OBJECTIVE: To assess the effect of increased use of intravenous penicillin for group B streptococcus (Streptococcus agalactiae, GBS) antibiotic prophylaxis on non-GBS neonatal sepsis and antibiotic resistance.
METHODS: We undertook a nonconcurrent cohort study. Microbiology cultures originating from infants with early-onset neonatal sepsis in our neonatal intensive care unit (NICU) from 1992 to 1999 were reviewed. Prevalence of non-GBS neonatal sepsis in the control period (January 1, 1992, through June 30, 1995) was compared with that in the study period (October 1, 1995, through August 31,

Both experimental and clinical evidence suggest a suppression of T-cell function in burn and sepsis. The objective of the present study was to evaluate splenocyte and purified T-cell proliferative response and IL-2 production in septic neonatal rats. We also examined if alterations in T-cell proliferation and IL-2 production in neonatal sepsis is due to elevation in PGE2. PGE2 is known to play a significant role in T-cell suppression during sepsis in adults. Sepsis was induced in 15-day-old neonatal Sprague-Dawley rats by implanting 0.1 cm3 of fecal pellet impregnated with Escherichia coli (50

UNLABELLED: Group B streptococcus (GBS) was recognized as a major pathogen of neonatal disease in the 1970s. With a case-fatality rate of 5% to 20%, prevention of GBS neonatal disease has been an ongoing concern. The Centers for Disease Control and Prevention (CDC), and American College of Obstetricians and Gynecologists (ACOG) published guidelines for preventive strategies in 1996. These strategies, either a risk-based or a culture-based program, have been responsible for reduced incidence of GBS-newborn disease from 1.7 to 0.4 per 1,000 live births in the years 1993 to 1999. However, there h

OBJECTIVE: To compare the relative effects of intrapartum antibiotic prophylaxis regimens on patterns of early-onset neonatal sepsis.
METHODS: We performed an historical cohort study of 17187 infants born at our center from September 1993 to February 2000. A risk-based strategy was employed prior to July 1996 and a screening-based strategy was utilized thereafter. Ampicillin was utilized prior to March 1995 and penicillin was used thereafter.
RESULTS: There were 75 cases of neonatal sepsis, 34 (4. 10/1000) in the risk-based era and 41 (4.63/1000) in the screening-based era (p = 0.62). There

The interesting association between delayed presentation of right-sided diaphragmatic hernia and neonatal Group B streptococcal infections occurs rarely and its pathogenesis is still obscure. Two preterm newborn infants with early onset of neonatal sepsis (one due to Group B Streptococcus) followed by recognition of right-sided diaphragmatic hernia on the 9th and 25th day of life are reported. In both cases the course of neonatal sepsis and pneumonia was complicated due to the appearance of right-sided pleural effusion and atelectasis. On serial chest roentgenograms right-sided bowel gas was n

A case report of a newborn with sepsis due to nontypable H.Influenzae biotype IV is presented. There were no prematurity nor maternal obstetrical complications involved. The child however suffered from severe respiratory distress. With the aspiration of secretions, the resuscitation with mask oxygen and the empirically started combination of ampicillin and cefotaxime, his condition rapidly improved.

In a retrospective study at Son Dureta Hospital (Palma de Mallorca, Mallorca, Spain) of the period 1977-1991, 334 cases of culture-proven sepsis and/or meningitis in neonates born at the facility were identified. Overall, there was an incidence rate of 4.9 cases per 1,000 live births. The case-fatality rate was 7.5%. Infection was more frequent in infants of low birth weight, with the exception of infants with meningitis and infections due to group B Streptococcus and Listeria species. The patterns of predominance among bacterial pathogens that were isolated changed during the period studied.

Seventy one per cent of neonates had a gestational age longer than 37 weeks. Sixteen showed respiratory distress, and shock occured in twelve cases. The onset of illness happened within the first 12 hours of life, in 15 cases. No differences were observed among preterm and term infants. Differential leukocyte count were altered in all infants, being particularly relevant the fact that the relation between immature neutrophil/total neutrophil, was elevated in 50 per cent of cases. Mortality rate was 52 per cent. Clinical alert and early treatment with penicillin, together with the use of fresh

Neutrophil extracellular traps (NETs) contribute to innate immunity in sepsis, but their diagnostic value in neonates is unclear. We evaluated whether circulating NET-associated biomarkers discriminate septic from non-infected neonates. In this prospective observational study 96 neonates (≥34 weeks gestational age) with clinical suspicion of infection were enrolled (36 sepsis, 60 controls). Serum cell-free DNA (cfDNA), myeloperoxidase-DNA complexes (MPO-DNA), neutrophil elastase-DNA complexes (NE-DNA), and citrullinated histone H3 (H3cit) were measured alongside CRP and IL-6 at days 1, 3, and

It is paramount to optimize prevention and therapy for severe neonatal infections, particularly in high-risk infants. This perspective highlights the lack of guidelines for parenteral nutrition (PN) during suspected or confirmed infections in vulnerable newborns, especially those born preterm. Drawing on insights from immunometabolism and translational preclinical models, we argue that nutritional support during infection should not only meet energy demands but also actively shape immunity by modulating inflammation, organ injury, and clinical outcome. Conventional glucose-rich PN may fuel exc

This study evaluated the diagnostic performance of mean neutrophil volume (MNV) in culture-positive neonatal sepsis. MNV (index test), measured using an automated hematology analyzer, was compared to blood culture (reference test) in a study including 535 neonates; 86 were culture-positive sepsis, 192 were probable sepsis, and 257 were not septic. The median (Q1, Q3) MNV (fL) was higher in culture-positive septic neonates compared to non-septic neonates [154 (152, 157) vs. 136 (133, 143), P = 0.001]. MNV > 151 fL diagnosed sepsis with 84.8% sensitivity, 89.4% specificity, 76% positive predicti

Probiotic supplementation in preterm infants is one of the most extensively studied interventions in neonatal medicine, with over 50 randomised controlled trials. This paper examines the relationship between probiotic supplementation and late onset sepsis (LOS), considering mechanistic pathways, clinical evidence, and safety profile. Multiple systematic reviews and meta-analyses consistently show that probiotics reduce necrotising enterocolitis (NEC) incidence and all-cause mortality in preterm infants, establishing them as one of the most beneficial interventions in neonatology. Current evide

OBJECTIVE: This study evaluates the time to blood culture positivity among neonates evaluated for sepsis to determine when antibiotics may be safely discontinued.
STUDY DESIGN: Retrospective review of clinically relevant blood culture time to positivity from infants in a neonatal intensive care unit. The primary endpoint was overall median time to blood culture positivity. Secondary endpoints compared time to positivity among gram-positive versus gram-negative organisms, early versus late onset sepsis, and a sub-analysis excluding contaminants.
RESULT: Among 151 cultures, the overall median

BACKGROUND: Low- and middle-income countries lack data on culture-confirmed sepsis in HIV-exposed infants, despite the reported heightened risk of infectious morbidity. This study describes culture-confirmed sepsis and antibiotic resistance patterns among HIV-exposed children in a large etiological cohort study in Kampala, Uganda.
METHODS: This was a prospective birth cohort study based at 2 Ugandan sites, as part of the Progressing Group B Streptococcal Vaccines (PROGRESS) study. Any infant with risk factors, signs, or symptoms of infection presenting before 3 months of age had a blood cultu

Neonatal sepsis, a bloodstream infection in the first 28 days of life, is a leading cause of morbidity and mortality among infants in both developing and developed countries. Additionally, sepsis is distinguished in neonates by unique pathophysiological and presentational factors relating to its development in immature neonatal immune systems. This review focuses on the current understanding of the mechanics and implications of neonatal sepsis, providing a comprehensive overview of the epidemiology, aetiology, pathophysiology, major risk factors, signs and symptoms and recent consensus on the

Introduction: Early-onset neonatal sepsis is a rare, but life-threatening condition. In 2017, the Hungarian Neonatal Society issued a national guideline to rationalize the use of antibiotic use in neonatal sepsis. Objective: To retrospectively determine the frequency of prescribed antibiotics before and after the introduction of national guidance. Method: Data of neonates (>34. gestational weeks) delivered in our hospital between 1st January 2014 and 31st December 2018 were analysed with focusing on signs of sepsis, number of neonates treated with antibiotics, incidence of early-onset neonatal

The neonatal period in dogs remains associated with high mortality rates. Sepsis is the main cause of neonatal losses during the first three weeks of life. Additionally, failure in the clinical assessment and early diagnosis of sick newborns is still common, leading to inadequate care, which contributes to a high mortality rate. Thus, the objective of this study was to describe the incidence of sepsis in canine newborns, the clinical aspects involved, the main isolated bacterial agents and mortality rates to facilitate clinicians' early recognition of this condition. Of the 152 litters and 762
