The authors report on a case of Paget's disease with hydrocephalus in an elderly female with a favourable result after ventriculoperitoneal shunt. The neurological examinations at the time of admission revealed mild dementia, ataxic gait and urinary incontinence. Skull X-ray, CT-scan and MRI showed hydrocephalus and basilar impression. The pathophysiological hypothesis is discussed and the technical difficulties during the shunt surgery are described.

A case of neurofibromatosis with varied clinical symptoms due to parental consanguinity is reported. The patient presented the following manifestations: mental retardation, curved tibias, kyphoscoliosis, basillary impression with pyramidal syndrome and parkinsonism, subluxation of the lenses, bilateral blindness, and hypogonadism. Subluxation of the lenses and hypogonadism deserve special mention because of the rarity of their presentation in this disease. The hypogonadism was of the hypogonadotrophic type without evidence of pituitary or gonadal tumor. The possible causes of endocrine dysfunc

A clinico-roentgenological examination of the transitional craniovertebral division has revealed a high incidence and a great diversity of isolated underdevelopment of the arches of the upper cervical vertebra--from their fissure-like splitting to complete absence. Usually these forms create no preconditions for disturbances of any functions, do not manifest clinically, and can be regarded as variants of normal structure of the craniovertebral division.

OBJECTIVE AND IMPORTANCE: Trigeminal neuralgia is a rare feature of basilar invagination, which is itself a complication of osteochondrodysplastic disorders. Microvascular decompression is an unattractive option in medically refractory cases. The conventional percutaneous approach to the trigeminal ganglion is anatomically impossible because the foramen ovale points inferiorly and posteromedially. We report a new technique for image-guided trigeminal injection in a patient with basilar invagination complicating osteogenesis imperfecta.
CLINICAL PRESENTATION: A 26-year-old woman with osteogene

A case of traumatic cervical spinal cord injury due to congenital craniocervical abnormalities is described. The autopsy revealed evidence of hyperextension of the neck due to frontal impact, congenital occipitalization of the atlas accompanied by basilar invagination of the odontoid process and atlanto-axial diastasis. The subsequent narrowing of the spinal canal led to a predisposition for this unusual mechanism of spinal cord injury.

OBJECTIVE: The objective of this study was to develop a vertebral artery (VA) classification system that improves risk assessment using a 2D map of the VA position at the craniovertebral junction (CVJ), and to outline surgical strategies for managing anomalous VAs during the posterior atlantoaxial joint release, distraction, and fusion technique in the treatment of basilar invagination (BI) and atlantoaxial instability (AAI).
METHODS: In this retrospective study, 125 patients (mean age 44.1 years) with BI and AAI who underwent surgery between January 2019 and April 2021 using the atlantoaxial

C3-4 cervical disc prolapse was associated with basilar invagination and short neck in a 21-year-old man and additionally with an extensive Klippel-Feil abnormality and fusion of multiple cervical vertebrae in a 32-year-old man. The transoral surgical route was adopted for cervical discectomy in the latter case and an additional odontoidectomy in the former case. Interbody plate and screw fixation was carried out in the patient with Klippel-Feil abnormality. Both the patients were relieved of symptoms and remained asymptomatic at follow up. Simultaneous fixation procedure is not mandatory afte

It is prudent to apply several methods of evaluation of cervicobasilar relationships in basilar impression and, in suggestive cases, to use tomographic evaluation for complete study. This discussion reviews normal cervicobasilar relationships and illustrates selected pathologic processes that produce basilar impression.

A 47-year-old male presented with abducens nerve palsy due to basilar impression associated with atlanto-occipital assimilation manifesting as slowly progressive bilateral trigeminal neuralgia and diplopia in the right lateral gaze. X-ray and computed tomography of the skull confirmed the diagnosis of basilar impression and atlanto-occipital assimilation, and magnetic resonance imaging disclosed tightness of the posterior cranial fossa. Surgical suboccipital decompression resulted in gradual resolution of the patient's complaints, and no additional symptoms were recognized. Impairment of the s

Adult patients with primary basilar impression were four times more likely to have a history of birth injury than a control group. Distortion of basicranial synchondroses during birth is suggested as a possible mechanism and, in association with hindbrain herniation and arachnoid adhesions, may contribute towards the development of syringomyelia.

We report on a male patient with exophthalmos of unclear etiology, basilar impression, syringohydromyelia and type II Arnold-Chiari malformation. Two diseases involving the orbital region were to be considered in differential diagnostic terms: endocrine orbitopathy and osseous orbit dysplasia. The typical physical appearance associated with basilar impression as well as suppurative keratitis in Lagophthalmos was striking. Tetraspasticity with pareses, bulbar symptoms, proximally pronounced muscular atrophy as well as a left hemihypesthesia was shown neurologically. Although the orbit CT was no

OBJECTIVE: The objective of this study was to analyze failed posterior fossa decompression (PFD) in patients with basilar invagination and atlantoaxial dislocation (BI-AAD). Revision surgery in these patients is challenging and has been rarely reported. In addition, the anatomical variations of the vertebral artery increase the risk of revision surgery. Here, we introduce the implementation of a new type of one-stage posterior revision surgery, whose difficulties and effects are summarized.
METHODS: A total of 21 patients with BI-AAD who underwent PFD were retrospectively analyzed in our cent

STUDY DESIGN: Retrospective study.
OBJECTIVE: To evaluate the feasibility of C2 pedicle screw fixation with the "in-out-in" technique in patients with basilar invagination (BI).
SUMMARY OF BACKGROUND DATA: The "in-out-in" technique is a fixation technique in which the screw enters the vertebrae through the parapedicle. The technique has been used in upper cervical spine fixation. However, anatomic parameters associated with the application of this technique in patients with BI are unclear.
MATERIALS AND METHODS: We measured the C2 pedicle width (PW), the distance between the vertebral arter

OBJECTIVE: The purpose of this study was to develop a new index that can reliably quantify the reduction of basilar invagination with atlantoaxial dislocation.
PATIENTS AND METHODS: Between May 2012 and September 2017, 40 patients with congenital basilar invagination and atlantoaxial dislocation as well as 100 sex-and age-matched control subjects were recruited for this study. All patients underwent direct posterior reduction and fixation. Mid-sagittal computerized tomography scan films were obtained before and after surgery as well as the vertico-horizontal atlantoaxial index (VHAI) was meas

PURPOSE: To describe the manifestations, surgical treatment, and potential complications of Hajdu-Cheney syndrome (HCS), and the management of these complications.
METHODS: The clinical presentation, management and outcome of HCS with severe osteoporosis and open skull sutures is presented, together with a literature review.
RESULTS: A 20-year-old female with HCS underwent posterior occipitocervical fusion for symptoms of progressive basilar invagination. Because of delayed lambdoid suture closure, the stiff fusion construct lead to increased suture distraction, most notably in the upright (

A 50-year-old man with myelopathy secondary to basilar impression developed bilateral vertebral artery dissection after undergoing treatment with 8 pounds of cervical traction. The vertebral artery dissection resulted in vertebrobasilar insufficiency and posterior circulation stroke. In this report, the current management philosophies in the treatment of basilar impression are discussed, and the pertinent neurovascular anatomy is illustrated. This report suggests that vertebral artery injury may result from attempted reduction of severe basilar impression. Regardless of the cause of cranial se
