Multimodal alexia: neuropsychological mechanisms and implications for treatment.

Letter-by-letter (LBL) reading is the phenomenon whereby individuals with acquired alexia decode words by sequential identification of component letters. In cases where letter recognition or letter naming is impaired, however, a LBL reading approach is obviated, resulting in a nearly complete inability to read, or global alexia. In some such cases, a treatment strategy wherein letter tracing is used to provide tactile and/or kinesthetic input has resulted in improved letter identification. In this study, a kinesthetic treatment approach was implemented with an individual who presented with sev

Virtual reality and haptics as an assessment device in the postacute phase after stroke.

Virtual reality (VR) technology is altering the health care environment and is changing the options that are available to therapists. This study describes how a haptic device was used as a cinematic assessment utility. Three chronic stroke inpatients at Sahlgrenska University Hospital with left hemisphere damage were assessed. The patients were administered by the box and block manual dexterity test. For comparisons, a reference group was added to the study. Several parameters, including time, speed, and movement of the right upper extremity, were extracted and evaluated. The results indicate

Brief report: postural reactivity to fast visual motion differentiates autistic from children with Asperger syndrome.

The aim of the present study was to search for a sensorimotor marker (i.e., visuopostural tuning) that could be correlated with the severity of motor impairments in children with autistic spectrum disorders. Given that autistic children were previously reported to be posturally hyporeactive to visually perceived environmental motion in comparison with normal control children (Gepner et al., 1995), we sought to determine whether children with Asperger syndrome (AS) would share the same postural hyporeactivity to visual motion. Three autistic children with mild to severe motor impairments, three

Timing of onset of afferent responses and of use of kinesthetic information for control of movement in normal and cerebellar-impaired subjects.

A coordinated triggering task requiring use of kinesthetic information was employed to assess the timing of use of kinesthetic information in normal subjects and patients with cerebellar dysfunction. Passive movements of varying velocity were imposed in the flexor direction about the metacarpophalangeal joint of the right index finger. Subjects attempted to depress a switch with their left thumb when the index finger moved past a specified angle that was learned during a training session. The velocities ranged from 10 degrees/s to 88 degrees/s in 2 degrees/s increments. After 200 trials, subje

Reflex myoclonus in olivopontocerebellar atrophy.

The presence of reflex myoclonus in response to touching and pin-pricking the wrist or stretching the fingers and to photic stimulation was assessed in 24 patients with a presumed diagnosis of olivopontocerebellar atrophy (OPCA) and in 30 age matched control subjects. Reflex myoclonus to soma-esthetic stimulation was found in 23 patients and in none of the controls. Photic myoclonus was present in 12 patients and in none of the controls. Electrophysiological study of the reflex myoclonus showed enhanced (> 10 microV) somatosensory evoked potentials and an associated reflex electromyographic di

The utility of testing tactile perception of direction of scratch as a sensitive clinical sign of posterior column dysfunction in spinal cord disorders.

Classical beliefs about the functions of the dorsal columns of the spinal cord have been attacked following recent evidence that position and vibration sensations may be carried in the dorsal spinocerebellar tracts. There is evidence that the one specific function of the dorsal columns is for the transmission of information concerning the direction of tactile cutaneous movement. Thirty normal controls, 43 patients with spinal cord disorders and 10 patients with functional disorders were examined prospectively using an easily administered "direction of scratch" protocol. Interpretation of the d

The role of cutaneous afferents in position sense, kinaesthesia, and motor function of the hand.

In reconstructive surgery and rehabilitation work, for decades it has been a basic problem for the author to discover which of the peripheral receptor systems provide the primary information on position, motion and degree of load (proprioception). At one time, joint receptors were accepted as the dominant source and more recently musculocutaneous receptors, whereas little attention has been given to cutaneous factors. A number of clinical observations, however, have indicated the importance of cutaneous afferents and this prompted the present study. There are no means of isolating the joint re

The Acute Effects of Blood Flow Restriction on Ankle Muscle Reaction Time and Proprioception in Healthy Individuals.

Blood flow restriction (BFR) induces hypoxic and metabolic stress, which may alter afferent feedback and neuromuscular control. However, its acute effects on ankle sensorimotor function remain unclear. The aim of the study was to investigate the acute effects of lower-limb BFR on multidimensional ankle sensorimotor function in healthy adults. Twenty-four participants (12 females, 12 males) completed two conditions in randomized order and a crossover design: BFR at 60% arterial occlusion pressure (AOP) and a control condition (20 mmHg). All measurements were performed during occlusion. Outcomes

Effects of illusory kinesthesia by tendon vibratory stimulation on the postoperative neural activities of distal radius fracture patients.

Activation of motor-related regions with illusory kinesthesia had been suspected to contribute toward pain alleviation. We used electroencephalography (EEG) to assess the sensorimotor cortex activation during illusory kinesthesia by vibratory tendon stimulation in patients who were in pain. We investigated the relationship between the sensorimotor cortex and postoperative pain to clarify the analgesia mechanism. Patients who had undergone distal radius fracture surgery were studied. A hand massager provided the vibration stimulus for the illusory kinesthesia on the extensor digitorum muscle at

Role of subjective visual vertical test during eccentric rotation in the recovery phase of vestibular neuritis.

OBJECTIVES: By assessing unilateral utricular function at the acute and subacute stages, we sought to determine the ability of the subjective visual vertical (SVV) during eccentric rotation (dynamic SVV) in detecting latent abnormality of otolith dysfunction in unilateral vestibular neuritis. In addition, we compared the validity of the dynamic SVV with static SVV during the compensated recovery phase. METHODS: The static SVV and dynamic SVV of 19 patients diagnosed with acute unilateral vestibular neuritis and 31 patients diagnosed with the subacute stage of vestibular neuritis were determin

Magnitude estimation of softness.

The human capacity to estimate the magnitude of softness of silicone rubber disks of differing compliance was studied under experimental conditions that altered the mode of contact. Subjects were able to scale softness regardless of whether they (1) actively indented each specimen by tapping or pressing it with the finger pad, (2) received passive indentation of the finger pad by each specimen via a force controlled tactile stimulator, thus eliminating kinesthetic cues, or (3) actively indented each specimen with a stylus that was manipulated either by tapping with one finger, or held by two f

Neuronal responses to passive movement in the globus pallidus internus in primary dystonia.

Abnormal sensory processing has been implicated in the pathophysiology of primary dystonia. In the globus pallidus internus (GPi), the primary output structure of the basal ganglia, many neurons respond to sensory (proprioceptive) stimulation. Here we have characterized GPi neuronal responses to passive movement of the contralateral limbs in 22 patients with primary dystonia undergoing microelectrode recording for placement of deep brain stimulator leads. We plotted coordinates of cells responding to limb movement in a common space. We observed distinct representations of leg and arm movement

Cervicocephalic kinesthetic sensibility in young and middle-aged adults with or without a history of mild neck pain.

Previous research has shown that there is no significant relationship between the degree of structural degeneration of the cervical spine and neck pain. We therefore sought to investigate the potential role of sensory dysfunction in chronic neck pain. Cervicocephalic kinesthetic sensibility, expressed by how accurately an individual can reposition the head, was studied in three groups of individuals, a control group of 20 asymptomatic young adults and two groups of middle-aged adults (20 subjects in each group) with or without a history of mild neck pain. An ultrasound-based three-dimensional

[Experimental studies of motion sickness on board of a research ship].

To date the incidence and treatment of motion sickness have not been satisfactorily elucidated. Discrepancies among individuals in tolerating strong velocities and accelerations are well-known. To prevent motion sickness evaluations of individual predispositions are important. There are several current theories on the nature of motion sickness. These include the mismatching of sensory inputs, a Coriolis force, non-physiological stimuli and modulation of vestibuloocular reflexes (VOR). The main influence of the vestibular organ in generating kinetosis is obvious. Analysis of the chief movements

Accuracy of weightbearing estimation by stroke versus healthy subjects.

This study was performed to describe and compare the accuracy of weightbearing at three target levels (25, 50, and 75% of body weight) of 14 ambulatory stroke subjects and 14 matched healthy subjects. Weightbearing through a designated lower extremity (stroke subjects-paretic, healthy subjects-randomly selected) was measured with digital scales. No significant difference was found in magnitude of weightbearing (%) between the stroke and healthy groups. Although making significantly greater errors in weightbearing than healthy subjects, stroke subjects did not consistently weightbear under targ

Acrylic cement stabilized joint replacements.

Surgical management of osteoarthritis, aseptic necrosis and rheumatoid arthritis has been revolutionized by the introduction of acrylic cement-stabilized joint surface replacement. Although single joint surface replacements have been employed extensively for more than half a century, total surface replacement operations with a wear-resistant high-density polyethylene and noncorrosive stainless steel stabilized by acrylic cement were introduced only a little more than 12 years ago. This evolved with Charnley's discovery of the high level of bone tolerance for acrylic cement. Acrylic cement made

Impact of lower limb proprioception on balance and gait in hemiparetic patients: biomechanical analysis.

BACKGROUND: Sensory and proprioceptive dysfunction in the lower extremities occurs in up to 89% of hemiparetic stroke patients and contributes to gait abnormalities, postural instability, and increased fall risk. However, the specific relationships between objectively measured proprioceptive deficits and quantitative balance/gait parameters remain incompletely characterized. This study investigated the associations between lower extremity proprioceptive impairments (foot sole sensation, position sense, and kinesthesia) and objective measures of balance and gait in hemiparetic individuals.
