[Non-distinctive features on CT scan of fungal sinusitis].

OBJECTIVE: This article was a retrospective analysis on the preoperative CT scan of fungal sinusitis patients. The non-distinctive clinic feature was suggested to improve the accuracy of preoperative diagnosis. METHOD: A retrospective analysis was made for 176 patients with fungal sinusitis, which were confirmed by postoperative pathologic diagnosis from January 2000 to January 2008. Every patient was performed both CT examination and nasal endoscopy preoperatively. All patients underwent endoscopic sinus surgery and the lesions in sinus were cleared and confirmed fungal infection by pathology

The effects of choanal atresia on development of the paranasal sinuses and turbinates.

PURPOSE: The objective of this study was to evaluate the volume of paranasal sinuses (PNS) and turbinate in patients with unilateral choanal atresia (CA). MATERIALS AND METHOD: Computed tomography images of PNS in 11 individuals with unilateral CA were evaluated retrospectively. Mucosal thickness and volume of the maxillary, frontal and sphenoidal sinuses were determined, in addition to the volume of the middle and inferior turbinate. The unaffected nasal side of patients was used as a control group for the measurements. The results comprised the measurements of the atresic side compared to t

[Frontal ethmoid metastases of prostatic carcinoma. Report of one case and review of the literature].

Prostatic metastases in the nose and paranasal sinuses are rare. Less than 100 cases have been reported in the literature. Kidney are the commonest site of primary tumour, followed by lung and breast. Only 10 cases have previously been reported in the world literature. Prostatic metastases have been mainly reported in the sphenoid sinus. This paper reports one case of metastases of prostatic carcinoma in the fronto-ethmoid sinus in a 72 years old male. The clinical picture includes acute fronto-ethmoid right sinusitis, severe exophthalmos and chemosis. The CT scan showed extensive soft tissue

Sphenoid Sinus Volumes of Patients with Unilateral Cleft Lip and Palate.

OBJECTIVE: There is only one study in the literature in which 3D evaluation of sphenoid sinus volume in patients with cleft lip and palate was used. This shows that this issue has not been adequately evaluated. The present study aimed to compare the volume of the sphenoid sinuses in patients with cleft lip and palate on cone-beam computed tomography images, and to compare them with healthy controls. The high variability of sphenoid sinus volumes poses a risk for iatrogenic lesions during surgical procedures. Because the degree of pneumatization due to increased volume increases the likelihood

The anatomical relations of the sphenoid sinus and their implications in sphenoid endoscopic surgery.

Alongside the past development of endoscopic sinus surgery, knowledge about the anatomy of the sinuses has become crucial for surgeons. The sphenoid sinus is one of the most variable of all sinuses. Its relations to vital vascular and nervous elements make its approach a challenge for endoscopic surgeons. These relations include the internal carotid artery, the optic nerve, the pituitary gland, the Vidian nerve, and other vascular and nervous elements depending on the size of the sinus. This paper will make a review of these anatomical relations and their importance in endoscopic sphenoid surg

A Rare Presentation of Sphenoid Sinus Lymphoma in an 18-Year-Old Female.

Sphenoid sinus lymphomas are extremely rare, constituting a small fraction of both sphenoid sinus malignancies and extranodal non-Hodgkin lymphomas. Their clinical presentation can vary widely, ranging from symptoms resembling chronic sinusitis to more severe manifestations such as nerve palsies and visual disturbances. An 18-year-old female with no significant medical history presented with blurred vision, diplopia, and a left-sided headache lasting one week. Neurological examination revealed mild limitation of lateral rectus movement in the left eye. Imaging studies, including non-contrast C

Long-Term Results of Fibrous Dysplasia of the Paranasal Sinuses.

INTRODUCTION: Fibrous dysplasia (FD) is a benign, idiopathic skeletal disorder, in which the normal bone and marrow are replaced by fibrous tissue and immature woven bone. The authors aim to analyze the long-term results of patients diagnosed with FD of the paranasal sinuses via imaging or surgical biopsy. METHODS: The authors retrospectively enrolled 21 patients with FD of the paranasal sinuses from 2006 to 2021. RESULTS: The mean age at the time of diagnosis of FD of the paranasal sinuses was very high at 45.9 years. The most common site ofoccurrence was the sphenoid sinus (71.4%). During

Paranasal sinus radiology, Part 3B: Sphenoidal sinus.

Mucoceles involving the sphenoidal sinus are not as common as those affecting the frontal and ethmoidal sinuses. The subtle expansile change produced by a sphenoidal sinus mucocele often requires conventional tomography or computed tomography examination to be detected. Similarly, involvement of surrounding structures by a sphenoidal mucocele may require special study, which this section illustrates. The sphenoidal enlargement and density change produced by fibrous dysplasia may mimic sinus disease. Focal or widespread bone destruction resulting from neoplastic involvement of the sphenoidal si

Sphenoid sinus pneumatization: the good, the bad, and the beautiful.

PURPOSE: To study different types of extension of sphenoid sinus pneumatization detected by CT and examine their correlation with other anatomical variants. METHODS: Sphenoid sinus pneumatization was characterized (780 hemi-sinuses) on CT scans. Its incidence and correlations with related anatomical variants were evaluated. Dimensions of surgical window were measured in the lateral type, where pneumatization extends beyond a line connecting Vidian (pteregoid) canal (VC) and foramen rotundum (FR). RESULTS: The lateral recess of the sphenoid sinus was pneumatized in 27.3% of patients with a me

Symptomatic enophthalmos due to sphenoid wing dysplasia appearing over 12 years in a patient with neurofibromatosis type 1: a case report and literature review.

Sphenoid wing dysplasia (SWD) is a common orbital complication of neurofibromatosis type 1 (NF1). However, enophthalmos associated with SWD is extremely rare, and details of its natural history are unclear. We present the case of a 14-year-old boy with an early childhood diagnosis of NF1 presenting with left blepharophimosis and enophthalmos for several months. Imaging demonstrated enlargement of the left lateral SWD, progression of the posteromedial deviation of the orbital contents, and sphenoid/ethmoid sinus deformation due to left temporal lobe compression over 12 years. Two characteristic

Sinus headaches: avoiding over- and mis-diagnosis.

'Sinus headache' is a term used by many patients and primary-care physicians and, contrary to popular belief, sinus headaches are uncommon. Headaches that are due to sinusitis are confined to a minority of patients who have acute frontal sinusitis or sphenoiditis. The International Headache Society classification is robust in qualifying the term sinus headache and says that "chronic sinusitis is not validated as a cause of headache and facial pain unless relapsing into an acute stage". The vast majority of people who present with a symmetrical frontal or temporal headache, sometimes with an oc

Unexpected consequences of transnasal balloon dilation of the maxillary ostium.

BACKGROUND: Nasal irrigations and topical sprays have been demonstrated to have benefit in chronic sinusitis. Increasingly, it is evident that delivery system, patient anatomy, and inflammatory process have significant impacts on irrigant distribution. METHODS: Intrasinus endoscopy was performed during sinus irrigation of 5 thawed fresh frozen cadavers (10 sides) before and after transnasal dilation of the maxillary, frontal, and sphenoid ostia with a 5-mm balloon. RESULTS: Guidewire insertion created false passages through maxillary fontanelle on all of 10 attempts; 1 of 10 frontal insertio

Bony abnormalities of the paranasal sinuses in patients with Wegener's granulomatosis.

Wegener's granulomatosis is a systemic vasculitis that can affect any organ system, but primarily involves the upper and lower respiratory tracts and the kidneys. Chronic sinusitis is a well-known clinical feature of the disease. Mucosal abnormalities of the nose and paranasal sinuses have been well-characterized and range from granulomatous lesions to diffuse mucosal thickening. In contrast, abnormalities of the underlying bone of the paranasal sinuses in this disease have not been well-described, and reports have been limited. To characterize bony abnormalities of the nose and paranasal sinu

The Endonasal Endoscopic Approach to Different Sinonasal Fungal Balls.

BACKGROUND: Fungal ball sinusitis is a sinonasal fungus ball that usually affects immunocompetent adults with female predominance. The most affected sinus is the maxillary sinus. Aspergillus species is the most typically found fungus. Computed tomography (CT) scan is the gold standard tool in order to diagnose fungal ball sinusitis. The ultimate method for a fungal ball is functional endoscopic sinus surgery (FESS), which has a high success rate and a low morbidity rate. OBJECTIVE: This study aims to demonstrate the various clinical presentations of fungal ball sinusitis including isolated ma
