physiological replacement therapy of other deficiencies, the etiology of hypopituitarism, and the side effects of tumor treatment. Only a few studies have investigated mortality in hypopituitary patients with GHD receiving GH replacement therapy (G

[Hormone replacement therapy in prostate cancer: side effects and their management].

Hormone replacement therapy in prostate cancer is androgen deprivation. This deprivation causes side effects that are well known. The most frequent complications are hot flashes, erectile dysfunction and libido disorders. These complications have a strong impact on the quality of life of the patient. The other side effects, namely metabolic syndrome and osteopenia, are less frequent, but have a worse prognosis due to their significant impact on patients' survival. The metabolic syndrome results in dyslipidaemia and insulin resistance. Patients under hormone therapy must have an initial evaluat

Effectiveness of compounded bioidentical hormone replacement therapy: an observational cohort study.

BACKGROUND: Bioidentical Hormone Replacement Therapy (BHRT) is believed it to be a safer and equally effective alternative to Conventional Hormone Therapy for the relief of menopausal symptoms; however, data are needed to support these claims. The objective of this study is to evaluate the effectiveness of compounded BHRT provided in six community pharmacies. METHODS: This was an observational cohort study of women between the ages of 18-89 who received a compounded BHRT product from January 1, 2003 to April 30, 2010 in six community pharmacies. Data included patient demographics, comorbiditi

Pyogenic granuloma occurring in a postmenopausal woman on hormone replacement therapy.

Pyogenic granuloma is a benign nodular lesion occurring most commonly on the gingiva of females during periods of elevated sex hormones such as puberty and pregnancy. Possible molecular mechanisms responsible for the appearance of pyogenic granuloma in this demographic have been suggested. Increased incidence of pyogenic granuloma in post menopausal women on hormone replacement therapy has not been reported. A 49-year-old woman with preexisting titanium implant placement in the left posterior mandible presented with complaint of food impaction and slight discomfort associated with the implant.

Current state of hormone replacement therapy: the case for using trimegestone.

Estrogen deficiency has a negative impact on the quality of life of postmenopausal women and is associated with vasomotor symptoms, insomnia and emotional lability. Other manifestations of estrogen deficiency include dry skin, dry vagina and dyspareunia, in addition to bone loss. Estrogen replacement effectively reverses these changes. The only indication for the administration of a progestogen is to protect the postmenopausal uterus against the potential development of endometrial hyperplasia and carcinoma.

Hormone replacement therapy to maintain cognitive function in women with dementia.

BACKGROUND: As estrogens have been shown to have several potentially beneficial effects on the central nervous system, it is biologically plausible that maintaining high levels of estrogens in postmenopausal women by means of estrogen replacement therapy (ERT) could be protective against cognitive decline and the development of Alzheimer's disease (AD) or other dementia syndromes. OBJECTIVES: To investigate the effects of ERT (estrogens only) or HRT (estrogens combined with a progestagen) compared with placebo in randomized controlled trials (RCTs) on cognitive function of postmenopausal wome

Cognitive effects of 10 years of hormone-replacement therapy with tibolone.

In an open study, self-ratings of bodily symptoms, mood (before and after stress), and cognitive performance were investigated in 25 women (aged 54-66 years) who for approximately 10 years had been taking an oral preparation of hormone replacement therapy (HRT), tibolone (Livial; 2.5 mg/ day). Tibolone has a unique profile, with estrogenic, progestogenic, and androgenic actions. The control group of 25 women had never taken HRT. Each woman in this group was pair-matched to one in the tibolone group on age, years since menopause, IQ, years of secondary education, and occupation. The groups were

Hormone replacement therapy and associated risk of stroke in postmenopausal women.

BACKGROUND: There is little information about the risk of stroke in relation to time since initiation of hormone therapy and in relation to estrogen dose. METHODS: We conducted a population-based case-control study at Group Health Cooperative (GHC), a health maintenance organization in the greater Seattle (Wash) area, to assess the association of hormone replacement therapy with the risks of incident ischemic and hemorrhagic stroke. Cases were all postmenopausal women with incident stroke at GHC during July 1989 through December 1998 (726 ischemic strokes and 213 hemorrhagic strokes). Control
