Leg ulcers: uncommon presentations.

Uncommon presentations like vasculitis or other immunologic causes and malignancy account for about 1% to 2% of patients suffering from leg ulcers. We focus on such uncommon leg ulcers including: cutaneous vasculitis causing cutaneous ulceration, other immunologic or metabolic cutaneous lesions such as pyoderma gangrenosum and necrobiosis lipoidica, and ulcers based on neoplastic etiology. A short description on leg ulcers in the tropics is also included. The described uncommon presentations of leg ulcers are typically difficult to diagnose and treat; it is a specialist's job to take care of p

Adoption of Health Information Technologies by Area Socioeconomic Deprivation Among US Hospitals.

IMPORTANCE: Access to and quality of care vary substantially by area socioeconomic status. Expanding hospital health information technology (HIT) adoption may help reduce these disparities, given hospitals' central role in serving underserved populations. OBJECTIVE: To examine variations in US hospital adoption of telehealth and health information exchange (HIE) functionalities by hospital service area (HSA) socioeconomic deprivation. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study links data from the 2018-2023 American Hospital Association Annual Survey and Information Technol

Autonomic dysfunction in the neurological intensive care unit.

Autonomic dysfunction is common in neuro-critical care patients and may compromise the function of various organs. Among the many diseases causing or being associated with autonomic dysfunction are traumatic brain injury, cerebrovascular diseases, epilepsy, Guillain-Barré syndrome (GBS), alcohol withdrawal syndrome, botulism and tetanus, among many others. Autonomic dysfunction may afflict various organs and may involve hyper- or hypo-activity of the sympathetic or parasympathetic system. In this short overview, we address only a small number of neuro-intensive care diseases with autonomic dys

Short term urinary catheterized patients in intensive care unit (ICU)--a need to screen.

Catheter-Associated Urinary Tract Infection (CAUTI) is the second most commonly reported healthcare associated infection, accounting for upto 40% of all nosocomial infections. Silent catheter associated bacteriuria comprises a huge reservoir of resistant organism in hospitals particularly in critical care units. We prospectively studied 100 newly short-term catheterized patients in a tertiary care hospital. Samples collected on day three and daily thereafter for semi quantitative urine culture and pus cell count. Antibiotic sensitivity was tested using Kirby-Bauer disc diffusion method. Out of

Identifying New Positives and Linkage to HIV Medical Care--23 Testing Site Types, United States, 2013.

Among the estimated 1.2 million persons living with human immunodeficiency virus (HIV) infection in the United States, approximately 14% have not had their HIV diagnosed. Certain populations, such as African Americans/blacks (in this report referred to as blacks), men who have sex with men (MSM), and Hispanics/Latinos (in this report referred to as Hispanics), are disproportionately affected by HIV. In areas where HIV prevalence is ≥0.1%, CDC recommends routine HIV screening in health care settings for persons aged 13-64 years. Implementation of HIV screening as part of routine care can increa

Analysis of hospital charges after simultaneous pancreas-kidney transplantation in the era of managed care.

BACKGROUND: The purpose of this study was to analyze and compare hospital charges in simultaneous pancreas-kidney transplant (SPKT) recipients before and after implementation of managed care principles. METHODS: Two groups were compared: 14 consecutive SPKT patients transplanted in 1991 vs. 15 consecutive SPKT patients transplanted in 1995. All patients underwent whole organ pancreas transplantation with bladder drainage and received quadruple immunosuppression with OKT3 induction. The two groups were well-matched; outliers were excluded (four in 1991 and five in 1995), and no attempt was mad

Effective healthcare coverage in Canada: The caring and responding in Edmonton project.

BACKGROUND: Despite the progress towards Universal Health Coverage (UHC) in Canada, individuals experiencing socioeconomic disadvantages continue to face barriers to accessing necessary health services. This study explored the observational insights of social care providers (SCPs), who regularly engage with vulnerable populations and healthcare systems, to better understand these barriers in Edmonton, Canada. METHODS: A qualitative descriptive design was employed, guided by COREQ criteria. Purposive sampling recruited SCPs from 22 community organizations serving Indigenous community members a

The role of extracorporeal membrane oxygenation in donation after circulatory death.

Donor scarcity and the increased need for organ transplantation has prompted the development of an alternative source of donors to the more conventional brain dead donor. While in a Beating-Heart donor, abdominal and intrathoracic organs are perfused, in a non-beating heart donor (NHBD, or DCD), perfusion should be maintained, after confirmation of death, by means of ECMO and inflation of intra-aortic balloon accordingly to the localization of the organs that should be transplanted. In this setting, ECMO allows selective perfusion of the organs which should be transplanted ("compartmental ECMO

Incidence of acute gastroenteritis and role of norovirus, Georgia, USA, 2004-2005.

Approximately 179 million cases of acute gastroenteritis (AGE) occur annually in the United States. However, lack of routine clinical testing for viruses limits understanding of their role among persons seeking medical care. Fecal specimens submitted for routine bacterial culture through a health maintenance organization in Georgia, USA, were tested with molecular diagnostic assays for norovirus, rotavirus, astrovirus, sapovirus, and adenovirus. Incidence was estimated by using national health care utilization rates. Routine clinical diagnostics identified a pathogen in 42 (7.3%) of 572 specim

The Population Health Value Framework: Creating Value by Reducing Costs of Care for Patient Subpopulations With Chronic Conditions.

PROBLEM: With the growth in risk-based and accountable care organization contracts, creating value by redesigning care to reduce costs and improve outcomes and the patient experience has become an urgent priority for health care systems. APPROACH: In 2016, UCLA (University of California, Los Angeles) Health implemented a system-wide population health approach to identify patient populations with high expenses and promote proactive, value-based care. The authors created the Patient Health Value framework to guide value creation: (1) identify patient populations with high expenses and reasons f

Patient Centered Medical Home: What May Be on the Horizon for Urologists.

INTRODUCTION: Medicine has used acronyms for hundreds of years to describe clinical conditions. METHODS: In urology the acronyms BPH (benign prostatic hyperplasia), ED (erectile dysfunction), IC (interstitial cystitis) and AUR (acute urinary retention) are examples with which we are all familiar. Today we use acronyms to describe the organizational and structural aspects of medicine. RESULTS: The 2 most recent medical acronyms are PPACA (Patient Protection and Affordable Care Act) and ACO (accountable care organization). Now a new acronym, PCMH (patient centered medical home), is looming on

[Fatal haemorrhagic rift valley fever: a case at Madagascar].

Rift valley fever (RVF) is a viral zoonosis that can also infect humans. Haemorrhagic RVF is a severe potentially fatal form of the disease. Although haemorrhagic RVF accounts for only 1% of all infections, death occurs in up to 5% of cases. The purpose of this report is describe a severe case of haemorrhagic RVF observed in a 22-year-old cattle breeder admitted to the intensive care units of the Joseph Raseta Befelatanana University Hospitals in Antananarivo. The disease presented as an infectious syndrome but hemorrhagic manifestations developed early (day 2). They consisted of diffuse haemo

Psychogenic stridor: diagnosis and management.

Psychogenic stridor is a rare cause of apparent acute upper airway obstruction, usually occurring in young female adults. The alarming presence of inspiratory stridor leads to suspicion of organic disease, with urgent management directed towards treatment of suspected pathology. Careful history taking may suggest the correct diagnosis and early examination of the larynx, preferably by fibreoptic nasendoscopic examination, may identify the underlying abnormality of paradoxical adduction of the vocal cords on inspiration, accounting for the stridor. This characteristic feature allows a positive

Epidemiological transition in Morocco (1960-2015).

Morocco is undergoing the third phase of its epidemiological transition. Mortality indicators have declined significantly. Life expectancy at birth has increased by 23 years over this 55-year study period. The total fertility rate has dropped from 7.06 in 1960 to 2.89 in 2015. The country is going through the epidemiological transition characterized by a shift in the overall burden of morbidity and mortality from infectious diseases to noncommunicable diseases and injuries. Chronic diseases now account for 75 % of all deaths. Cardiovascular diseases, diabetes, and cancer are among the leading

Co-Designing an Initiative to Increase Shared Access to Older Adults' Patient Portals: Stakeholder Engagement.

BACKGROUND: The patient portal is a widely available secure digital platform offered by care delivery organizations that enables patients to communicate electronically with clinicians and manage their care. Many organizations allow patients to authorize family members or friends-"care partners"-to share access to patient portal accounts, thus enabling care partners to receive their own identity credentials. Shared access facilitates trilateral information exchange among patients, clinicians, and care partners; however, uptake and awareness of this functionality are limited. OBJECTIVE: We part

Hemophagocytic Lymphohistiocytosis Associated with Synergistic Defects of AP3B1 and ATM Genes: A Case Report and Literature Review.

Hemophagocytic lymphohistiocytosis (HLH) is an overwhelming immune system activation that manifests as hyperinflammation and life-threatening multiple organ failure. However, the clinical manifestations of the systemic inflammatory response in sepsis and fulminant cytokine storm caused by HLH macrophage activation are very similar and difficult to distinguish. HLH triggered by two novel gene defects manifesting with multiorgan dysfunction syndrome (MODS) and distributive shock has not been reported. A 14-year-old male patient was hospitalized with a high fever, his condition deteriorated rapid

Therapeutic objectives and their practical achievement in type 2 diabetes.

This paper offers a broad but selective account of the context of type 2 diabetes at the start of the new millennium. It outlines the major long-term threats to health and life of people with type 2 diabetes and indicates that, while their relative impacts may differ, the burden of the specific "microvascular" and the atherosclerotic "macrovascular" complications of diabetes weigh as heavily on people with type 2 as on those with type 1 diabetes. Correction of hyperglycemia still has a leading role among therapeutic objectives in type 2 diabetes, but the concept of the "defence in depth" and t
