Rare association of extensive myelinated retinal nerve fibers with vascular abnormalities and vitreous hemorrhage: A case report.

PURPOSE: To report a rare case of unilateral extensive myelinated retinal nerve fibers (MRNFs) associated with retinal vascular changes, including diffusely sclerosed vessels, complicated by vitreous hemorrhage (VH). OBSERVATIONS: A 39-year-old woman, with no history of systemic disease, presented with a 2-day history of sudden blurred vision in the right eye. She had a history of anisometropic amblyopia in the same eye. Fundus examination of the right eye revealed vitreous hemorrhage and extensive, flame-shaped MRNFs radiating from a crowded optic disc and involving the superior and inferior

Catching the red eye: A retrospective review of factors associated with retinal hemorrhage in child physical abuse.

BACKGROUND/PURPOSE: Accurate identification of child physical abuse is crucial during the evaluation of injured children. Retinal hemorrhages (RH) are used for diagnosis, but clear criteria for screening with direct fundoscopic exam are lacking. We sought to identify key factors associated with RH to guide evaluations. METHODS: Electronic medical records for patients <1 year of age presenting to a Level I Pediatric Trauma Center with unwitnessed head injury from January 2015 to December 2018 were retrospectively reviewed. Multivariable logistic regression was used to identify factors associat

Acute retinal hemorrhage after Pipeline embolization device placement for treatment of ophthalmic segment aneurysm: A case report.

Introduction Ophthalmic segment aneurysms may present with visual symptoms due to direct compression of the optic nerve. Treatment of these aneurysms with the Pipeline embolization device (PED) often results in visual improvement. Flow diversion, however, has also been associated with occlusion of the ophthalmic artery and visual deficits in a small subset of cases. Case report A 49-year-old Caucasian female presented with subarachnoid hemorrhage due to a ruptured anterior communicating artery aneurysm. On follow-up imaging, the patient was found to have a right asymptomatic ophthalmic segment

A Challenging Approach: Iris Neovascularization and Vitreous Hemorrhage 40 Years after Treatment of Retinal Giant Tear Detachment.

BACKGROUND: Iris neovascularization and vitreous hemorrhages are typical sequels of ischemic retinopathies. Here, we report about a patient who developed marked iris neovascularization without signs of posterior retinal ischemia. CASE PRESENTATION: A 56-year-old patient developed marked iris neovascularization after surgery for rhegmatogenous retinal detachment due to a giant retinal tear had been performed 40 years earlier. With repeatedly applied intravitreal injections of an anti-VEGF drug being unsuccessful, anterior retinal cryocoagulations with deep indentation by the cryoprobe to someh

Development of a Deep Learning Model for Retinal Hemorrhage Detection on Head Computed Tomography in Young Children.

IMPORTANCE: Abusive head trauma (AHT) in children is often missed in medical encounters, and retinal hemorrhage (RH) is considered strong evidence for AHT. Although head computed tomography (CT) is obtained routinely, all but exceptionally large RHs are undetectable on CT images in children. OBJECTIVE: To examine whether deep learning-based image analysis can detect RH on pediatric head CT. DESIGN, SETTING, AND PARTICIPANTS: This diagnostic study included 301 patients diagnosed with AHT who underwent head CT and dilated fundoscopic examinations at a quaternary care children's hospital. The s

Hemorrhagic posterior vitreous detachment: A comprehensive review of the risks of retinal breaks, detachment, and management options.

Patients with hemorrhagic posterior vitreous detachment (hPVD), defined as concomitant PVD and vitreous hemorrhage, are at heightened risk of retinal breaks (RBs) and detachment (RD). Despite this, patients are routinely given the option between conservative management and early intervention with vitrectomy, as no consensus has been reached regarding the superiority of either strategy. Herein, we provide a scoping review of the current evidence regarding the risk of RBs and RD and management of patients with hPVD. Findings suggest that 30-60% of patients with hPVD may suffer from retinal break

A finite element analysis of the retinal hemorrhages accompanied by shaken baby syndrome/abusive head trauma.

We aimed to elucidate the mechanism of the retinal hemorrhage (RH) accompanied by shaken baby syndrome or abusive head trauma (SBS/AHT) by analyses using a computational model. We focused on a hypothesis that the vitreoretinal traction due to acceleration and deceleration caused by abusive shaking leads to retinal hemorrhage. A finite element (FE) mechanical model with simple spherical geometry was constructed. When the FE mechanical model was virtually shaken, the intensity of the stress applied to the retinal plane agreed well with the results from an analysis using a physical model made of

[Two children with cerebral and retinal hemorrhages: do not diagnose shaken baby syndrome too rapidly].

We report on 2 cases associating retinal (RH) and cerebral hemorrhages (CH), which first suggested the diagnosis of shaken baby syndrome (SBS). After an etiologic search, the diagnosis was corrected: the first case was a late hemorrhagic disease of the newborn and the second case hemophilia A. RH is a major feature of SBS, although not pathognomonic. There is no specific RH of SBS but they usually affect the posterior retinal pole. Typically, RHs of SBS are present in both eyes, although unilateral RHs do not exclude the diagnosis of SBS. The relationship between RH and CH has been reported in

Extracellular carbonic anhydrase mediates hemorrhagic retinal and cerebral vascular permeability through prekallikrein activation.

Excessive retinal vascular permeability contributes to the pathogenesis of proliferative diabetic retinopathy and diabetic macular edema, leading causes of vision loss in working-age adults. Using mass spectroscopy-based proteomics, we detected 117 proteins in human vitreous and elevated levels of extracellular carbonic anhydrase-I (CA-I) in vitreous from individuals with diabetic retinopathy, suggesting that retinal hemorrhage and erythrocyte lysis contribute to the diabetic vitreous proteome. Intravitreous injection of CA-I in rats increased retinal vessel leakage and caused intraretinal ede

Magnetic resonance imaging-based detection of retinal hemorrhages in a multicenter cohort of abusive head trauma.

BACKGROUND: In pediatric abusive head trauma (AHT), retinal hemorrhages are a key diagnostic feature. Detection by fundoscopy may be delayed or limited, whereas magnetic resonance imaging (MRI) enables non-invasive, objective assessment on routine brain sequences. OBJECTIVE: To evaluate the diagnostic utility of different MRI sequences-particularly susceptibility-weighted imaging (SWI), T2*-weighted (T2*w), and morphological sequences-in detecting retinal hemorrhages in AHT. MATERIALS AND METHODS: In this retrospective multicenter study (2006-2015), 57 well-documented AHT cases from three Ge
