Massive Upper Extremity Hemihyperplasia Secondary to Neurofibromatosis.

CASE: A 30-year-old woman with type 1 neurofibromatosis presented with enlargement of the right upper extremity. The condition began in childhood and progressed until the limb reached a nonfunctional and burdensome size from elbow to fingertips. Radiographs showed elongated, thinned bones, and CT demonstrated diffuse soft tissue infiltration. Above-elbow amputation was performed. Pathology revealed diffuse neurofibromatosis of skin, subcutaneous tissues, and muscle without malignant change. Postoperatively, she was fitted with an aesthetic prosthesis and reported marked physical and emotional

Acute Compartment Syndrome of the Upper Extremity.

Acute compartment syndrome of the upper extremity is a rare but serious condition of elevated intracompartmental pressures leading to tissue ischemia. Prompt diagnosis and emergent fasciotomy are critical to optimize patient outcomes, and delays to care can result in myonecrosis, contracture, and limb dysfunction. Acute compartment syndrome of the upper extremity most commonly occurs in the forearm but may also affect the arm and the hand. Upper-extremity acute compartment syndrome most commonly occurs following trauma, but nontraumatic etiologies include prolonged decubitus position, bleeding

Upper Extremity Variant Vascular Anatomy: Navigation Techniques.

Upper limb approaches for diagnostic cardiac catheterization and percutaneous coronary intervention (PCI) have gained widespread adoption due to their advantages in reducing bleeding complications, improving patient comfort, and enabling earlier ambulation compared to transfemoral access. However, the success and safety of these procedures is heavily dependent on a thorough understanding of the vascular anatomy of the upper limb, particularly its numerous congenital and acquired variations. This article explores the spectrum of upper extremity vascular anomalies and their clinical implications

Management of the Mutilating Upper Extremity Injury.

Hand injuries are common due to environmental risks and require rapid, skilled management to preserve function, as the hand constitutes a significant portion of upper extremity and overall body utility. Severity scoring systems, while helpful, have limitations in predicting outcomes for complex upper extremity trauma. Initial care focuses on life and limb preservation, followed by detailed surgical intervention including irrigation, debridement, and reconstruction using advanced techniques such as grafts and free tissue transfers. Spare part and ectopic banking procedures are considered for se

Postoperative Psychiatric Outcomes After Upper-Extremity Amputations.

PURPOSE: Upper-extremity amputations alter patient form and, in cases of proximal amputation, may diminish overall function. Psychological outcomes following these procedures have been evaluated primarily on a single-institution basis. This study aimed to evaluate the incidence of psychiatric conditions following upper-extremity amputation on a population-wide basis. METHODS: The TriNetX database was queried for all patients treated for upper-extremity amputations between January 1, 2010, and December 31, 2022. Preoperative and date-of-surgery diagnoses were collected to identify conditions p

Alternative Flap Options for Upper Extremity Reconstruction.

For major upper limb defects, a wide range of established pedicled and free flap options can be used. These include the latissimus dorsi/thoracodorsal artery perforator, lateral arm, posterior interosseous artery, rectus abdominis, gracilis, and anterolateral thigh flaps. Technical proficiency is essential, and favorable success rates in terms of functional and esthetic outcomes can be achieved. Herein, alternative flap options (both pedicled and free) are introduced and discussed through a few illustrative case examples.

Targeted muscle reinnervation in upper extremity amputations.

Targeted muscle reinnervation (TMR) is a relatively recent surgical innovation that involves the coaptation of major peripheral nerves to a recipient motor branch that innervates an expendable muscle target. The original indication for TMR was augmentation and optimization of myoelectric signals in the amputated limb for use of myoelectric prosthetics. Incidentally, surgeons and patients discovered that the technique also could treat and prevent phantom and residual limb pain. TMR is performed at the time of amputation or delayed any time after the amputation, and TMR can also be performed at

Double Crush Syndrome of the Upper Extremity.

»: The double crush hypothesis originally proposed that compression of a peripheral nerve at a proximal site increases its susceptibility to additional distal lesions. »: The etiology of double crush syndrome is likely multifactorial as mechanical, systemic, pharmacologic, or environmental factors may increase a patient's susceptibility to this syndrome. »: To date, there remains no standardized definition of double crush syndrome, and there is no consensus on its exact pathophysiology or diagnostic criteria. »: Patients with double crush syndrome should be counseled that surgical repair ma

Upper extremity Histoplasma capsulatum treatment with isavuconazole.

Extrapulmonary Histoplasma capsulatum infections in the immunocompetent population are rare and pose a diagnostic challenge. Upper extremity histoplasmosis without a primary lung infection is uncommon. It is possible to acquire it by inadvertent trauma with direct inoculation. Our case describes an immunocompetent patient with progressive swelling with minimal pain in the wrist associated with a small puncture wound on the left dorsal forearm. The initial workup failed to identify a specific etiology. For the following six weeks, the patient experienced progressive worsening of symptoms, warra

Free Tissue Transfer for Upper Extremity Reconstruction.

With the advent of the Industrial Revolution, traumatic injuries of the upper extremity increased exponentially. As a result, surgeons began to reevaluate amputation as the standard of care. Following the Second World War, local and regional pedicled flaps became common forms of traumatic upper extremity reconstruction. Today, microsurgery offers an alternative when options lower on the reconstructive ladder have been exhausted or will not produce a desirable result. In this article, the authors review the use of free tissue transfer for upper extremity reconstruction. Flaps are categorized as

Bone Graft Options in Upper-Extremity Surgery.

Bone grafting in the upper extremity is an important consideration in patients with injuries or conditions resulting in missing bone stock. A variety of indications can necessitate bone grafting in the upper extremity, including fractures with acute bone loss, nonunions, malunions, bony lesions, and bone loss after osteomyelitis. Selecting the appropriate bone graft option for the specific consideration is important to ensure optimal patient outcomes. Considerations such as donor site morbidity and the amount and characteristics of bone graft needed all weigh in the decision making regarding w

Atypical Mycobacterial Infections of the Upper Extremity.

Atypical mycobacterial infections of upper extremity synovial-lined structures are often misdiagnosed and unrecognized. Despite an increasing incidence, lack of physician awareness of these pathogens may result in considerable delay in diagnosis and management, potentially leading to permanent disability. The authors conducted a literature review and analyzed 31 cases of penetrating atypical mycobacterial infection to better understand the clinical characteristics and to evaluate their posttreatment complication rate compared with available literature. Medical records for culture-positive case

Management of Upper Extremity Injury in Divers.

Modern competitive diving-especially platform diving-applies dramatic stress to the upper extremity. Some stress occurs during handstand-style takeoffs, but more force is delivered to the hand, wrist, elbow, and shoulder during hands-first entry. Hand positions that minimize the amount of splash result in forceful wrist extension. This repetitive impact can lead to chronic upper extremity pain and acute injuries that require operative intervention. Many divers use taping and bracing to prevent or treat this type of injury, but these are only modestly effective. Although minor injuries can impr

Treatment of the Upper Extremity Contracture/Deformities.

Patients with arthrogryposis multiplex congenita have a characteristic upper extremity resting posture consisting of internal rotation of the shoulders, elbow extension, flexed wrists, thumb-in palm deformities, and variable degrees of finger contractures. Treatment of these patients is aimed at improving independence and performance of activities of daily living. Although each area needs to be assessed independently for the most appropriate surgical procedure, often multiple areas can be addressed at the same operative setting. This limits the number of anesthetic exposures and cast immobiliz

Morel-Lavallee Lesion in the Upper Extremity.

Background: The Morel-Lavallee lesion (MLL) is a closed internal degloving injury that results from shearing of the skin and subcutaneous tissue from the underlying fascia. Disruption of the perforating blood vessels and lymphatics results in a lesion filled with serosanguinous fluid and necrotized fat. MLLs are most commonly described in association with pelvic and lower extremity trauma, and there are limited reports of these lesions in other locations. Methods: This case report describes a 58-year-old male referred from his primary care physician with a soft tissue mass in the upper arm. Ca

Imaging the Injured Pediatric Athlete: Upper Extremity.

Increased physical activity in childhood has resulted in a large number of sports-related injuries. Although there is overlap between the sports-related injuries seen in pediatric and adult patients, important differences exist in the injury patterns of pediatric patients. These differences are related to the continuous changes in the developing skeleton and its relationship with adjacent soft tissues. The imbalance in strength between the growing bones and the nearby tendons and ligaments makes the bones prone to acute and chronic injuries. Acute injuries are more common in contact sports suc

Trans-femoral thromboaspiration for upper extremity ischemia.

INTRODUCTION: Endovascular trans-femoral access catheter aspiration of thrombus within the proximal subclavian, brachial, radial and ulnar arteries for symptomatic upper extremity ischemic pain has not been previously reported. We describe a case in which a successful clinical outcome was achieved using long length neuro-endovascular aspiration catheters. PRESENTATION OF CASE: A 45 year old female presented with diffuse left upper limb pain. Sonography revealed compromised upper extremity blood flow. Thrombus was identified in the proximal left subclavian artery by CT angiography. Surgical re
