New concepts in intracranial compliance: pathophysiology, monitoring and clinical implications.

PURPOSE OF REVIEW: This review synthesizes recent advancements in understanding intracranial compliance (ICC) pathophysiology, explores novel monitoring techniques, and discusses their evolving clinical implications. We highlight how a shift from static intracranial pressure (ICP) thresholds to dynamic ICC assessment is transforming the management of acute brain injury. RECENT FINDINGS: ICC is the brain's ability to accommodate volume changes without significant ICP elevation, is a critical determinant of outcome in neurocritical care. The paradigm in ICC is evolving from a focus on absolute

Roles of skeletal muscle-derived exosomes in osteoporosis.

Bone maintains a relatively stable bone mass by balancing bone formation and resorption. The development of osteoporosis is closely associated with the disruption of this balance. Muscles and bones, integral components of the musculoskeletal system, are functionally interconnected, and the onset of osteoporosis is frequently linked to the decline in skeletal muscle function. Exosomes play a crucial role in facilitating chemical information exchange between muscles and bones. This study aims to elucidate the effects of skeletal muscle-derived exosomes on bone formation and resorption, investiga

Multimodal AI and tumour microenvironment integration predicts metastasis in cutaneous melanoma.

Accurate prognostication is essential to guide clinical management in localised cutaneous melanoma (CM), the form of skin cancer with the highest mortality. While the tumour microenvironment (TME) plays a key role in disease progression, current staging systems rely on limited tumour features and exclude key clinicopathological prognostic features. Here we show that MelanoMAP, a multimodal AI model integrating TME-derived digital biomarkers and clinicopathological features from over 3,500 histology slides, improves prognostication of localised CM. MelanoMAP achieved a C-index of 0.82, a 24% im

The evolving roles of pathologists in paragangliomas and pheochromocytomas.

Intra-adrenal (historically "pheochromocytoma") and extra-adrenal paragangliomas (PPGLs) represent a unique subset of non-epithelial neuroendocrine neoplasms that feature remarkable biological diversity with significant diagnostic complexity. Over the past decade, advances in molecular genetics, biomarker discovery, and integrative tumor classification have transformed our understanding of PPGL pathobiology and redefined the pathologist's role in their diagnosis and clinical management. This review outlines the current multidisciplinary approach to PPGLs, emphasizing practical diagnostic strat

Pituitary adenomas: biology, nomenclature and clinical classification.

An 'adenoma' is a benign neoplasm composed of epithelial tissue, and has been standard nomenclature for primary pituitary neoplasms. In 2022, the fifth edition of the WHO Classification of Endocrine Tumours and of Central Nervous System Tumours, renamed pituitary adenomas as neuroendocrine tumours (NETs), assigning an oncology label to pituitary invariably benign neoplasms. Multidisciplinary workshops convened by the Pituitary Society have questioned the process, validity, and merit of this arbitrary change, while addressing the adverse clinical implications of the proposed new nomenclature. Integrating additional factors into the TNM staging for cutaneous melanoma by machine learning.

BACKGROUND: Integrating additional factors into the TNM staging system is needed for more accurate risk classification and survival prediction for patients with cutaneous melanoma. In the present study, we introduce machine learning as a novel tool that incorporates additional prognostic factors to improve the current TNM staging system. METHODS AND FINDINGS: Cancer-specific survival data for cutaneous melanoma with at least a 5 years follow-up were extracted from the Surveillance, Epidemiology, and End Results Program of the National Cancer Institute and split into the training set (40,781 c

Whole-genome surveillance supports hazard profiling of Escherichia coli lineages in recycled water treatment systems.

UNLABELLED: The use of treated wastewater is increasingly important for sustainable water management under a changing climate, yet conventional monitoring based on Escherichia coli enumeration provides limited insight into strain diversity and associated public health hazards. Here, we applied longitudinal whole-genome sequencing (WGS) to 180 E. coli isolates collected across the treatment continuum of a recycled water facility, from influent to final effluent. Genomic analysis revealed extensive strain-level heterogeneity, comprising 88 sequence types across eight phylogroups, with greater di

Sustainability in Jordan's Municipal Solid Waste Management System: Reduction of Methane Emissions and Benefits to Public Health.

Municipal solid waste management (MSWM) in Jordan faces significant challenges. Jordan's primary waste management strategy relies heavily on final disposal, with up to 80% of all generated solid waste ultimately being landfilled. These unsustainable practices lead to environmental degradation and public health risks. Increased methane emissions have harmed air quality: methane can lead to the formation of ground-level ozone, which in turn causes respiratory problems including asthma and pneumonia. The main objective of this study is to evaluate the impact of improved solid waste management str

Assessment and Protection of Cerebral Function in Patients with Acute Liver Failure.

Hepatic encephalopathy (HE) is a frequent and life-threatening neurological complication of acute liver failure (ALF) encountered in the intensive care unit and remains a major determinant of short-term mortality and long-term neurological outcomes. Increasing evidence indicates that HE in ALF is primarily driven by hyperammonemia, with synergistic contributions from systemic inflammation, cerebral hemodynamic dysregulation, metabolic failure, and osmotic imbalance. These interacting mechanisms promote astrocytic swelling, disruption of the blood-brain barrier, cerebral edema, and intracranial

AI-driven framework for accurate detection of Alzheimer's disease in EEG.

Alzheimer's Disease (AD) is a rapidly growing neurodegenerative disorder that severely impairs cognitive function, particularly among older adults. Early detection is critical for timely intervention and effective management. While electroencephalography (EEG) provides a non-invasive, cost-effective tool with high temporal resolution for diagnosing Alzheimer's Disease (AD), traditional EEG-based approaches often struggle to extract informative features accurately from complex brain signals, thus limiting their diagnostic performance. This study addresses these challenges by proposing a novel a

Acupuncture for patients with type 2 diabetes mellitus with dry eye: protocol for a systematic review and meta-analysis.

INTRODUCTION: The global incidence of patients with type 2 diabetes mellitus (T2DM) with dry eye is increasing annually, which imposes additional healthcare costs and financial burden on families and societies. In clinical practice, artificial tears are often used for symptomatic treatment, but these can only relieve the symptoms of discomfort. Acupuncture is a widely used alternative therapy. Indeed, randomised trials have found that acupuncture confers a definite therapeutic effect on patients with T2DM with dry eye. However, systematic reviews on the effectiveness and safety of acupuncture

Disease modification in advanced Parkinson's disease: a review and roadmap for paving the way for next-generation interventions.

Parkinson's disease (PD) exhibits highly heterogeneous clinical trajectories, yet "advanced PD" (aPD) lacks a standardized definition. Current reliance on clinical milestones (e.g., motor fluctuations, cognitive decline) is limited by non-linear progression and the absence of objective measures. Although biomarkers like aggregated α-synuclein, MRI, and PET are under investigation, their correlation with clinical progression remains modest. Robust, reproducible endpoints are urgently needed to evaluate disease-modifying therapies across diverse phenotypes, accounting for genetic background, age
