OBJECTIVE: Anterior choroid artery aneurysms (AChAAs) constitute 2%-5% of all intracranial aneurysms. They are usually small, thin walled with one or several arteries originating at their base, and often associated with multiple aneurysms. In this article, we review the practical microsurgical anatomy, the preoperative imaging, surgical planning, and the microneurosurgical steps in the dissection and the clipping of AChAAs.
METHODS: This review, and the whole series on intracranial aneurysms (IAs), are mainly based on the personal microneurosurgical experience of the senior author (J.H.) in t

Mucus-secreting adenomas or acinar adenoma of the choroid plexus are very rare. We report the case of a 79-year-old male with a 3-year history of occipital headaches with vomiting, ataxia and cerebellar signs. He was first seen due to difficulty while walking. He was admitted to the hospital with significant tumor expansion and clinical deterioration. CT and MRI revealed obstructive hydrocephalus secondary to a large fourth ventricular cyst mass, which enhanced markedly on contrast administration. Pathological findings were consistent with an acinar choroid plexus adenoma. The tumor was attach

The purpose of this article is to describe a patient with presumed choroidal metastasis from an eccrine adenocarcinoma of the scalp. A 45-year-old Caucasian woman presented with decreased visual acuity. Ophthalmologic examination was unremarkable. Her past medical history was significant for eccrine carcinoma of the scalp. The patient developed bilateral cervical lymph node metastases, and received chemotherapy and radiotherapy. She had recurrence of the scalp lesion and developed bone metastasis. The patient was again referred to an ophthalmologist owing to reduced visual acuity. Multiple cho

Notch3 has been studied in the context of brain development, but whether it plays a role in the formation of brain tumors is unclear. We demonstrate that the introduction of constitutively active Notch3 into periventricular cells of embryonic day 9.5 mice causes the formation of choroid plexus tumors (CPTs). Tumors arose in the fourth ventricles in 83% of animals and were associated with hydrocephalus. They were microscopically highly similar to choroid plexus papillomas in humans, with an ongoing proliferation rate of 4-6%. Signs of Notch pathway activity were also present in human choroid pl

BACKGROUND: Choroid plexus papillomas are rare, accounting for less than 1% of all intracranial tumours in adults. However, they are relatively more common in childhood and constitute 1.5 to 4% of intracranial tumours. They are most often located in the lateral ventricle, followed by the fourth and third ventricles and, rarely, in the cerebellopontine angle. Bilateral lateral ventricle choroid plexus papilloma is very rare and only a few cases has been reported.
CLINICAL PRESENTATION: A 3-year-old boy was admitted to our hospital with a history of irritability and vomiting. Neurological exami

Various viral, bacterial, parasitic and fungal agents have been found to cause infections of retina and choroidea in HIV-infected patients. Usually these infections are opportunistic infections caused by the profound immunodeficiency, which is a result of the decay of lymphocytes by HIV. Before the HIV epidemic only rare cases of cytomegalovirus (CMV) retinitis were known in the literature. Now CMV retinitis has become the most common infection of the eye in AIDS patients. Ocular toxoplasmosis in HIV-infected patients can have a severe clinical appearance without treatment. Spontaneous recover

We report a case of nocardia choroid plexitis with extension into the parenchyma of the left parietal lobe in a previously well 13-year-old girl. The neuroimaging findings mimicked those of an intracranial neoplasm. This case illustrates the importance of including infectious etiologies in the differential diagnosis of aggressive choroid plexus lesions. The differential diagnosis of enhancing lesions of the choroid plexus in children is discussed.

Choroid plexus cysts may be detected in the fetal choroid plexus on routine second trimester ultrasound scanning. The presence of these cysts is associated with trisomy 18 (Edward syndrome) in 3.47% of cases and with trisomy 21 (Down syndrome) in 0.46% of cases. The cysts themselves almost always disappear by 23 weeks and are thought to be a normal developmental variant. The world literature experience would indicate that the size of the choroid plexus cyst and the presence of bilateral cysts has no bearing on the magnitude of risk of chromosomal abnormality; 76% of babies with trisomy 18 also

Whether karyotyping is indicated in a fetus with choroid plexus cysts who is otherwise structurally normal is still controversial. Many authors have suggested basing the decision on cyst size, bilaterality, persistence with advancing gestational age, and association with other anomalies. We report a case of large bilateral choroid plexus cysts in a fetus with trisomy 21 who had no evidence of congenital anomalies or ultrasonographic signs of Down syndrome. Cyst sizes diminished by half over a 3-week period of follow-up. It appears that diminishing size alone should not be considered sufficient

Twenty-six cases of fetal choroid plexus cysts were diagnosed using ultrasonography at the Ultrasound Out-patients clinic of the University of Turin during the period 1989-1991. In 21 of these cases fetal karyotype was ascertained since, as has been reported in the literature, cysts of the choroid plexus may be associated with an anomalous karyotype (trisome 18 or 21). One of the 21 cases had an altered karyotype (trisome 21) (4.2%). The Authors stress the importance of a detailed ultrasound study of fetal morphology since, in the presence of structural anomalies, the incidence of trisome 18 i

We report the case of a patient with successful total removal of a previously undescribed intracranial tumor arising in the right lateral ventricle (probably of metaplastic origin). The histopathological examination revealed benign chondromatosis of the choroid plexus. The pathogenesis of this lesion, with special reference to synovial chondromatosis and to the differential diagnosis of solid neoplastic chondromas, their clinical features, and computed tomographic and magnetic resonance imaging findings, is presented and discussed.

Injections of air into the anterior chamber were applied by the authors in choroidal detachment and flat anterior chamber after 22 cataract extractions and 4 fistulizing operations in glaucoma (together 26 cases) after an ineffective conservative treatment. The deepening of the anterior chamber and reapposition of the choroid was achieved in all the cases. Special attention in applying this method was called to the deepening of the anterior chamber and to supply a considerable amount of air into the eye, so as to raise the intraocular pressure up to the normal values or even more. The air rema

A young women presented with chronic headaches associated with a cyst of the right lateral ventricle. The diagnosis of intraventricular so-called "arachnoid" cyst was supported by CT scan, MRI and stereotactic puncture. MRI was of great value for demonstrating that the cyst was located within the lateral ventricule, that it was delineated by a thin wall adherent to the choroid plexus and that the cyst content was CSF-like.

Choroid plexus tumors are most common in the first year of life and are quite large at presentation. Total resection is the treatment of choice, and many children require diversion of the cerebrospinal fluid despite removal of the tumor. Tumor size and associated hydrocephalus present formidable challenges to the pediatric neurosurgeon. The operative approaches as well as other treatment options are discussed.

Four cases of choroid plexus papilloma extending through the foramen of Monro are presented. All four patients were evaluated by contrast-enhanced CT and two of these were also evaluated by neurosonography. No other intraventricular tumor commonly extends through a ventricular outlet. The radiographic demonstration of this extension of an intraventricular mass through the foramen of Monro, cerebral aqueduct, or foramen of Luschka or Magendie represents an ancillary diagnostic sign of choroid plexus papilloma.

In order to test the hypothesis that overproduction of cerebrospinal fluid can cause the hydrocephalus seen in choroid plexus papillomas, adult mongrel dogs with and without hydrocephalus were subjected to high pressure intraventricular infusions of artificial cerebrospinal fluid (CSF). Pre- and postinfusion volumes were calculated, using a new method for on-line measurement of ventricular volume involving linear measurements from Conray ventriculograms. Ventricular volumes increased an average of 8% in 36 h. The dogs previously made hydrocephalic by the intracisternal injection of kaolin had
