Artificial urinary sphincter insertion in congenital neuropathic bladder.

A series of 44 patients with neuropathic bladder dysfunction due to congenital myelodysplasia underwent implantation of an artificial urinary sphincter (AUS); in 26 patients a reconstructive procedure was performed at the same time. Five patients who initially had AUS implantation alone developed detrusor hyper-reflexia and required a clam cystoplasty. Forty patients (90%) are continent, 2 are occasionally damp and 2 are wet and awaiting further surgery; 66% of patients who underwent AUS insertion and reconstruction were found to have significant residual urine and they perform intermittent se

Improving nerve and muscle function: an exploration of targeted nerve function replacement following differential delay periods in a rat model.

BACKGROUND: Targeted Muscle Reinnervation (TMR) improves real-time control of EMG-based prostheses by connecting severed nerves to adjacent muscles, creating new EMG signals. However, TMR requires cutting original nerve connections, which can cause denervation atrophy and limit functional recovery. As an alternative, Targeted Nerve Function Replacement (TNFR) offers a fundamentally different approach by establishing a direct end-to-end anastomosis between an intact donor nerve and the original nerve of a target muscle, preserving existing neural pathways while providing supplementary neural in

Enhancing skin-implant integration in lower-limb transcutaneous prostheses: From interface biology to bioactive, antimicrobial and cell-based strategies.

UNLABELLED: Transcutaneous prosthetic systems, which directly connect an external limb prosthesis to the skeleton, offer substantial biomechanical and clinical advantages over conventional socket-based devices. By eliminating the soft tissue loading inherent to the socket interfaces, these systems enable improved load transfer, comfort, and more natural sensory feedback through direct skeletal attachment. However, their widespread clinical adoption remains limited by complications at the skin-implant interface, including infection, marsupialization, and epithelial down-growth.Advances in surfa

Sensitivity to temporal parameters of intraneural tactile sensory feedback.

BACKGROUND: Recent studies have shown that neural stimulation can be used to provide artificial sensory feedback to amputees eliciting sensations referred on the amputated hand. The temporal properties of the neural stimulation modulate aspects of evoked sensations that can be exploited in a bidirectional hand prosthesis. METHODS: We previously collected evidence that the derivative of the amplitude of the stimulation (intra-digit temporal dynamics) allows subjects to recognize object compliance and that the time delay among stimuli injected through electrodes implanted in different nerves (i

Hindlimb motor responses evoked by microstimulation of the lumbar dorsal root ganglia during quiet standing.

UNLABELLED: Somatosensory afferent pathways have been a target for neural prostheses that seek to restore sensory feedback from amputated limbs and to recruit muscles paralyzed by neurological injury. These pathways supply inputs to spinal reflex circuits that are necessary for coordinating muscle activity in the lower limb. The dorsal root ganglia (DRG) is a potential site for accessing sensory neurons because DRG microstimulation selectively recruits major nerve branches of the cat hindlimb. Previous DRG microstimulation experiments have been performed in anesthetized animals, but effects on

Exploring applications of electrically evoked compound action potentials using the vestibulo-cochlear implant.

OBJECTIVES: Vestibulo-cochlear implants are a potential treatment approach for patients with loss of vestibular function. A critical aspect is the development of reliable and objective outcome measures, to assess their efficacy and optimize their performance. As in cochlear implant care and other neural prostheses, electrically evoked compound action potentials (eCAP) can be used to measure the neural response to implant stimulation. eCAPs might provide information about the presence and misalignment of the electrically evoked vestibulo-ocular reflex, and potential vestibulo-cochlear interacti

Wearable non-invasive neuroprosthesis for targeted sensory restoration in neuropathy.

Peripheral neuropathy (PN), the most common complication of diabetes, leads to sensory loss and associated health issues as pain and increased fall risk. However, present treatments do not counteract sensory loss, but only partially manage its consequences. Electrical neural stimulation holds promise to restore sensations, but its efficacy and benefits in PN damaged nerves are yet unknown. We designed a wearable sensory neuroprosthesis (NeuroStep) providing targeted neurostimulation of the undamaged nerve portion and assessed its functionality in 14 PN participants. Our system partially restor

Fabrication of bioactive conduits containing the fibroblast growth factor 1 and neural stem cells for peripheral nerve regeneration across a 15 mm critical gap.

Nerve conduits are often used in combination with bioactive molecules and stem cells to enhance peripheral nerve regeneration. In this study, the acidic fibroblast growth factor 1 (FGF1) was immobilized onto the microporous/micropatterned poly (D, L-lactic acid) (PLA) nerve conduits after open air plasma treatment. PLA substrates grafted with chitosan in the presence of a small amount of gold nanoparticles (nano Au) showed a protective effect on the activity of the immobilized FGF1 in vitro. Different conduits were tested for their ability to bridge a 15 mm critical gap defect in a rat sciatic

Neural signal sensing, transmission and functional regeneration on different toads' bodies.

The presence of neural signals is the most important feature of animals' life. Monitoring, analysis and regeneration of neural signals are important for the rehabilitation of paralyzed patients. In this paper, the neural signal regeneration between the proximal and the distal end of an injured nerve is introduced. In the experiment a microelectronic module is used as a channel bridge. The regeneration of nerve signals is realized from one toad's sciatic nerve to another's. Corresponding neural signals and EMG were recorded and analyzed. It will be a reference to further study on the neural sig

Augmenting rehabilitation robotics with spinal cord neuromodulation: A proof of concept.

Rehabilitation robotics aims to promote activity-dependent reorganization of the nervous system. However, people with paralysis cannot generate sufficient activity during robot-assisted rehabilitation and, consequently, do not benefit from these therapies. Here, we developed an implantable spinal cord neuroprosthesis operating in a closed loop to promote robust activity during walking and cycling assisted by robotic devices. This neuroprosthesis is device agnostic and designed for seamless implementation by nonexpert users. Preliminary evaluations in participants with paralysis showed that the

Neuroprosthesis for Decoding Speech in a Paralyzed Person with Anarthria.

BACKGROUND: Technology to restore the ability to communicate in paralyzed persons who cannot speak has the potential to improve autonomy and quality of life. An approach that decodes words and sentences directly from the cerebral cortical activity of such patients may represent an advancement over existing methods for assisted communication. METHODS: We implanted a subdural, high-density, multielectrode array over the area of the sensorimotor cortex that controls speech in a person with anarthria (the loss of the ability to articulate speech) and spastic quadriparesis caused by a brain-stem s

Targeted Muscle Reinnervation for Intuitive Prosthetic Control.

The convergence of medical and technological advances has led to a substantial expansion of therapeutic approaches for patients with limb amputations. Targeted muscle reinnervation (TMR) is one of the strategies to improve control of bionic arms and has been applied for more than two decades. Through surgical rewiring of the nerves responsible for hand functions to a new muscle target, a neuromuscular interface of the lost extremity is created, enabling improved prosthetic control. Neural information that would otherwise be lost through transected peripheral nerves is thus reestablished and ca

Flexible Magnetic Skin Sensor Array for Torsion Perception.

Prosthetic hands help upper limb amputees and people who were born without hands. Currently, these prostheses are rather rudimentary and do not provide adequate sensing capabilities compared to a human hand. People use their natural hands to perceive complex tactile phenomena such as shear and torsion using thousands of mechanoreceptors in their fingertips. The capability to detect torsional loads at the fingertips is a notable gap in prosthetic hand sensation. Flexible tactile sensors are a promising new technology that would be ideal for prosthetic hands since they allow for stretching and m

Cobalt Neurotoxicity: Transcriptional Effect of Elevated Cobalt Blood Levels in the Rodent Brain.

Metal-on-metal (MoM) hip implants made of cobalt chromium (CoCr) alloy have shown early failure compared with other bearing materials. A consequence of the abnormal wear produced by these prostheses is elevated levels of cobalt in the blood of patients, which can lead to systemic conditions involving cardiac and neurological symptoms. In order to better understand the implications for patients with these implants, we carried out metal content and RNA-Seq analysis of excised tissue from rats treated intraperitonially for 28 days with low concentrations of cobalt. Cobalt blood levels in dosed ra

Bionic Technology in Prosthetics: Multi-Objective Optimization of a Bioinspired Shoulder-Elbow Prosthesis with Embedded Actuation.

The development of upper-limb prostheses is often hindered by limited dexterity, a restricted workspace, and bulky designs, primarily due to performance limitations in proximal joints like the shoulder and elbow, which contribute to high user abandonment rates. To overcome these challenges, this paper presents a novel, bioinspired, and integrated prosthetic system as an advancement in bionic technology. The design incorporates a shoulder joint based on an asymmetric 3-RRR spherical parallel mechanism (SPM) with actuators embedded within the moving platform, and an elbow joint actuated by low-v

An OpenSim-Based Closed-Loop Biomechanical Wrist Model for Subject-Specific Pathological Tremor Simulation.

OBJECTIVE: A pathological tremor (PT) is an involuntary rhythmic movement of varying frequency and amplitude that affects voluntary motion, thus compromising individuals' independence. A comprehensive model incorporating PT's physiological and biomechanical aspects can enhance our understanding of the disorder and provide valuable insights for therapeutic approaches. This study aims to build a biomechanical model of pathological tremors using OpenSim's realistic musculoskeletal representation of the human wrist with two degrees of freedom. METHODS: We implemented a Matlab/OpenSim interface fo

A gait phase prediction model trained on benchmark datasets for evaluating a controller for prosthetic legs.

Powered lower-limb assistive devices, such as prostheses and exoskeletons, are a promising option for helping mobility-impaired individuals regain functional gait. Gait phase prediction plays an important role in controlling these devices and evaluating whether the device generates a gait similar to that of individuals with intact limbs. This study proposes a gait phase prediction method based on a deep neural network (DNN). The long short-term memory (LSTM)-based model predicts a continuous gait phase from the 250 ms history of the vertical load, thigh angle, knee angle, and ankle angle, comm

Coordination of Voluntary Residual Muscle Contractions in Transtibial Amputees: a Pilot Study.

Recently there has been considerable interest in the use of electromyography (EMG) for the control of powered, lower-limb prostheses. However, little is understood regarding amputee residual muscle, specifically the ability for lower-limb amputees to coordinate previously antagonist residual muscles for different control tasks. In this study, we aimed to investigate the capability of transtibial amputees in coordinating residual gastrocnemius and tibialis anterior for performing a high-level task. Specifically, we examined how three transtibial amputees and one ablebodied subject used residual
