Brain abscess caused by multidrug-resistant Acinetobacter baumannii. Case report.

This 24-year-old soldier had a history of polytrauma caused by firearm missiles of a fragmentation weapon. He was referred to the Hospital Militar Central, where multiple shrapnel wounds in the head, face, thorax, and extremities were found. A brain abscess was documented and drained, and a culture grew a multidrug-resistant Acinetobacter baumannii. An appropriate antibiotic treatment was started but did not lead to a good response, and the patient died. The clinical course of the illness is presented, as is its treatment and the role of A baumannii as an etiological agent of a brain abscess.

[Activity of new fluoroquinolones against clinical isolates of Acinetobacter baumannii].

Acinetobacter baumannii is an opportunistic pathogen associated with numerous nosocomial infections. In recent years it has shown extraordinary ease in developing resistance to most antimicrobial agents, which is a serious problem as it makes these infections difficult to treat. We determined the in vitro activity of eight quinolones, five betalactam agents and colistin in 160 clinical isolates of A. baumannii. In general, we observed a high rate of resistance to the quinolones (90%), excluding clinafloxacin (25%), and to ampicillin-sulbactam (61.25%) and imipenem (50%). Colistin is the agent

Treatment of multidrug-resistant Acinetobacter baumannii meningitis with ampicillin/sulbactam.

The clinical features and the outcomes of eight cases of nosocomial Acinetobacter baumannii meningitis treated with ampicillin/sulbactam are reported. All the patients had fever, neck stiffness or meningeal signs, and a low consciousness level, and in their cerebrospinal fluid (CSF), pleocytosis, a low glucose level, and an elevated protein level were noted. For all CSF isolates of A. baumannii, the MIC of ampicillin/sulbactam was < or = 8/4 microg/mL. The MICs of sulbactam by microdilution in two cases were 4 microg/mL. All isolates were resistant to cefotaxime, ceftriaxone, ceftazidime, urei

Clinical evaluation of succinylated Acinetobacter glutaminase-asparaginase in adult leukemia.

We treated 13 adult patients with acute leukemia or chronic myelocytic leukemia (CML) in blast phase using succinylated Acinetobacter glutaminase-asparaginase (SAGA) administered on a daily dose schedule. SAGA reduced the peripheral blast count in two patients with acute lymphoblastic leukemia and two with blastic CML; however, no patient achieved either complete or partial remission. Marked central nervous system toxic effects (encephalopathy and coma) were observed, limiting treatment in patients whose disease appeared responsive; this effect finally prompted early discontinuance of the tria

Phase I evaluation of succinylated Acinetobacter glutaminase-asparaginase in adults.

Succinylated Acinetobacter glutaminase-asparaginase (SAGA) has broader antitumor activity than Escherichia coli L-asparaginase in experimental systems; moreover, drug resistance does not develop in tumor cell lines initially sensitive to this enzyme. We have investigated the pharmacology and toxicology of SAGA after both single-dose and serial daily dose injections in 20 adult patients. Glutaminase activity in plasma after i.v. injection of single doses did not follow simple first-order kinetics (half-life during the initial 24 hr was 21 +/- 9 hr. A linear relation was observed between increas

Complete Genome Sequencing of Acinetobacter baumannii AC1633 and Acinetobacter nosocomialis AC1530 Unveils a Large Multidrug-Resistant Plasmid Encoding the NDM-1 and OXA-58 Carbapenemases.

Carbapenem-resistant Acinetobacter spp. are considered priority drug-resistant human-pathogenic bacteria. The genomes of two carbapenem-resistant Acinetobacter spp. clinical isolates obtained from the same tertiary hospital in Terengganu, Malaysia, namely, A. baumannii AC1633 and A. nosocomialis AC1530, were sequenced. Both isolates were found to harbor the carbapenemase genes blaNDM-1 and blaOXA-58 in a large (ca. 170 kb) plasmid designated pAC1633-1 and pAC1530, respectively, that also encodes genes that confer resistance to aminoglycosides, sulfonamides, and macrolides. The two plasmids wer

The Acinetobacter Outer Membrane Contains Multiple Specific Channels for Carbapenem β-Lactams as Revealed by Kinetic Characterization Analyses of Imipenem Permeation into Acinetobacter baylyi Cells.

The number and type of outer membrane (OM) channels responsible for carbapenem uptake in Acinetobacter are still not well defined. Here, we addressed these questions by using Acinetobacter baylyi as a model species and a combination of methodologies aimed to characterize OM channels in their original membrane environment. Kinetic and competition analyses of imipenem (IPM) uptake by A. baylyi whole cells allowed us to identify different carbapenem-specific OM uptake sites. Comparative analyses of IPM uptake by A. baylyi wild-type (WT) cells and ΔcarO mutants lacking CarO indicated that this OM

Characterization and depolymerase activity analysis of Acinetobacter baumannii bacteriophage vB_AbaP_IME546.

In this study, we selected Acinetobacter baumannii Ab2897, a strain from our laboratory, as the host, while the lytic bacteriophage vB_AbaP_IME546 was isolated from untreated hospital sewage. We examined the phage's biological characteristics and found that its latent period was under 5 min, with complete lysis of the bacteria occurring within 60 min. The plaques formed by the bacteriophage showed a halo, indicating the presence of an antibacterial protein. Consequently, this phage underwent next-generation sequencing. A BLAST analysis of the assembled genome sequence indicated that vB_AbaP_IM

Global strategies to fight carbapenem-resistant Acinetobacter baumannii (CRAB) infections.

Carbapenem-resistant Acinetobacter baumannii (CRAB) has emerged as a formidable healthcare-associated pathogen, particularly in intensive care units, where it causes severe infections associated with prolonged hospitalization, limited treatment options, and high mortality. This review provides an integrated analysis of the global epidemiology of CRAB, the molecular mechanisms driving carbapenem resistance, and the increasing clinical significance of antibiotic hetero-resistance, which contributes to diagnostic failure and poor therapeutic outcomes. We highlight advances in diagnostic technolog

Therapeutic challenges and emerging strategies against carbapenem-resistant Acinetobacter baumannii.

Carbapenem-resistant Acinetobacter baumannii (CRAB) remains a formidable clinical challenge, driven by limited therapeutic options and multifaceted resistance mechanisms. While current polypharmacy regimens require evidence-based refinement, several emerging antimicrobials offer renewed hope. Sulbactam-durlobactam has demonstrated superior clinical resolution in CRAB-induced pneumonia and meningitis compared to traditional polymyxin-based regimens. Eravacycline achieves 100% microbiological clearance in abdominal infections without promoting resistance, and cefiderocol retains activity against

Virulence arsenal of Acinetobacter baumannii: mechanisms driving persistence and resistance.

Acinetobacter baumannii is a Gram-negative opportunistic pathogen and a major cause of healthcare-associated infections, including ventilator-associated pneumonia, bacteraemia, meningitis, and urinary tract infections. Its persistence in hospital environments is due to its ability to survive desiccation, resist disinfectants, and colonize both biotic and abiotic surfaces. Virulence in A. baumannii is largely associated with structures such as Csu pili, biofilm-associated protein (Bap), and outer membrane protein A (OmpA), which enable surface attachment, biofilm formation, and host cell damage

Disruption of rcnB modulates colistin susceptibility in Acinetobacter baumannii AB5075.

Acinetobacter baumannii AB5075 is a clinically relevant multidrug-resistant (MDR) isolate that poses a major therapeutic challenge. Although colistin has been reinstated as a last-resort antibiotic against MDR Gram-negative infections, the rapid emergence of colistin resistance threatens its clinical utility. Here, we employed a CRISPR-Cas9-based genome editing system to generate an A. baumannii AB5075 ΔrcnB mutant and uncovered a previously underappreciated role of rcnB in modulating colistin susceptibility. Loss of rcnB markedly potentiated colistin-mediated killing through multiple associat

Quercetin as tigecycline adjuvant reverses multidrug-resistant Acinetobacter baumannii infection.

The rise and global dissemination of multidrug-resistant (MDR) Acinetobacter baumannii has become a major public health threat. However, the constant emergence of drug-resistant mutants has resulted in increasingly limited therapeutic options. Tigecycline (TGC) continues to be one of the few antibiotics still effective against many A. baumannii strains. Herein, we discovered that quercetin (Qu) synergizes with TGC, enhancing its therapeutic efficacy against MDR A. baumannii by reducing the minimum inhibitory concentration (MIC) by 4- to 32-fold. Mechanistic analysis revealed that the synergist
