Limbus-Sparing Congenital Central Corneal Dermoid: A Rare Developmental Anomaly.

The authors report a rare case of a limbus-sparing central corneal dermoid in a 1-year-old boy, contributing to the limited existing literature of only three such documented cases worldwide. This case report describes the clinical presentation, surgical management, and histopathologic characteristics of this unique developmental anomaly. A comprehensive clinical evaluation was conducted, including ophthalmic examination, B-scan ultrasonography, and surgical intervention. The dermoid was excised along with penetrating keratoplasty, with the excised specimen undergoing detailed histopathologic a

Presence of Protease Inhibitor 9 and Granzyme B in Healthy and Pathological Human Corneas.

The aim of this study was to find out whether protease inhibitor 9 (PI-9) and granzyme B (GrB) molecules that contribute to immune response and the immunological privilege of various tissues are expressed in healthy and pathological human corneas. Using cryosections, cell imprints of control corneoscleral discs, we showed that PI-9 was expressed particularly in the endothelium, the superficial and suprabasal epithelium of healthy corneas, limbus, and conjunctiva. GrB was localized in healthy corneal and conjunctival epithelium, while the endothelium showed weak immunostaining. The expression o

Peripheral Cornea Crosslinking Before Deep Anterior Lamellar Keratoplasty.

Since Cornea crosslinking (CXL) has been proven to halt progression and biomechanically stabilize keratoconus, we hypothesized that CXL of the corneal periphery 3 months prior to corneal transplantation can reduce the incidence of recurrent ectasia by strengthening the peripheral corneal tissue and causing apoptosis of diseased peripheral host keratocytes. Thus, the aim of this case-report was to propose a novel peripheral CXL technique prior to keratoplasty and evaluate its safety. A 22-year-old woman was admitted with advanced right keratoconus and corrected distance visual acuities of 20/30

A Quantitative Measurement of Ocular Rotation in Small Incision Lenticule Extraction: Double Marking of Corneal Limbus and Sclera.

The small incision lenticule extraction (SMILE) has become a mainstream surgical procedure for correcting myopia and astigmatism. Nevertheless, the lack of an active cyclotorsion-tracking system prevents it from correcting rotational errors, which may compromise astigmatic correction accuracy. Evidence regarding the influence of intraoperative cyclotorsion on SMILE outcomes remains scarce, largely because of the unavailability of viable and dependable methods of measuring the degree of rotation during an operation. To address this gap, this protocol describes a novel corneoscleral double-marki

Quantifying the Size of the Pterygium Head by Evaluating the Fibrous Area or the Vascularized Area.

PURPOSE: Pterygium is characterized by the development of wing-shaped fibrovascular tissue from the bulbar conjunctiva across the limbus onto the cornea. The purpose of this study was to quantify the discrepancy of the pterygium size between the fibrous area and the vascularized area. METHODS: Color photographs of 146 eyes with pterygia obtained with a single-lens reflex camera system were analyzed by 2 independent graders for width (vertical dimension), length (horizontal dimension), and surface area of the pterygium head using 2 methods: the fibrous method, which measures any fibrovascular

Optical Analysis and Reappraisal of the Peripheral Light Focusing Theory of Nasal Pterygia Formation.

PURPOSE: Pterygia are much more common nasally than temporally. Ultraviolet (UV) radiation is a major risk factor. Coroneo proposed that the nasal preference is caused by the "peripheral light focusing effect," (PLF), in which UV at an oblique angle passes through temporal cornea and is concentrated on and damages nasal limbal stem cells. This study evaluates whether the PLF is sufficient to explain the nasal preference. METHODS: Whereas Coroneo and colleagues derived the maximum PLF intensity gain (UV concentration factor) as a function of incident angle (i.e., different nasal limbal positio

Novel anti(lymph)angiogenic treatment strategies for corneal and ocular surface diseases.

The cornea is one of the few tissues which actively maintain an avascular state, i.e. the absence of blood and lymphatic vessels (corneal [lymph]angiogenic privilege). Nonetheless do several diseases interfere with this privilege and cause pathologic corneal hem- and lymphangiogenesis. The ingrowths of pathologic blood and lymphatic vessels into the cornea not only reduce transparency and thereby visual acuity up to blindness, but also significantly increases the rate of graft rejections after subsequent corneal transplantation. Therefore great interest exists in new strategies to target patho

Punctate keratitis induced by subconjunctivally injected microfilariae of Onchocerca lienalis.

An experimental model of human onchocercal keratitis was produced by injecting guinea pigs subconjunctivally with microfilariae (Mf) of Onchocerca lienalis. Actively motile O lienalis Mf spontaneously penetrated the central cornea and produced gray-white midstromal opacities, 0.2 to 0.4 mm in diameter, resembling those of human onchocercal punctate keratitis. Histologically, small foci of eosinophil and mononuclear cell infiltration associated with small pockets of interstitial edema were present in the central corneal stroma, with eosinophil infiltrates in the conjunctiva, episclera, limbus,

Is refractive surgery justified?

Refractive surgery may be defined as any kind of eye surgery in which attempt is made to produce a level of spectacle error that is accepted to the patient. Cataract surgery with lens implantation is the most often done refractive surgery on the eye. The following kinds of refractive surgery are being practised: With normal crystalline lens, phacic intra-ocular lens, surgery with cornea, using excimer laser, other newer techniques. Radial keratotomy is one procedure of refractive surgery with cornea. In this operation, radial cuts are made on the cornea starting near the edge of the pupil and

[Reticulated polyethylene oxide for gel injection adjustable keratoplasty. Biocompatibility in critical situation].

BACKGROUND: GIAK is a surgical technique developed for the correction of myopia. We evaluated the biocompatibility of a gamma-ray crosslinked poly(ethylene oxide) gel in two critical situations. MATERIALS AND METHODS: In 3 rabbits, a 5 mm inner diameter annular channel of 1 mm width was delaminated at 80% corneal depth and a sterile gamma-ray crosslinked polyethylene oxide gel was injected in the channel. In cornea 1 (control), the annulus was centered on the corneal apex and the channel was fully filled with gel. In cornea 2, the delamination was purposely decentered and the channel was appr

An essential role for dendritic cells in vernal keratoconjunctivitis: analysis by laser scanning confocal microscopy.

BACKGROUND: CD4+ T helper type 2 cells play a central role in the pathogenesis of vernal keratoconjunctivitis (VKC), and antigen-presenting cells are required for the cell activation. In this study, we aimed to survey the density, distribution, and morphology of dendritic cells (DCs) in patients with VKC by in vivo confocal microscopy. METHODS: Thirty-five patients (mean, 12.4 ± 5.3 years) affected by VKC were included. All patients were treated with 0.1% fluorometholone eye drops and 0.5% cyclosporine A eye drops. The density and morphological and distributional characteristics of DCs in eac

Extracellular Vesicles of Tears and Ocular Surface: An Enigma.

PURPOSE: A stable ocular surface is crucial for maintaining ocular health by protecting against various infections. This is achieved by coordinated function of ocular structures (cornea, limbus, conjunctiva), innervation, and the tear film which forms a protective barrier over the ocular surface ensuring proper hydration, lubrication, and overall ocular comfort. This complex three-layered tear film secreted by different sources ensures its stability by adhesion to the corneal epithelium. Ocular surface fluid kinetics and tear secretion involve complex processes influenced by neural regulation,

Schwann Cells Are Key Regulators of Corneal Epithelial Renewal.

PURPOSE: Corneal sensory nerves protect the cornea from injury. They are also thought to stimulate limbal stem cells (LSCs) to produce transparent epithelial cells constantly, enabling vision. In other organs, Schwann cells (SCs) associated with tissue-innervating axon terminals mediate tissue regeneration. This study defines the critical role of the corneal axon-ensheathing SCs in homeostatic and regenerative corneal epithelial cell renewal. METHODS: SC localization in the cornea was determined by in situ hybridization and immunohistochemistry with SC markers. In vivo SC visualization and/or

MicroRNA and Protein Cargos of Human Limbal Epithelial Cell-Derived Exosomes and Their Regulatory Roles in Limbal Stromal Cells of Diabetic and Non-Diabetic Corneas.

Epithelial and stromal/mesenchymal limbal stem cells contribute to corneal homeostasis and cell renewal. Extracellular vesicles (EVs), including exosomes (Exos), can be paracrine mediators of intercellular communication. Previously, we described cargos and regulatory roles of limbal stromal cell (LSC)-derived Exos in non-diabetic (N) and diabetic (DM) limbal epithelial cells (LECs). Presently, we quantify the miRNA and proteome profiles of human LEC-derived Exos and their regulatory roles in N- and DM-LSC. We revealed some miRNA and protein differences in DM vs. N-LEC-derived Exos' cargos, inc

Eyes open on stem cells.

Recently, the murine cornea has reemerged as a robust stem cell (SC) model, allowing individual SC tracing in living animals. The cornea has pioneered seminal discoveries in SC biology and regenerative medicine, from the first corneal transplantation in 1905 to the identification of limbal SCs and their transplantation to successfully restore vision in the early 1990s. Recent experiments have exposed unexpected properties attributed to SCs and progenitors and revealed flexibility in the differentiation program and a key role for the SC niche. Here, we discuss the limbal SC model and its broade

E-PIX: An enhanced grading for acute ocular burns.

An enhanced online and manual grading system, based on the I's and E's, for acute ocular chemical injuries is being proposed. E-PIX is designed to be an online/manual grading system that includes all the parameters that adversely affect the outcome of acute chemical injuries. The importance of addressing the I's and E's in chemical burns cannot be underestimated. These include the need for the documentation and management of epithelial defect (E), intraocular pressure (P) (IOP), ischemia (scleral) (I), and exposure (X) (acronym - E-PIX). Epithelial defect includes that involving the limbus (L)
