Changes in periprosthetic bone mineral density and bone turnover markers after osseointegrated implant surgery: A cohort study of 20 transfemoral amputees with 30-month follow-up.

BACKGROUND: The osseointegrated implant system is a treatment option for people with transfemoral amputation, but implant removal is not uncommon. The association between bone mineral density changes or bone turnover markers and the need for implant removal has not previously been investigated. OBJECTIVES: The aim was to evaluate changes in bone mineral density and bone turnover markers in people with transfemoral amputations treated with osseointegrated implants. STUDY DESIGN: This is a prospective cohort study. METHODS: Nineteen patients were followed up for 30 months or until implant rem

Swallowed by the bone - Is skin reaction the "tip of the iceberg"?

The most frequent complications of osseointegrated implantation include implant-site infections, soft tissue overgrowth and failure of osseointegration. Bone overgrowth is also a complication at the abutment site and infrequently reported. We describe a rare case of difficult control and exuberant bone overgrowth, with total implant involvement, in a short period of time never before described in the literature. Bone growth around the implant is frequently underestimated and can be the cause of difficult control of skin reactions, that in some cases is just the "tip of the iceberg". Timely sur

Abutment-free bone-anchored hearing devices in children: initial results and experience.

OBJECTIVES: Bone-anchored implantable hearing devices are widely accepted as a surgical option for certain types of hearing loss in both adults and children. Most commercially available devices involve a percutaneous abutment to which a sound processor attaches. The rate of complications with such bone conduction systems is greater than 20%. Most complications arise from the abutment. Recently, the Sophono (Boulder, CO) Alpha 1, an abutment-free system, has been introduced. STUDY DESIGN AND METHODS: We conducted a retrospective chart review of the first five patients who underwent implantatio

Reconstruction using a three-dimensional orbitozygomatic skeletal model of titanium mesh plate and soft-tissue free flap transfer following total maxillectomy.

The surgical strategy for maxillary reconstruction after maxillectomy has yet to be standardized. The authors developed a technique using a three-dimensional orbitozygomatic skeletal model of a titanium mesh for skeletal reconstruction after maxillectomy. From May of 1996 to September of 2000, 18 patients underwent reconstruction using the titanium mesh model in conjunction with a soft-tissue free flap following total maxillectomy for a maxillary malignancy. The soft-tissue free flap was conventional and consisted of two skin paddles to the maxillary defect. One skin paddle became the lateral

Simulated gait training improves joint loading symmetry in unilateral transfemoral bone-anchored limb users.

Bone-anchored limbs (BALs) are promising alternatives for addressing socket-related pathologies, but evidence suggests movement and joint loading asymmetries persist 12-months following transfemoral BAL implantation. Gait training targeting biomechanical symmetry is common in rehabilitation for people with transfemoral amputation. However, it is unknown if people with transfemoral BALs can achieve symmetrical gait due to absent anatomical structures. Our objective was to determine if gait symmetry is feasible in transfemoral BAL users and investigate the biomechanical effects of enforcing symm

Comparing the mechanical energetics of walking among individuals with unilateral transfemoral limb loss using socket and osseointegrated prosthetic interfaces.

Osseointegration (OI), or bone-anchoring, of a prosthesis is a transformative procedure for addressing issues of socket fit among individuals with lower limb loss. Mechanically, the removal of the socket interface substantially alters the transmission of load and the flow of energy through the prosthetic limb. Here, we compared the mechanical energetics of walking between socket and OI interfaces using biomechanical data and custom models of 8 service members pre- and ~24-months post-OI. Relative to a socket interface, an OI interface shifted loads toward the intact limb, which increased colli

The use of medical modeling in microvascular free tissue transfer reconstruction with osseointegrated implantation in complex midface defects.

Osseointegrated implants in conjunction with microvascular free tissue transfer reconstruction has been increasingly adopted in our head and neck cancer population. However, the majority of the literature on dental rehabilitation in free flaps have been for mandibular defects. Midface and maxillectomy defects are challenging defects for reconstruction. The utilization of medical modeling technology has allowed for improved efficiency and accuracy of microvascular free tissue transfer reconstruction of these midface defects and opened the possibility of immediate osseointegrated implant placeme

Bone-anchored hearing implant surgery: our experience with linear incision and punch techniques.

In recent years, bone-anchored hearing implants (BAHIs) have found wider application in the treatment of conductive and mixed hearing loss. Several surgical techniques have been developed to reduce complications, enhance healing and improve audiological and aesthetic results. We report our experience on the use of three BAHI surgery techniques: Group 1, linear incision with thinning of the subcutaneous tissue; Group 2, linear incision without thinning of the subcutaneous tissue; Group 3, punch technique (Minimally Invasive Ponto Surgery, MIPS). We retrospectively analysed patients undergoing B

Reconstruction of the extensor mechanism after major knee resection.

In periarticular knee resections, the relative lack of soft tissue coverage and need to reattach the extensor mechanism after en bloc resection of the tibial tuberosity with the tumor specimen complicate reconstructions and decrease postoperative function and stability of the knee joint. Distal femoral reconstructions are less problematic; muscular attachments are relatively few, neurovascular structures are not immediately adjacent to bone, and the knee extensor mechanism is usually not compromised from bone tumors. In the proximal tibia, the close proximity of the neurovascular structures in

A prospective clinical study on titanium implants in the zygomatic arch for prosthetic rehabilitation of the atrophic edentulous maxilla with a follow-up of 6 months to 5 years.

BACKGROUND: Prosthetic rehabilitation with implant-supported prostheses in the atrophic edentulous maxilla often requires a bone augmentation procedure to enable implant placement and integration. However, a rigid anchorage can also be achieved by using so-called zygomatic implants placed in the zygomatic arch in combination with regular implants placed in residual bone. PURPOSE: The aim of the present study was to report on the clinical outcome of using zygomatic and regular implants for prosthetic rehabilitation of the severely atrophic edentulous maxilla. MATERIALS AND METHODS: Sixty-nine

Does Transhumeral Osseointegration After Amputation Improve Patient-reported Outcomes and Prosthesis Use?

BACKGROUND: Individuals who have undergone transhumeral amputation often have difficulty wearing conventional socket-based prostheses, leading to frequent dissatisfaction and abandonment of the prosthesis. Osseointegration surgery may alleviate or mitigate many of these issues by providing a firm, reliable attachment between the humerus and the prosthesis, potentially leading to better functionality and quality of life; however, it is unclear whether the risks inherent to transdermal bone-anchored implants outweigh the potential functional benefits. QUESTIONS/PURPOSES: (1) Were there improvem

Soft-Tissue Contouring and Nerve Management During Lower-Limb Osseointegration Surgery.

BACKGROUND: Osseointegration provides a direct prosthesis interface for lower-limb amputees, many of whom are poor candidates for traditional socket-suspended prostheses. These skeletally anchored implants eliminate the skin-prosthesis interface, reducing complications such as soft-tissue breakdown, ulceration, and pain1. Soft-tissue contouring and management of peripheral nerves are essential aspects of osseointegrated implantation surgery for both transfemoral and transtibial amputees. These techniques protect the bone-implant interface, prevent skin-implant friction, and reduce neuroma form
