Adjudicative competence.

PURPOSE OF REVIEW: Although the basic standards of adjudicative competence were specified by the US Supreme Court in 1960, there remain a number of complex conceptual and practical issues in interpreting and applying these standards. In this report we provide a brief overview regarding the general concept of adjudicative competence and its assessment, as well as some highlights of recent empirical studies on this topic. RECENT FINDINGS: Most adjudicative competence assessments are conducted by psychiatrists or psychologists. There are no universal certification requirements, but some states a

Scratching the surface: mental health training for rural health workers.

This paper will describe the Mental Health Training for Rural Health Workers Project that was completed in 1995 under the Rural Health Support Education and Training Program. The lack of specialist mental health services in rural areas means that generalist health workers need to provide mental health services in rural communities. Generalist health workers often feel ill-equipped to help people who require mental health services. The aim of this project was to provide generalist workers in rural areas with practical skills and knowledge in order to help people with mental health disorders and

Decision Support Framework for Quality Assurance and Enhancement of Therapeutic Artificial Intelligence Systems: Mixed Methods Pilot Study.

BACKGROUND: Therapeutic chatbots are increasingly deployed across digital mental health services, yet most evaluation efforts remain diagnostic rather than actionable. Organizations lack structured pathways to translate evaluation findings into validated quality improvements aligned with health care quality assurance requirements. OBJECTIVE: This study aims to introduce EvaluationPlus, a decision support framework that operationalizes a reproducible evaluation-to-enhancement loop for therapeutic artificial intelligence systems. We aimed to demonstrate its feasibility through expert-guided dia

Sexual orientation differences in treatment expectation, alliance, and outcome among patients at risk for suicide in a public psychiatric hospital.

BACKGROUND: Sexual minority (SM) individuals (gay, lesbian, bisexual, or otherwise nonheterosexual) are at increased risk for mental disorders and suicide and adequate mental healthcare may be life-saving. However, SM patients experience barriers in mental healthcare that have been attributed to the lack of SM-specific competencies and heterosexist attitudes and behaviors on the part of mental health professionals. Such barriers could have a negative impact on common treatment factors such as treatment expectancy or therapeutic alliance, culminating in poorer treatment outcomes for SM versus h

The adolescent outcome of hyperactive children: predictors of psychiatric, academic, social, and emotional adjustment.

OBJECTIVE: To investigate predictors of adolescent outcome in a large sample of hyperactive children. METHOD: 123 hyperactive children were followed prospectively over an 8-year period. Multiple linear and logistical regression equations were used to relate childhood predictor variables to adolescent academic, psychiatric, social, and emotional adjustment. RESULTS: Adolescent academic skills were related to childhood cognitive and academic competence while school conduct was predicted by other variables including early family stress. Duration of mental health treatment received often was neg

Non-clinical Psychosocial Mental Health Support Programmes for People with Diverse Language and Cultural Backgrounds: A Critical Rapid Review.

Low accessibility to mainstream psychosocial services disadvantages culturally and linguistically diverse (CALD) populations, resulting in delayed care and high rates of unsupported psychological distress. Non-clinical interventions may play an important role in improving accessibility to psychosocial support, but what characterises best practice in this space remains unclear. This critical rapid review addressed this gap by searching for, and critically analysing, existing research on non-clinical psychosocial support services, drawing from a critical realist framework and Brossard and Chandl

Insight and capacity to consent to electroconvulsive therapy.

OBJECTIVE: To critically examine a recent decision of the Victorian Supreme Court that found that the Mental Health Tribunal and the Victorian Civil and Administrative Tribunal erred in the application of the capacity test in the Mental Health Act 2014 (Vic) and that compulsory electroconvulsive therapy would infringe upon the human rights of two patients who had no insight into their chronic schizophrenia. CONCLUSIONS: After considering the concepts of insight and capacity to consent to treatment, the paper concludes that the decision in NJE and PBU v Mental Health Tribunal [2018] VSC 564 is

Diversion of Mentally Ill Offenders from the Criminal Justice System in Ireland: Comparison with England and Wales.

AIM: It is generally accepted that certain people who are mentally ill and have contact with the criminal justice system should be diverted to psychiatric care rather than imprisoned. We sought to comment on priorities relating to the development of diversion services in Ireland through comparison with developments in a neighbouring jurisdiction. METHODS: A comparative review was undertaken in relation to the provision for psychiatric diversion across the offender pathway in Ireland and England and Wales. This included legal and service related considerations. RESULTS: In both jurisdictions,

[Ethical questions in clinical research with the mentally ill].

The review deals with present problems of protecting mentally ill patients who are incompetent to give informed consent to participating in clinical research, and of assessment of the capacity to consent. 1. Clinical trials of drugs on efficacy and safety in incompetent patients are ethically justified and legally admissible if the investigational drug can be expected to exert a direct potential individual benefit and if such trials will be performed under defined criteria to protect these vulnerable patients. In Germany it is questionable how far these prescriptions of the German Drug Law (AM

Emotion Regulation in Sexually Abused Preschoolers.

Emotion regulation is closely related to mental health in children and adults. Low emotion regulation competencies have been found in school-aged sexually abused girls. The aim of the present study was to investigate emotion regulation competencies in sexually abused preschool girls and boys using a multi-informant approach. Emotion regulation was assessed in 62 sexually abused and 65 non-abused preschoolers using the Emotion Regulation Checklist and the MacArthur Story Stem Battery. Both parents and educators reported lower emotion regulation competencies in sexually abused preschoolers, espe

Stress, perceived competence and guilt as predictors of depression in parents with chronic pain.

INTRODUCTION: Suffering from chronic pain (CP) and coping with parenthood can be challenging for parental mental health. Pain can hinder the ability to deal with demands related to parenthood, which can negatively affect their psychological well-being because of unmet caregiving expectations. METHODS: Considering the limited amount of research regarding the mental health of parents with CP, the study's main aim was to test a predictive model based on previous scientific literature, using structural equation analysis, in which parental competence and parental guilt partially mediate the relati

Operationalizing Excellence in Psychedelic Care Through Advancing Competencies in Nurses: State-of-the Science Review.

As interest in psychedelic-assisted therapies grows ahead of regulatory standardization, mental health nursing stands at a critical clinical and professional inflection point. This state-of-the-science review examines psychedelic-assisted therapies through a mental health nursing lens and advocates for the formal recognition of psychedelic nursing as a specialty with defined competencies, training pathways, and professional standards. Emerging evidence from neurobiology, empirical data, policy, and patient experience literature was synthesized alongside nursing scope of practice, ethics, and m

Empowering mental health professionals in asynchronous online psychotherapy with GenAI.

Background Asynchronous online psychotherapy (AOP) offers a promising solution to improve access to mental healthcare, particularly in times of global crises. However, therapists practicing AOP face challenges such as emotional strain, isolation, and increased workloads. The integration of generative artificial intelligence (GenAI) may provide support, yet little is known about its impact on the empowerment of mental health professionals, specifically regarding their perceived competence and autonomy, and how it can be integrated into blended care environments. Methods A mixed-method study was

Implementation of a wound care education project to improve the wound care competency among psychiatric nurses: A quality improvement project and feasibility study.

WHAT IS KNOWN ON THE SUBJECT?: Compared to the general population, people with mental illness die several years prematurely. The prevalence of chronic physical illnesses such as diabetes and cardiovascular disease is higher in people with mental health illnesses, putting them at increased risk of developing skin wounds. The majority of the studies on wound care education and management were carried out in acute care and long-term care settings. A very few addressed the wound care needs for psychiatric patients and the educational needs of nurses in the mental health setting. WHAT THE PAPER ADD
