Administration of tranexamic acid for victims of severe trauma within pre-hospital care ambulance services (PHCAS) in Malaysia.

INTRODUCTION: Trauma is a Global threat and the 5th highest cause of all-cause mortality in Malaysia caused predominantly due to road traffic accidents. Majority of trauma victims are young adults aged between 21-40 years old. In Malaysia, 24 out of 100,000 population die annually due to trauma, rating us amongst the highest in South East Asia. These alarming figures justify aggressive preventive and mitigation strategies. The aim of this paper is to promote the implementation of evidence-based interventions that will reduce the rate of preventable death because of trauma. Tranexamic acid is o

Participation in Pulmonary Rehabilitation by Veterans Health Administration and Medicare Beneficiaries After Hospitalization for Chronic Obstructive Pulmonary Disease.

BACKGROUND: Hospitalization with acute exacerbation of chronic obstructive pulmonary disease (COPD) is common and costly to the health care system. Pulmonary rehabilitation (PR) can improve symptom burden and morbidity associated with COPD. The use of PR among Medicare beneficiaries is poor, and the use by Veterans Health Administration (VHA) beneficiaries is unknown. We sought to determine whether participation in PR was similarly poor among eligible veterans compared with Medicare beneficiaries. METHODS: We performed a retrospective study using national VHA and Medicare data to determine th

Early Hospital Readmissions and Mortality in Patients With Decompensated Cirrhosis Enrolled in a Large National Health Insurance Administrative Database.

BACKGROUND: Patients with decompensated cirrhosis have high rates of morbidity and mortality and frequently require hospital admission. Few studies have examined early readmission as an indicator of 90 day and overall mortality. Analysis of large databases is needed to evaluate the association between early readmission and mortality in decompensated cirrhosis. METHODS: We analyzed 5 years of private, employer-based, health insurance claims data associated with HealthCare Services Corporation on 13.5 million members over 4 states from 2010 to 2014. We defined early readmission as an admission

Drug safety of macrolide and quinolone antibiotics in a tertiary care hospital: administration of interacting co-medication and QT prolongation.

PURPOSE: Some macrolide and quinolone antibiotics (MQABs) are associated with QT prolongation and life-threatening torsade de pointes (TdP) arrhythmia. MQAB may also inhibit cytochrome P450 isoenzymes and thereby cause pharmacokinetic drug interactions (DDIs). There is limited data on the frequency and management of such risks in clinical practice. We aimed to quantify co-administration of MQAB with interacting drugs and associated adverse drug reactions. METHODS: We conducted an observational study within our pharmacoepidemiological database derived from electronic medical records of a terti

The need to immobilise the cervical spine during cardiopulmonary resuscitation and electric shock administration in out-of-hospital cardiac arrest.

In cases of out-of hospital cardiac arrest (OHCA), falling to the ground can cause brain and neck trauma to the patient. We present a case of a man in his mid-60s who suffered from an OHCA resulting in a violent collapse. The patient received immediate cardiopulmonary resuscitation, but his spine was immobilised only after a large frontal haematoma was found. The resuscitation efforts resulted in return of spontaneous circulation and discharge from hospital. After this, doctors performed angioplasty, followed by a cardiopulmonary bypass. Later, CT scan examination reported a displaced and unst

A multicenter study of the coding accuracy of hospital discharge administrative data for patients admitted to cardiac care units in Ontario.

BACKGROUND: Cardiac health services researchers frequently use cohorts derived from administrative hospital discharge abstract data to study the outcomes and treatment of coronary artery disease. However, relatively limited data exist on the accuracy of the coding of cardiac diagnoses in discharge abstract data. The goal of this study was to examine the accuracy of the coding of acute myocardial infarction and other cardiac diagnoses in the Canadian Institute of Health Information hospital discharge abstracts. METHODS: Patients admitted to 58 cardiac care units (CCUs) in Ontario that particip

Hospitalization at home use analysis for immune checkpoint inhibitors administrations in France: Care pathways, costs and development opportunities.

INTRODUCTION: The number of patients treated with immune checkpoint inhibitors (ICIs) has increased over time. Hospital-at-home (HAH) administration has been described in international studies as a model that may enhance patients' quality of life and relieve hospital capacity. The extent to which it has been adopted in France remains unknown. This study aimed to describe HAH use for ICI administration in France in 2021-2022, characterize care pathways, and assess reimbursed costs using national claims data. METHODS: Using French national hospital databases, this retrospective observational st

Assessment of Early Antibiotic Administration for Patients With Sepsis in the Emergency Department: A Retrospective Observational Study at a Tertiary Care Hospital.

Background Sepsis is a severe medical illness brought on by the body's exaggerated response to an infection. It develops when the body's reaction to the infection results in extensive inflammation, which may lead to the potential risk of organ failure or damage. Sepsis is an acute health crisis that requires immediate treatment, inevitably in a hospital with antibiotics and supportive care. Early administration of broad-spectrum antibiotics is a cornerstone of sepsis management and is strongly recommended by the guidelines. Delays in antibiotic initiation are associated with worse clinical out

Impact of nurse empowerment for early adrenaline administration for in-hospital cardiac arrest resuscitation: A pre-post intervention study.

BACKGROUND: Timely administration of adrenaline during in-hospital cardiac arrest (IHCA) with non-shockable rhythms is recommended, yet delays are common when drug administration depends on physician authorisation. AIM: To evaluate whether empowering nurses to administer the first dose of adrenaline could reduce delays and improve outcomes. METHODS: In this study of prospectively collected registry data, the primary outcome of time to first dose of adrenaline in IHCA events with non-shockable rhythms (asystole/pulseless electrical activity) was compared between the pre-intervention phase (Se

Out-of-pocket expenditure for cataract surgery in rural China: a cross-sectional survey combined with hospital administrative data.

BACKGROUND: Cataract is the primary cause of blindness in China and has a low surgery uptake rate where financial factors are crucial determinants. We report updated data on the financial protection of patients with cataract surgery after the integration of urban and rural medical insurance. METHODS: A population-based survey and hospital administrative data were matched to analyze surgical and total out of pocket (OOP) costs and their burden relative to income; the proportion of total surgical costs paid OOP; and the breakdown of total surgical costs. Multivariate regressions, including key

Evaluating Hospital Resource Utilization for Hypoplastic Left Heart Syndrome Through Linkage of a Clinical Registry and an Administrative Database.

Resource utilization estimates for hypoplastic left heart syndrome (HLHS) in early childhood are lacking. We linked the National Pediatric Cardiology Quality Improvement (NPC-QIC) clinical registry to the Pediatric Health Information Systems (PHIS) database to estimate hospital resource utilization for a contemporary HLHS cohort. All hospitals with both PHIS and NPC-QIC data available were eligible for inclusion. Patients discharged from their Norwood procedure 10/1/2009-9/30/2015 were linked using indirect identifiers. Estimated cost and length-of-stay data for all inpatient episodes across t
