Ambulatory inhalation therapy in obstructive lung diseases.

The inhalation route is widely used for the treatment of obstructive lung diseases, because administered drugs have a high therapeutic index as they reach their target directly; this requires that they be of adequate size to penetrate the conducting airways, i.e. 2-5 microns. Pressurized inhalers were the first reliable metered-dose devices available, and are still the most frequently used. However, metered-dose inhaler (MDI) suspensions will have to be reformulated: they contain chlorofluorocarbons (CFCs), whose production has to be stopped within a few years, as they affect the stratosphere.

Comparative Safety and Effectiveness of Inhaled Corticosteroid and Long-Acting β2-Agonist Combinations in Patients With COPD.

BACKGROUND: The differential risk of pneumonia among inhaled corticosteroid (ICS) use in patients with COPD requires more investigation, especially regarding beclomethasone-containing inhalers. The goal of this study was to compare the risk and benefit profile of different ICS/long-acting β2-agonist (LABA) combinations in patients with COPD. METHODS: This retrospective cohort study was conducted by using national health insurance claims data from the years 2009 to 2015 in Taiwan and included patients with COPD with new ICS/LABA use. Propensity score matching and Cox regression models were use

Comparison of the effectiveness of inhaler devices in asthma and chronic obstructive airways disease: a systematic review of the literature.

BACKGROUND: Asthma and chronic obstructive pulmonary disease (COPD) are common diseases of the airways and lungs that have a major impact on the health of the population. The mainstay of treatment is by inhalation of medication to the site of the disease process. This can be achieved by a number of different device types, which have wide variations in costs to the health service. A number of different inhalation devices are available. The pressurised metered-dose inhaler (pMDI) is the most commonly used and cheapest device, which may also be used in conjunction with a spacer device. Newer chlo

Dry-powder inhaler alternatives for chronic obstructive pulmonary disease patients post total laryngectomy.

OBJECTIVES: Patients undergoing total laryngectomy who have chronic obstructive pulmonary disease are unable to use dry-powder inhalers due to the anatomical changes after surgery and are therefore at greater risk of post-operative pulmonary complications. In our practice, we identified a scarcity of resources available to help clinicians decide on a suitable alternative. Hence, we sought to provide a solution that would address this issue. METHODS: Within our head and neck centre, we worked with the respiratory team and specialist pharmacists to create a simple conversion table to help clini

Effect of low climate impact vs. high climate impact inhalers for patients with asthma and COPD-a nationwide cohort analysis.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) and asthma can be treated with inhaled corticosteroids (ICS) delivered by low climate impact inhalers (dry powder inhalers) or high climate impact inhalers (pressurized metered-dose inhalers containing potent greenhouse gasses). ICS delivered with greenhouse gasses is prescribed ubiquitously and frequent despite limited evidence of superior effect. Our aim was to examine the beneficial and harmful events of ICS delivered by low and high climate impact inhalers in patients with asthma and COPD. METHODS: Nationwide retrospective cohort st

What is new in asthma: new dry powder inhalers.

Asthma affects an estimated 4.8 million children. The pressurized metered-dose inhaler (pMDI), despite problems associated with its use and concern that most of these inhalers contain ozone-damaging chlorofluorocarbons (CFCs), is currently the device most frequently used to deliver inhaled medication. Concerns regarding pMDIs that contain CFCs have led to further development of alternative delivery devices, including CFC-free pMDIs, breath-actuated devices, and dry powder inhalers (DPIs). Advantages and disadvantages of these devices are discussed briefly, with emphasis on the new DPIs. A brie

[Translated article] Environmental impact of inhaled therapies in a cystic fibrosis unit: Strategies for sustainability.

OBJECTIVE: Inhaled therapy is essential in cystic fibrosis; however, inhalers have a significant environmental impact due to the greenhouse gases (GHGs) emitted. The environmental impact of a product is estimated by its carbon footprint (CF). Pressurized metered-dose inhalers (pMDIs) have a higher CF than dry powder inhalers (DPIs) and soft mist inhalers (SMIs) due to the incorporation of GHGs. The objectives are to analyze the consumption of inhalers (β2-adrenergic agonist bronchodilators, anticholinergics, and/or corticosteroids) in a cystic fibrosis unit and estimate the generated CF. Comparison of bronchoprotective and bronchodilator effects of a single dose of formoterol delivered by hydrofluoroalkane and chlorofluorocarbon aerosols and dry powder in a double blind, placebo-controlled, crossover study.

BACKGROUND: In response to the phasing out of chlorofluorocarbon (CFC) inhalers, a metered dose hydrofluoroalkane (HFA) formulation, Modulite (Chiesi Farmaceutici S.p.A, Parma, Italy), to be delivered with a pressurized metered dose inhaler (pMDI), has been developed. Modulite is a HFA formulation technology that has been designed to provide stable and uniform dose delivery of HFA-based formulations to enable an easy transition from CFC to HFA inhalers. OBJECTIVES: The aim of this study was to compare the bronchoprotective and bronchodilator effects of a single dose of 12 microg of formoterol

Efficacy and safety of different inhaler types for asthma and chronic obstructive pulmonary disease. a systematic review and meta-analysis.

Pressurised metered dose inhalers (pMDIs) contain propellant gases with high global warming potential yet remain a cornerstone of management for asthma and chronic obstructive pulmonary disease (COPD). The aim of this study was to determine whether non-propellant alternatives of dry powder inhalers (DPIs) and soft mist inhalers (SMIs) had similar efficacy and safety. A systematic review was performed finding 44 randomised trials (24,710 participants) and moderate certainty evidence for most outcomes. No statistically significant or clinically important differences were found between inhaler ty

Inhaler Technique and Adherence to Inhaled Medications among Patients with Acute Exacerbation of Chronic Obstructive Pulmonary Disease in Vietnam.

Sub-optimal chronic obstructive pulmonary disease (COPD) management has been found largely due to patients' medication non-adherence and incorrect inhaler technique. This study aimed to examine inhaler use technique and medication adherence among Vietnamese COPD patients as well as potential associated factors. A cross-sectional study involving 70 COPD exacerbators was conducted. Inhaler technique and adherence were evaluated by the 10-item and 12-item Test of Adherence to Inhaler (TAI). Data on the history of COPD, home prescription of inhalers and duration of hospitalization were also collec

Fifteen-minute consultation: Maintenance and reliever therapy for the management of asthma in children and young people.

Asthma is the most common chronic disease of childhood. Acute asthma attacks are a frequent reason for emergency presentation in children and young people (CYP), and fatal asthma attacks can occur even in those thought to have mild disease. Asthma treatment in the UK has until recently relied heavily on a strategy of regular 'maintenance' inhaled corticosteroid (ICS) and separate 'reliever' inhalers containing short-acting beta-agonist (SABA) taken as needed to relieve symptoms (most commonly relievers contain salbutamol). This strategy is vulnerable to poor adherence to maintenance treatment

Principles of aerosol therapy.

Aerosol therapy with a variety of drugs is superior to oral and, except perhaps in status asthmaticus, parenteral therapy. An understanding of aerosol physics and the physiologic characteristics relating to ventilation allows optimum aerosol delivery for maximum benefit in the majority of patients. In those patients who are unable to use metered-dose inhalers effectively, simple inhalation devices have been developed which assure breath-actuated aerosol delivery to the lower respiratory tract with minimal side effects.

New propellant-free technologies under investigation.

Interest in the use of solution inhalers and dry powder inhalers (DPIs) has increased in recent years, broadening from systems for the delivery of traditional respiratory medications to the delivery of inhaled proteins, peptides, and analgesics. This has led to numerous developments and a range of inhaler designs and technology, each with advantages and distinctive features. Novel devices for aerosolization of solutions can be broadly divided into three main classes: adaptive aerosol delivery devices, metered dose liquid inhalers (MDLIs), and breath-actuated nebulizers. Similarly, the wide ran

Chlorofluorocarbon-free aerosol therapy in patients with pulmonary airflow obstruction.

Following the recognition that the chlorofluorocarbon (CFC) propellants used in all earlier pressurized metered dose inhalers (pMDIs) damage the Earth's protective ozone layer in the stratosphere, alternative aerosol applicators have become mandatory. Therefore it is clear that prescribers have to move away from CFC-containing pMDIs. Today the ozone hole over the South Pole reaches about the size of Europe. This means for the physician to prescribe aerosol therapy completely free of CFCs, e.g. by DPIs and nebulizers or CFC-free pMDIs.

Challenges in changing to non-chlorofluorocarbon inhalers in the treatment of asthma.

The chlorofluorocarbon (CFC)-based metered dose inhaler, which has been the mainstay of the management of obstructive lung diseases, will soon be phased out world wide and replaced by CFC-free devices. Patients will have to be changed to the devices in a co-ordinated manner to avoid any risk to their health and safety. The different shapes and aerosol delivery characteristics of the new inhalers, as well as their distinctive taste, could add to the levels of poor drug use already experienced in asthma. From previous change scenarios in disease management, the potential for unstable asthma cont

Modulation of aerosol clouds produced by pressurised inhalation aerosols.

The inclusion of non-volatile components such as glycerol or polyethylene glycol in hydrofluoralkane (HFA) solution formulations for pressurised metered dose inhalers (pMDIs), greatly increases the particle size of the aerosol. Cloud characteristics can be further modulated by permuting this factor with the choice of propellant and the dimensions of the actuator, to give a chosen fine particle dose and particle diameter. This principle has been used to design solutions which closely match the performance of chlorofluorocarbon based suspension formulations containing beclomethasone dipropionate

Demystifying Dry Powder Inhaler Resistance with Relevance to Optimal Patient Care.

The selection of an inhaler device is a key component of respiratory disease management. However, there is a lack of clarity surrounding inhaler resistance and how it impacts inhaler selection. The most common inhaler types are dry powder inhalers (DPIs) that have internal resistance and pressurised metered dose inhalers (pMDIs) that use propellants to deliver the drug dose to the airways. Inhaler resistance varies across the DPIs available on the market, depending largely on the design geometry of the device but also partially on formulation parameters. Factors influencing inhaler choice incl

Salmeterol inhaler using a non-chlorinated propellant, HFA134a: systemic pharmacodynamic activity in healthy volunteers.

BACKGROUND: Metered dose inhalers for the treatment of asthma use chlorofluorocarbons as propellants. These face an international ban due to their effect on the ozone layer. Salmeterol has been reformulated using the non-chlorinated propellant Glaxo inhalation grade HFA134a. METHODS: The safety, tolerability and systemic pharmacodynamic activity of the salmeterol/HFA134a inhaler, the current salmeterol inhaler, and placebo (HFA134a) were compared in 12 healthy volunteers in a double blind, randomised crossover study using a cumulative dosing design. How to match the optimal currently available inhaler device to an individual child with asthma or recurrent wheeze.

Inhaled medications are the cornerstone of treatment in early childhood wheezing and paediatric asthma. A match between patient and device and a correct inhalation technique are crucial for good asthma control. The aim of this paper is to propose an inhaler strategy that will facilitate an inhaler choice most likely to benefit different groups of children. The main focus will be on pressurised metered dose inhalers and dry powder inhalers. In this paper we will discuss (1) practical difficulties with the devices and with inhaled therapy and (2) the optimal location for deposition of medicines
