Characteristics of the ventricular insertion sites of accessory pathways with anterograde decremental conduction properties.

BACKGROUND: Accessory pathways (APs) with anterograde decremental conduction properties referred to as Mahaim fibers have recently been recognized as originating from the right lateral atrium. Little information is available about their distal insertion. The purpose of this study was to determine the different kinds of APs involved and the characteristics of their distal insertion site. METHODS AND RESULTS: Twenty-one patients (mean age, 28 +/- 13 years) with reciprocating tachycardia or atrial fibrillation were studied. Right-sided atrial and/or ventricular endocardial mapping during tachyca

Concealed Accessory Pathways with a Single Ventricular and Two Discrete Atrial Insertion Sites.

BACKGROUND: Atrioventricular reciprocating tachycardia (AVRT) utilizing a concealed accessory pathway is common. It is well appreciated that some patients may have multiple accessory pathways with separate atrial and ventricular insertion sites. METHODS: We present three cases of AVRT utilizing concealed pathways with evidence that each utilizing a single ventricular insertion and two discrete atrial insertion sites. RESULTS: In case one, two discrete atrial insertion sites were mapped in two separate procedures, and only during the second ablation was the Kent potential identified. Ablation

Cardioneural and accessory atrioventricular pathway ablation for vagally mediated atrial fibrillation in PRKAG2 syndrome.

Cardioneural ablation (CNA) is an emerging strategy for vagally mediated arrhythmias, but its role in patients with underlying conduction system disease remains undefined. Here, a young patient with PRKAG2-mediated cardiomyopathy presented with refractory paroxysmal atrial fibrillation (AF) despite prior accessory atrioventricular pathway (AP) ablation and antiarrhythmic therapy. Episodes of AF were triggered consistently by cold milk ingestion, and dual-chamber pacemaker interrogation revealed a high atrial pacing burden due to sinus pauses, suggesting a vagally mediated mechanism superimpose

A Single-lead ECG algorithm to differentiate right from left manifest accessory pathways: A reappraisal of the P-Delta interval.

INTRODUCTION: Despite numerous ECG algorithms being developed to localize the site of manifest accessory pathways (AP), they often require stepwise multiple-lead analysis with variable accuracy, limitations, and reproducibility. The study aimed to develop a single-lead ECG algorithm incorporating the P-Delta interval (PDI) as an adjunct criterion to discriminate between right and left manifest AP. METHODS: Consecutive WPW patients undergoing electrophysiological study (EPS) were retrospectively recruited and split into a derivation and validation group (1:1 ratio). Sinus rhythm ECG analysis i

Simple electrocardiography algorithm for localizing accessory pathway in patients with Wolff-Parkinson-White syndrome.

BACKGROUND: Existing algorithms to predict the location of an accessory pathway (AP) in Wolff-Parkinson-White Syndrome (WPW) have good sensitivity and specificity but complex with various accuracy and inter-observer agreement rates. A simple algorithm with high accuracy and inter-observer agreement rates is needed. METHODS: This was a cross-sectional and retrospective diagnostic study. The data were collected by total population sampling from January 2015 to January 2017. Forty-seven patients were included in the study. Data collected were pre-ablation 12-lead ECGs and ablation reports. These

Ventricular ectopy following accessory pathway ablation in WPW syndrome.

INTRODUCTION: Many Authors already described automatic activity arising from accessory pathways, but the underlying mechanism has not been clarified yet. They may be due to embryogenetic features of myocardium or may be related to specific excitability during radiofrequency ablation. CASE: Our report shows that ventricular accelerated rhythm may transiently arise from the ventricular edge of a common myocardium made accessory pathway right after the ablation. No further action were required in our experience, since the phenomenon self extinguished in approximately hour. CONCLUSIONS: If this

Forensic significance of conduction system abnormalities as a precise cause of accidental death.

We examined the pathology findings of the cardiac conduction system in 42 human autopsies who were considered to have died an accidental death. Abnormalities in serial sections of the conduction system were found in seven and of these three had an accessory pathway in the conduction system. In one of these there was mild Ebstein's anomaly (ME), two had fibromuscular dysplasia of the atrioventricular node artery with mitral valve prolapse, and two others had abnormal routes showing fragmentation of the bundle of His with ME. A full investigation of the cardiac conduction system can therefore be

Electrophysiological Characteristics of Coronary Sinus Activation Sequence Alteration During Left Lateral AP Ablation.

BACKGROUND: The occurrence of a left free-wall (LFW) accessory pathway (AP) with concentric activation sequences in the coronary sinus (CS) during orthodromic atrioventricular reentrant tachycardia (OAVRT) is an uncommon phenomenon. METHODS: Using three-dimensional (3D) mapping system in eight patients with LFW-AP (mean age 44±9.9 years), we systematically analyzed mitral annular (MA) activation during OAVRT. RESULTS: The primary location of the APs was in the left lateral region in seven patients. The tachycardia, characterized by varying CS sequences, remained sustainable in three patients

Pre-excitation syndrome with a change in terminal QRS vector.

Most accessory pathways in pre-excitation syndrome are capable of antegrade conduction from atrium to ventricle, and identified by characteristic ECG abnormalities such as a delta wave (ventricular pre-excitation) and a shortened PR interval. Therefore, the traditional diagnosis method is to detect the initial changes of the QRS complex. Here we report a patient initially mistaken as having atrioventricular re-entrant tachycardia (AVRT) and concealed pre-excitation syndrome. A diagnosis of incomplete latent pre-excitation syndrome was established on the basis of differences between the ECGs dur

Differential entrainment distinguishes atrioventricular nodal reentry tachycardia from atrioventricular reentrant tachycardia.

BACKGROUND: Entrainment from the right ventricular (RV) apex and the base has been used to distinguish atrioventricular reentrant tachycardia (AVRT) from atrioventricular nodal reentry tachycardia (AVNRT). The difference in the entrainment response from the RV apex in comparison with the RV base has not been tested. METHODS: Fifty-nine consecutive patients referred for ablation of supraventricular tachycardia (SVT) were included. Entrainment of SVT was performed from the RV apex and base, pacing at 10-40-ms faster than the tachycardia cycle length. SA interval was calculated from stimulus to

Comparison of cryoablation with 3D mapping versus conventional mapping for the treatment of atrioventricular re-entrant tachycardia and right-sided paraseptal accessory pathways.

UNLABELLED: Aim Transcatheter cryoablation is a well-established technique for the treatment of atrioventricular nodal re-entry tachycardia and atrioventricular re-entry tachycardia in children. Fluoroscopy or three-dimensional mapping systems can be used to perform the ablation procedure. The aim of this study was to compare the success rate of cryoablation procedures for the treatment of right septal accessory pathways and atrioventricular nodal re-entry circuits in children using conventional or three-dimensional mapping and to evaluate whether three-dimensional mapping was associated with

Septal VA Time: How Short Is Too Short for Atrioventricular Re-Entrant Tachycardia?

BACKGROUND: A septal ventriculoatrial interval (VAI) of <70 ms often excludes atrioventricular reentrant tachycardia (AVRT) as a mechanism of supraventricular tachycardia (SVT). CASE SUMMARY: A 49-year-old woman presented with symptomatic Wolff-Parkinson-White syndrome. Her electrophysiology study showed an AVRT with an unusually short VAI of 45 ms. The difference between the atrial signal interval during right ventricular pacing in sinus rhythm and the VAI during SVT was 30 ms. A functional left bundle branch block was induced, with a VAI of 85 ms (40 ms longer than the narrow complex SVT),

Cryosurgical ablation of accessory atrioventricular connections: a method for correction of the pre-excitation syndrome.

Cryothermia, a new technique for definitive treatment of the pre-excitation syndrome, is described in two patients. The first patient presented with a normal P-R interval with a delta wave and reciprocating tachycardia. Preoperative electrophysiologic study suggested a free-wall atrioventricular connection on the left posterior atrioventricular (A-V) groove. At surgery, epicardial mapping confirmed the site of pre-excitation on the posterior left ventricular (LV) wall. An electrogram arising from the accessory pathway (AP) was recorded at the site of earliest ventricular activation. Interatria

Surgical alternatives for supraventricular tachycardias.

Since Sealy's pioneering surgical intervention for Wolff-Parkinson-White syndrome, surgical electrophysiologic interventions have been developed for all supraventricular arrhythmias. The surgical rationales are based on the site of origin of the arrhythmic mechanism and the associated pathology that characterizes the "arrhythmogenic substrate." The Wolff-Parkinson-White syndrome is characterized by an accessory atrioventricular (AV) connection distinct from the AV node-His bundle system. It is associated with AV reentrant tachycardia or atrial fibrillation, or both, with fast ventricular respo

Anterograde conduction to the His bundle during right ventricular overdrive pacing distinguishes septal pathway atrioventricular reentry from atypical atrioventricular nodal reentrant tachycardia.

BACKGROUND: Distinguishing orthodromic atrioventricular reciprocating tachycardia (ORT) using a retrograde septal accessory pathway (AP) from atypical atrioventricular nodal reentrant tachycardia (AVNRT) may be challenging. Specifically, excluding the presence and participation of an AP may require multiple diagnostic maneuvers. OBJECTIVE: The purpose of this study was to assess the relative value of commonly used right ventricular (RV) pacing maneuvers, including identification of anterograde His-bundle activation with entrainment, to differentiate ORT using a retrograde septal AP from atypi

[Interventional rhythmology].

Interventional cardiorhythmology was born with the invention of electrical catheter ablation of the common atrioventricular bundle of His as a palliative treatment of supraventricular arrhythmias refractory to medicinal treatment. This method is now used as a curative treatment. In Wolff-Parkinson-White syndrome, all accessory pathways, whatever their location, can be destroyed with a very high success rate (96 p. 100) and very low morbidity and mortality rates. Reentrant nodal tachycardias can also be treated by catheter ablation with, however, a low risk of atrioventricular block which, for

Zero Fluoroscopy Catheter Ablation of Accessory Pathways and AV Nodal Re-Entry Tachycardia: A Single Institution Study.

BACKGROUND: Minimizing radiation exposure during electrophysiological studies remains an important priority in the catheterization laboratory. With the advancement of imaging and electroanatomic mapping systems, eliminating fluoroscopy from catheter ablation of arrhythmias is becoming more feasible, even for left side procedures requiring transseptal puncture. Nevertheless, there remain concern over the safety and efficacy of a zero-fluoroscopy approach. OBJECTIVES: The aim of this report was to evaluate the safety and efficacy of a zero-fluoroscopy catheter ablation approach for accessory pa

Transcatheter cryoablation for atrioventricular nodal reentrant tachycardia and septal accessory pathways: Experience from a tertiary center.

INTRODUCTION AND OBJECTIVES: For atrioventricular nodal tachycardia (AVNRT) and right-septal anatomically high-risk accessory pathways (AP) with risk of atrioventricular (AV) block, cryoablation is accepted as a safer option. The aim was to evaluate the efficacy, safety and recurrence rates of focal catheter cryoablation for the treatment of AVNRT and right-sided septal AP. METHODS: We performed a retrospective single-center study of patients undergoing focal cryoablation between January 2007 and May 2025 with the electrophysiologic diagnosis of AVNRT or AP based on electrophysiological crite
