Improving calf muscle strength in patients with spastic equinovarus deformity by transfer of the long toe flexors to the Os calcis.

The split tibialis anterior tendon transfer (SPLATT), Achilles tendon lengthening, and toe flexor release are proven and effective procedures for correcting a spastic equinovarus deformity of the foot. Paresis is a prominent feature of upper motoneuron syndrome. Lengthening the Achilles tendon, although necessary to correct the equinus, further weakens the gastrocnemius-soleus muscle group. The calf paresis commonly results in the need for an ankle-foot orthosis (AFO) during ambulation. Previous studies have shown that despite the correction of the equinovarus deformity, only one third of pati

Intra-rater reliability, standard error of measurement, and minimal detectable change of two muscle compressibility measurement methods using ultrasonography in spastic hand flexor muscles.

PURPOSE: Calculate the intra-rater reliability of measuring muscle compressibility for spastic hand flexors using ultrasonography in two different methods among post-stroke patients. METHODS AND MATERIALS: Twenty-four patients with a mean age of 54.50 ± 9.44 participated in this study. Muscle compressibility was measured using two methods in the middle of forearm at the ulnar side: longitudinal and transverse scanning. All measurements were conducted over 2 days with a 24-h interval. Standard error of measurement (SEM) and minimal detectable change (MDC) were also calculated. RESULTS: The an

Identification of spastic muscles involved in abnormal joint posture in patients with upper motor neuron syndrome: a narrative review.

Few studies have specifically investigated which muscles are involved in abnormal joint posture (AJP) due to muscle spasticity and should therefore be targeted for botulinum toxin injections. This gap has significant implications for treatment efficiency, safety, health economics, and sustainable healthcare. A 2000 to 2025 (July) PubMed search identified 3,488 articles, but only 7 articles met the criteria for providing a method to determine the muscles involved in AJP due to muscle spasticity. Of these, just 2 have proposed how to measure each muscle contribution and only 1 focused on identif

Is the Silfverskiöld Test a valid tool for evaluating calf muscles spastic overactivity in patients with stroke? A retrospective observational study.

BACKGROUND: Spastic equinus (plantar flexed) foot is a common postural pattern in patients who suffer from post-stroke spasticity. To date, some clinicians use the Silfverskiöld Test in their practice to differentiate between gastrocnemius and soleus muscle overactivity in patients with spastic equinus (plantar flexed) foot. This use of the Silfverskiöld Test goes beyond its original aim, which was to distinguish isolated gastrocnemius contracture in patients with equinus deformity. AIM: The aim of this study was to investigate the Silfverskiöld Test validity for evaluating spastic equinus (p

Electrical Stimulation of Injected Muscles to Boost Botulinum Toxin Effect on Spasticity: Rationale, Systematic Review and State of the Art.

Botulinum toxin type A (BoNT-A) represents a first-line treatment for spasticity, a common disabling consequence of many neurological diseases. Electrical stimulation of motor nerve endings has been reported to boost the effect of BoNT-A. To date, a wide range of stimulation protocols has been proposed in the literature. We conducted a systematic review of current literature on the protocols of electrical stimulation to boost the effect of BoNT-A injection in patients with spasticity. A systematic search using the MeSH terms "electric stimulation", "muscle spasticity" and "botulinum toxins" an

Location of motor branches of tibialis posterior muscle and its relation in treatment of spastic equinovarus foot: a cadaveric study.

BACKGROUND AND OBJECTIVES: Nerve block or neurolysis is an important approach in the treatment of spastic equinovarus foot. To illustrate the accurate location of the nerve branch to the tibialis posterior muscle (TP) in clinical practice, 21 adult cadavers were dissected and 14 complete both lower limb specimens were obtained. A total of 28 lower limbs were included. METHODS: We measured the length of the motor branch nerve (LM) of the tibialis posterior muscle, the length of the fibula (LF), the vertical distance (D1) from the midpoint of LM to the fibula tip as well as the horizontal dista

Mechanical muscle and tendon properties of the plantar flexors are altered even in highly functional children with spastic cerebral palsy.

BACKGROUND: Recent ultrasound studies found increased passive muscle stiffness and no difference in tendon stiffness in highly impaired children and young adults with cerebral palsy. However, it is not known if muscle and tendon mechanical properties are already altered in highly functional children with cerebral palsy. Therefore, the purpose of this study was to compare the mechanical and material properties of the plantar flexors in highly functional children with cerebral palsy and typically developing children. METHODS: Besides strength measurements, ultrasonography was used to assess gas

Muscle relaxant action of 2-chloroadenosine in genetically spastic rats is independent of gamma-aminobutyric acid-mediated inhibition.

The role played by benzodiazepine (BDZ) receptors and GABAergic mechanisms in the muscle relaxant effect of the adenosine receptor agonist, 2-chloroadenosine (2-CLA) was evaluated in genetically spastic rats. 2-CLA reduced in a dose-related manner the spontaneous tonic activity in the electromyogram (EMG) of the gastrocnemius-soleus muscle. While the muscle relaxant effect produced by the adenosine analogue was abolished by aminophylline, a methylxanthine with potent antagonistic activity of adenosine-mediated inhibition, the two gamma-aminobutyric acid (GABA) antagonists bicuculline and picro

Familial spastic paraplegia with distal muscle wasting in the Old Order Amish; atypical Troyer syndrome or "new" syndrome.

The Troyer syndrome was found by Cross & McKusick (1967) in 20 members of 12 Old Order Amish families in Holmes County, Ohio; it is a form of hereditary spastic paraplegia combined with distal muscle wasting, i.e. signs of involvement of lower motor neurons. The condition usually begins at 1 to 2 years and progresses at variable rates. Further manifestations include growth retardation, delayed speech development with dysarthria and drooling, and cerebellar signs; mental functions are usually not affected but severe emotional lability is a common finding. Increasing the Dose of Onabotulinum Toxin A From 400 to 600 Units for Post-stroke Spasticity: A Retrospective Study on Safety and Muscle Selection.

In Japan, the approved maximum dosage of Onabotulinum Toxin A (BoNT-A) (BOTOX, Allergan plc, Irvine, CA, USA) for upper and lower limb spasticity was increased to 600 units in November 2022. As reports on this high dosage are rare internationally, this study aimed to confirm its safety and evaluate the muscles administered. The study included 24 stroke patients (18 men, six women) treated with 600 units of BoNT-A between December 2024 and January 2025. Medical records were reviewed for treatment history, including the number of injection cycles and target muscles. On the final injection day, p

Is There a New Road to Spinal Cord Injury Rehabilitation? A Case Report about the Effects of Driving a Go-Kart on Muscle Spasticity.

BACKGROUND: Traumatic spinal cord injury (SCI) is a neurological disorder that causes a traumatic anatomical discontinuity of the spinal cord. SCI can lead to paraplegia, spastic, or motor impairments. Go-karting for people with SCI is an adapted sport that is becoming increasingly popular. The purpose of this case report is to shed light on the effects of driving a go-kart on a patient with SCI-related spasticity and to deepen understanding of the possible related role of whole-body vibration (WBV) and neuroendocrine reaction. METHODS: The patient was a 50-year-old male with a spastic parapl

Passive muscle properties are altered in children with cerebral palsy before the age of 3 years and are difficult to distinguish clinically from spasticity.

AIM: Clinical determination of spasticity is confounded by the difficulty in distinguishing reflex from passive contributions to muscle stiffness. There is, therefore, a risk that children with cerebral palsy (CP) receive antispasticity treatment unnecessarily. To investigate this, we aimed to determine the contribution of reflex mechanisms to changes in the passive elastic properties of muscles and tendons in children with CP. METHOD: Biomechanical and electrophysiological measures were used to determine the relative contribution of reflex and passive mechanisms to ankle muscle stiffness in
