Evaluation of a new treatment strategy for Paget-Schroetter syndrome: spontaneous thrombosis of the axillary-subclavian vein.

PURPOSE: The purpose of this study was to evaluate the efficacy of a staged, multimodal algorithm of therapy for durable correction of Paget-Schroetter syndrome. METHODS: Fifty consecutive patients were entered into a sequential treatment program for spontaneous axillary-subclavian vein thrombosis. Forty-three had initial thrombolytic or anticoagulant treatment followed by longer-term warfarin sodium therapy. Thirty-six (72%) underwent surgical correction of the underlying structural abnormality, and nine patients had postoperative balloon angioplasty. RESULTS: At the time of final evaluatio

Case of early infant congenital bilateral axillary vein thrombosis discovered by superficial venous dilatation; a case report.

BACKGROUND: Upper extremity deep venous thrombosis (UEDVT) is a disease caused by angiectopia and hypercoagulation, which result in the superficial vein to become dilatated by the development of collateral circulation. There have been no reports to date of cases of bilateral axillary vein thrombosis developing in the early infant period. Moreover, whether anticoagulant administration is indicated varies among patients. CASE PRESENTATION: A 1-month-old boy presented with dilatated superficial veins in the right temporalis, right auricle, whole face, and bilateral upper limbs. Venous dilatation

High success rates of ultrasound-guided distal internal jugular vein and axillary vein approaches for central venous catheterization: A randomized controlled open-label pilot trial.

BACKGROUND: Ultrasound (US)-guided axillary vein (AV) catheterization has been considered as the preferred site of insertion to minimize catheter-related infections. Given its difficulty of realization, internal jugular vein (IJV) access remains, thus, the first choice of catheter insertion site. This descriptive study was aimed to assess the success and complication rates of in-plane short axis approach of IJV in the lower neck and the AV approach under US-guidance. METHODS: In a prospective randomized controlled open-label pilot trial, all patients requiring central venous catheterization (

Silent threat after surgery: subclavian and axillary vein thrombosis following clavicle fixation leading to thoracic outlet syndrome.

Thoracic outlet syndrome (TOS) is a rare but significant complication that may occur following clavicular fractures, especially after surgical fixation. We present the case of a 32-year-old woman who developed partial thrombosis of the subclavian and axillary veins as a delayed complication after open reduction and internal fixation (ORIF) of a clavicle fracture with an anatomical plate. Approximately 1 month after surgery, the patient began experiencing pain in the axillary region and mid-arm, symptoms that raised suspicion for TOS. Doppler ultrasonography confirmed partial thrombosis of the

Ultrasound-guided Axillary Vein Puncture in Cardiac Lead Implantation: Time to Move to a New Standard Access?

Cardiac stimulation therapy has evolved significantly over the past 30 years. Currently, cardiac implantable electronic devices (CIED) are the mainstream therapy for many potentially lethal heart conditions, such as advanced atrioventricular block or sustained ventricular tachycardia or fibrillation. Despite sometimes being lifesaving, the implant is surgical and therefore carries all the inevitable intrinsic risks. In the process of technology evolution, one of the most important factors is to make it safer for the patient. In the context of CIED implants, complications include accidental pun

Postoperative upper limb edema and dysfunction generated by axillary node excision with or without axillary vein tributary preservation in breast cancer patients (PLEDGE-Surgery): a randomized controlled study.

BACKGROUND: The incidence of upper limb lymphedema and dysfunction caused by axillary lymph node dissection (ALND) in breast cancer patients is high, and there is a lack of effective prevention strategies. METHODS: We conducted a multicenter, prospective, double-blind, randomized, controlled clinical study to compare the effects of ALND with or without axillary vein tributary preservation on the incidence of postoperative upper limb edema and dysfunction in breast cancer patients. Patients aged 18-69 years with stage II-III breast cancer requiring ALND were enrolled. The primary endpoint was

A New Technique for the Treatment of Venous Popliteal Aneurysms Using the Axillary Vein: Medium and Long-Term Results.

BACKGROUND: Venous popliteal aneurysms are a rare but serious disease due to the risk of pulmonary embolism (PE). Aneurysms larger than 20 mm in diameter or with an embolic episode should be treated. The classic surgical technique is the tangential resection of the aneurysm with venorrhaphy, which may raise the risk of recurrence. We report a series of patients treated with a new technique consisting of aneurysmal resection and reconstruction using the axillary vein. The primary objective was to evaluate the patency of the transposed venous graft. The secondary objectives were to study the fea

Sonographic visualization and cannulation of the axillary vein in two arm positions during mechanical ventilation: A randomized trial.

BACKGROUND: Abduction of the arm has been used for ultrasound-guided infraclavicular axillary vein cannulation. We evaluated the influence of arm position on sonographic visualization and cannulation of the axillary vein in patients receiving mechanical ventilation. METHODS: Sixty patients scheduled to undergo surgery under general anaesthesia with controlled mechanical ventilation were included in this prospective randomized study. The depth, size and distance of axillary vein to the pleura were recorded at three points: Point A, the most proximal part of the axillary vein visualized with ad

Diagnosis and treatment of effort-induced thrombosis of the axillary subclavian vein due to venous thoracic outlet syndrome.

Venous thoracic outlet syndrome (VTOS) is uncommon but most frequently occurs in young, active, healthy patients. This condition typically presents as subclavian vein (SCV) effort thrombosis, also known as Paget-Schroetter syndrome. The pathophysiology underlying VTOS is chronic repetitive compression injury of the SCV in the costoclavicular space, resulting in progressive venous scarring, focal stenosis, and eventual thrombosis. Clinical evaluation includes a history and physical examination followed by catheter-based venography, for definitive confirmation of the diagnosis and initial treatm

"Effort" thrombosis of the axillary and subclavian vein associated with cervical rib and oral contraceptives in a young woman athlete.

"Effort" thrombosis, also called the Paget-Schroetter syndrome or primary thrombosis of the upper extremity, has been well documented in the literature. However, in our review of the United States, Canadian, and British literature, we found only 52 cases in which it was related to sports participation. We report a case of axillary and subclavian vein "effort" thrombosis in a young woman athlete, who was predisposed to thrombosis by all three postulates of the Virchow triad: namely, (1) stasis caused by constriction from a cervical rib, (2) increased coagulability as a result of oral contracept

Fragmentation of axillary fibrofatty tissue during dissection facilitates preservation of the intercostobrachial nerve and the lateral thoracic vein.

The preservation of neurovascular elements passing through the axillary fibrofatty tissue (the intercostobrachial nerve and the lateral thoracic vein) could be techniqually demanding if an en bloc axillary dissection is performed in the conventional way. In this paper we describe a surgical technique for more successful preservation of these elements, by which fragmentation of the axillary fibrofatty tissue is planned and performed "in advance". The techniques of axillary sampling biopsies, where lymphatic vessels are always divided, have shown that cutting of the lymph routes does not increas

The risk of injury to the axillary nerve, artery, and vein from proximal locking screws of humeral intramedullary nails.

An elderly female cadaver fore-quarter amputation was dissected, and a custom straight intramedullary nail with a 55 degrees oblique downward lateral to medial proximal screw was inserted with a trocar protruding beyond the medial cortex of the humeral surgical neck. The main trunk of the axillary nerve was found to be at risk with any penetration from anterior to posterior and any screw penetration beyond the medial cortex with internal rotation. The axillary artery and vein were at risk with penetration of over 3 cm by a drill point or screw tip whether a transverse or oblique downward screw

A comparative study of safety and efficacy of ultrasound-guided infra-clavicular axillary vein cannulation versus ultrasound-guided internal jugular vein cannulation in adult cardiac surgical patients.

BACKGROUND: Ultrasound (US)-guided internal jugular vein (IJV) cannulation is a widely accepted standard procedure. The axillary vein (AV) in comparison to the subclavian vein is easily visualized, but its cannulation is not extensively studied in cardiac patients. AIMS: This study is an attempt to study the efficacy of real-time US-guided axillary venous cannulation as a safe alternative for the time-tested US-guided IJV cannulation. DESIGN: This is a prospective randomized controlled study. MATERIALS AND METHODS: A total of 100 adult patients scheduled for cardiac surgery were divided equ

Comparison of comfort and complications of Implantable Venous Access Port (IVAP) with ultrasound guided Internal Jugular Vein (IJV) and Axillary Vein/Subclavian Vein (AxV/SCV) puncture in breast cancer patients: a randomized controlled study.

BACKGROUND: Axillary vein/subclavian vein (AxV/SCV) and Internal jugular vein (IJV) are commonly used for implantable venous access port (IVAP) implantation in breast cancer patients for chemotherapy. Previous research focused on comparison of complications while patient comfort was ignored. This study aims to compare patient comfort, surgery duration and complications of IVAP implantation between IJV and AxV/SCV approaches. METHODS: Two hundred forty-eight breast cancer patients were enrolled in this randomized controlled study from August 2020 to June 2021. Patients scheduled to undergo IVA

Extrathoracic subclavian-axillary vein location and morphological features over the first rib for pacemaker and defibrillator lead implantation.

BACKGROUND: We aimed to describe the variations of extrathoracic subclavian-axillary vein location and its morphology over the first rib by venography in order to facilitate venous puncture using fluoroscopic landmarks without contrast venography, and evaluate the success rate of punctures, which is made with our method. METHODS: Patients who had undergone de novo lead implantation with the help of prepuncture venography between 2011 and 2015 were enrolled. For detection of the segmental location of the axillary vein, the zones were defined (Zone 1: Posterior, Zone 2: Lateral, Zone 3: Medial)
