Proliferative diabetic retinopathy in typical retinitis pigmentosa.

A 39-year-old woman with typical retinitis pigmentosa (RP) for 9 years and a positive family history of night blindness was diagnosed with diabetes mellitus (DM). She developed proliferative diabetic retinopathy (PDR) during the course of disease. She was promptly managed with pan retinal photocoagulation (PRP). PDR developing in a case of typical RP is extremely rare and has not been reported in the literature to date. Recognition of this rare, vision threatening complication, points out a definite need to further look deep into the pathogenesis of diabetic retinopathy.

Optic disc pit with sectorial retinitis pigmentosa.

Sectorial retinitis pigmentosa (RP) and optic disc pit (ODP) are rare clinical conditions. We present a 40-year-old woman with a history of mild night blindness and decreased vision in the right eye for about 5 years. Fundus examination revealed retinal pigmentary changes in the superior and inferotemporal sectors covering the macula and reduced arterial calibre and ODP at the temporal edge of the optic disc. In addition, fundus autofluorescence, spectral-domain optical coherence tomography, fluorescein angiography, and multifocal electroretinogram scans confirmed these clinical findings. Visu

Next-generation genetic testing for retinitis pigmentosa.

Molecular diagnostics for patients with retinitis pigmentosa (RP) has been hampered by extreme genetic and clinical heterogeneity, with 52 causative genes known to date. Here, we developed a comprehensive next-generation sequencing (NGS) approach for the clinical molecular diagnostics of RP. All known inherited retinal disease genes (n = 111) were captured and simultaneously analyzed using NGS in 100 RP patients without a molecular diagnosis. A systematic data analysis pipeline was developed and validated to prioritize and predict the pathogenicity of all genetic variants identified in each pa

Weill-Marchesani syndrome associated with retinitis pigmentosa.

Retinitis pigmentosa (RP) is associated with a wide variety of ocular and systemic disorders. The Weill-Marchesani syndrome is a multi-system disorder with microspherophakia as one of the common manifestations. A 14-year-old girl presented with short stature, short and stubby fingers, hypodontia and low-set ears. Slit-lamp examination revealed microspherophakia, with shallow anterior chambers with irido and phacodonesis. Ultrasonographic biomicroscopy confirmed the clinical findings and revealed hypoplastic ciliary body. Electroretinogram confirmed the diagnosis of RP. Though RP has been assoc

Retinitis pigmentosa: visual function and multidisciplinary management.

Retinitis pigmentosa (RP) is a leading cause of blindness and visual disability in younger people. Optometrists have a major role in detecting RP and in reducing the visual disability associated with RP. This review summarises the literature relating to visual function in people with RP, with particular attention given to night-blindness, visual acuity decrease and visual field contraction. The range of low vision aids available for people with RP is reviewed and suggestions given on aids that have been found to be most successful. Most importantly, this review overviews the range of services

Retinitis pigmentosa associated with Fuchs' heterochromic uveitis.

OBJECTIVE: To investigate whether the combination of Fuchs' heterochromic uveitis (FHU) and retinitis pigmentosa (RP) in the same patient is coincidental or represents a true association. METHODS: We have examined the frequency of FHU in 338 patients with RP and in 1984 patients who were seen in our primary care ophthalmic clinic because of reasons other than RP. RESULTS: Of 338 patients with RP, 4 (1.2%) had the typical findings of FHU. Three of them had Usher syndrome type II, and 1 had RP simplex. By contrast, only 1 patient in the control group had FHU (5%), and the difference in the fre

Comparative tolerability of therapies for cytomegalovirus retinitis.

Cytomegalovirus (CMV) retinitis is a potentially sight-threatening complication of advanced HIV infection. The acute infection can be controlled with one of several therapies, including intravenous ganciclovir, foscarnet or cidofovir, slow release ganciclovir intraocular implants or serial intraocular injections of ganciclovir or foscarnet. The initial induction course of therapy is typically followed by lifelong maintenance therapy. In addition to the aforementioned treatments, oral ganciclovir and intravitreal fomivirsen injections are other options for maintenance therapy. The choice of age

The epidemiology of CMV retinitis in Africa.

AIM: To analyze the prevalence of cytomegalovirus (CMV) retinitis in African AIDS patients. METHOD: A Medline survey was carried out to identify reports on the ocular manifestations of HIV infection in African patients. Relevant information on CMV retinitis was identified and analyzed. RESULTS: The reported prevalence of CMV retinitis in African AIDS patients varies from 0 to 8.5%. CONCLUSION: The low prevalence of CMV retinitis in Africa compared to Europe and the United States suggests that most African patients either die from other diseases before they enter the stage of severe immune d

Primary angle closure glaucoma and retinitis pigmentosa.

The prevalence of primary open angle glaucoma in patients with retinitis pigmentosa ranges from 2-12%. To the best of our knowledge the prevalence of primary angle closure glaucoma in retinitis pigmentosa patients has not been reported. This study was conducted to determine the prevalence of primary angle closure glaucoma in patients with retinitis pigmentosa and the relationship between the two. We reviewed 538 retinitis pigmentosa patients' charts seen between 1974-1987, in the Ocular Genetic Clinic in the Hospital of Sick Children, Toronto, Canada. The prevalence of primary angle closure gl

[Secondary prevention of cytomegalovirus retinitis with ganciclovir].

The prophylaxis of cytomegalovirus retinitis reactivation is effective in reducing the risk of blindness and in prolonging the remission interval and time of survival if given daily throughout life. In this study, a newly developed therapeutic regimen with 3 infusions a week was compared to the conventional maintenance therapy of 5 infusions a week using the same total weekly dose. For this purpose, ten patients were given 10 mg ganciclovir/kg 3 times a week (group A), and 18 received 6 mg ganciclovir/kg once daily for 5 days a week (group B). Only patients with newly diagnosed retinitis were

[Morphologic aspects of therapy-resistant cytomegalovirus retinitis].

Intravenous ganciclovir treatment was performed in eight male AIDS patients with primary unilateral CMV-retinitis. Three patients developed slowly progressive CMV-retinitis in the fellow eye despite adequate dose of ganciclovir. These different CMV-manifestations are shown in a sequence of fundus pictures. Three types of CMV-lesions were observed in connection with this study. Untreated central lesions showed the aspect of crumbled cheese and ketchup. Untreated lesions in the peripherie were yellowish-white, granular, "dry" and showed in most cases no haemorrhages. Lesions appearing during tre

Rhodopsin mutations in autosomal dominant retinitis pigmentosa.

DNA samples from 161 unrelated patients with autosomal dominant retinitis pigmentosa were screened for point mutations in the rhodopsin gene by using the polymerase chain reaction and denaturing gradient gel electrophoresis. Thirty-nine patients were found to carry 1 of 13 different point mutations at 12 amino acid positions. The presence or absence of the mutations correlated with the presence or absence of retinitis pigmentosa in 174 out of 179 individuals tested in 17 families. The mutations were absent from 118 control subjects with normal vision.

Clinical and genetic heterogeneity in retinitis pigmentosa.

The clinical course of defective vision and blindness has been investigated in relation to different modes of genetic transmission in a large series of 93 families with retinitis pigmentosa (RP). For autosomal dominant RP, two clinical subtypes could be distinguished according to the delay in macular involvement. In the severe form, macular involvement occurred within 10 years, while in the mild form, macular involvement occurred after 20 years. Interestingly, a significant increase of mean paternal age (38.8 years, mean controls in France = 29.1 years, P less than 0.001) was found in this for

Is taurine a marker for retinitis pigmentosa?

In view of contrary reports in the literature on anomalies of taurine concentration in the blood of retinitis pigmentosa patients, taurine clearance has been determined in 19 patients and 17 controls. No significant difference has been found between the average values of the two populations. The taurine content in cataractous crystalline lenses of retinitis pigmentosa patients has been found higher than the taurine content in lenses with senile cataract, but the same behaviour was also shown by five selected proteogenic amino acids. The results are discussed together with related literature da
