The largely untold story of welfare reform and the human services.

Welfare reform has placed the lives of clients, the jobs of social workers, and the mission of agencies in jeopardy. Based on interviews with senior staff at 107 nonprofit human services agencies, this article documents the largely untold story of how nonprofit agencies' workers responded to the impact of welfare reform on their clients, their jobs, and the delivery of services. Workers reported less time for social services because of welfare-related regulations, penalties, work mandates, crises, and paper work. They also reported more service dilemmas including less control of the job, more

Functional impairment and disability across mood states in bipolar disorder.

BACKGROUND: Bipolar disorder (BD) represents a chronic and recurrent illness that can lead to severe disruptions in family, social, and occupational functioning. The severity of mood symptomatology has been associated with functional impairment in this population. However, the majority of studies have assessed global functioning without considering specific domains. The main objective of the current study was to assess specific life domains of functioning as well as the overall functioning in patients with BD across different mood states ([hypo] mania, depression, or euthymia) compared with he

Remote supervision in rural psychiatry training: Theory and lived experience.

ObjectiveTo examine the utility and limitations of remote supervision in a rural psychiatry training program through narrative review and dual reflective accounts.ConclusionsConsistent, well-structured remote supervision can nurture clinical expertise, ensure good clinical governance, improve patient safety, and promote graded trainee autonomy. Limitations can include reduced opportunity for direct observation, challenges processing complex relational dynamics, and heightened trainee isolation. Clear pedagogical structures, reliable technology, and a strong local multidisciplinary team are key

Supporting People with Swallowing Problems in Home Care.

Dysphagia management in home-based care presents distinct challenges that may increase professional vulnerability and complicate clinical decision-making. Persistent misconceptions regarding the role of aspiration in pneumonia pathogenesis, as well as the efficacy of dietary modification as a primary intervention, can contribute to adverse clinical outcomes. A synthesis of the current literature highlights alternative strategies and supports a paradigm shift in the management of swallowing disorders. Adopting an evidence-based approach that emphasizes personal autonomy while providing clinicia

Abortion and informed consent requirements.

Supreme Court decisions have liberalized a woman's right to decide whether to obtain an abortion. Some state and local governments have tried to circumvent these decisions by enacting requirements designed to discourage abortions by, among other things, dictating to physicians an elaborate litany of specific information that must be communicated to a patient as a necessary precondition of her informed consent for an abortion. This article discusses the legal status of such requirements, their implications for the professional autonomy of physicians, and the role of the medical profession in ch

Peer learning in birth units: Experiences of preceptors, student coordinators and managers.

AIM: To explore managers', student coordinators' and preceptors' experiences of implementing and using peer learning in birth units. BACKGROUND: Clinical learning in birth units is influenced by multiple factors and may be challenging for both midwifery students and preceptors. To complement traditional one-to-one supervision, the Birth Unit Peer Learning model was developed and implemented in Swedish birth units. The model aims to promote student-centred learning, collaboration, reflection and active participation in clinical practice while maintaining the quality of learning and care. DESI

Is it ethically permissible for GPs to promote non-directed altruistic kidney donation to healthy adults?

Doctors hold coexisting ethical duties to avoid causing deliberate harm to their patients (non-maleficence), to act in patients' best interests (beneficence), to respect patients' right to self-determination (autonomy) and to ensure that costs and benefits are fairly distributed among patients (justice). In the context of non-directed altruistic kidney donations (NDAKD), doctors' duties of autonomy and justice are in tension with those of non-maleficence and beneficence. This article examines these competing duties across three scenarios in which general practitioners (GPs) could promote NDAKD

General practitioners' views on drug-assisted rehabilitation.

BACKGROUND: The Norwegian model for opioid maintenance treatment (OMT) "Drug-assisted rehabilitation" (DAR) is a cross-disciplinary tripartite model for the treatment of opioid dependence. The model requires collaboration among GPs, the social services and the specialist health services. To some degree it restricts the doctor's professional autonomy. The investigation aims to examine GPs' attitudes to the model and in particular the experiences of those who have actively participated. MATERIAL AND METHOD: An electronic questionnaire (Questback) was sent to Norwegian GPs listed on the members'

[Social and family considerations in genetic counselling].

Genetic knowledge can be viewed as an important tool in the prevention of hereditary disease. This article discusses the social issues that the link between genetics and prevention raises in cancer genetic counselling and testing. In particular, the moral dilemmas which health professionals and patients face in relation to disclosing genetic information to kin are described. The authors argue that questions about disclosing information and balancing autonomy and responsibility are not only ethical questions but biopolitical issues.

'Part of my humanity left me the day I walked into medical school': Navigating medical students' identity dissonance through the medical humanities.

INTRODUCTION: Medical humanities curricula offer a lens for understanding how medical learners construct meaning and professional identity. While intended to cultivate patient-centered physicians, this process can provoke identity dissonance rooted in the hidden curriculum, leading learners to suppress, compartmentalize, or defer emotional engagement. This study examines how a medical humanities exercise shaped learners' reflections on identity dissonance, affect, and professional identity formation. Through qualitative analysis of reflective narratives, we explored how students interpreted te

[Liaison psychiatric aspects in the field of child and adolescent psychiatry].

'Liaison'-psychiatric services are defined by a continuous collaboration of the child and adolescent psychiatrist in a non-psychiatric team (e.g. in educational, social welfare or medical/nursing teams). As prerequisites clear arrangements, mutual acknowledgement of skills and autonomy, a common professional language and a long-lasting engagement of the same persons are absolutely necessary. Amongst other services to be rendered, the recognition and working through of mechanism like the 'projective identification' and the 'false self', be it with single members or with the whole of the therape

Graduating vascular surgery trainee proficiency in endovascular and open peripheral revascularization procedures.

BACKGROUND: Endovascular interventions for peripheral artery disease have increased in prevalence over time given the inherent benefits of minimally invasive approaches. Although it is essential that vascular surgery graduates are facile with endovascular techniques, the results of the BEST-CLI (Best Endovascular vs. Best Surgical Therapy in Patients With Critical Limb Ischemia) trial highlight the equivalent importance of ensuring trainee competence in open skills. Recent studies demonstrate increasing case volume of both endovascular and open procedures during vascular surgery training. Case

Ethical analysis of community-based dementia screening for unhoused older adults.

Screening for dementia and mild cognitive impairment (MCI) in community-based settings helps connect vulnerable older adults to medical and social support services promoting well-being. Referencing a universal screening program for unhoused older adults seeking emergency shelter services as a case example, this paper calls attention to alignment of programmatic features with the four principles of biomedical ethics, beneficence, nonmaleficence, autonomy, and justice. Regarding beneficence, homeless services workers gain insight into clients' needs and can facilitate engagement with resources t

A scoping review of personalisation in the UK: approaches to social work and people with learning disabilities.

There have been rapid developments in personalisation of health and social care in the UK over the past 5 years to develop a more flexible model of provision based upon greater choice and control for service users. This has been important for people with learning disabilities who are often dependent on others such as social workers to support their autonomy and independence. This article discusses a study carried out to explore the impact of personalisation on people with learning disabilities and the role of social workers to support this. A scoping review of the UK literature from 1996 to 20
