Cogan's syndrome (CS) is a rare autoimmune disease in which approximately 10%-13% of people with the condition develop neurological symptoms. While glucocorticoids are the standard of care for patients with CS, disease-modifying anti-rheumatic drugs (DMARDs) and biologics agents are more widely used to treat the systemic and vestibular auditory manifestations of CS. Herein, we report a rare case of CS with central nervous system damage who failed to respond to systemic use of glucocorticoids and DMARDs. However, his symptoms were successfully improved by intrathecal injection of methotrexate (

PURPOSE: To report a patient with severe and progressive bilateral deafness who suffered recurrent episodes of diffuse lamellar keratitis (DLK) in both eyes > or = 4 months after undergoing uneventful LASIK for myopia.
METHODS: Four months after LASIK, the patient presented with blurred vision in the left eye. Best spectacle-corrected visual acuity was 0.9. Slit-lamp examination revealed aggregates of white cells at the paracentral and mid-peripheral interface without affecting the visual axis. Diffuse lamellar keratitis stage II was diagnosed and intensive corticosteroid treatment was starte

We report here a case of Cogan's syndrome associated with systemic vasculitis as well as myeloperoxidase-specific antineutrophil cytoplasmic antibody (MPO-ANCA)-related glomerulonephritis. A 71-year-old woman with the diagnosis of aortitis syndrome and pulmonary fibrosis for 7 years, complained of vertigo and hearing impairment. A diagnosis of serous otitis media was made. Although steroid therapy was effective, the symptoms relapsed several times. Seven months after the first manifestation of aural symptoms, she developed painful red eyes bilaterally and proteinuria. On admission, perinuclear

PURPOSE: Tubulointerstitial nephritis and uveitis (TINU) syndrome is a rare autoimmune disorder characterized by concurrent renal and ocular inflammation. Auditory features resembling Cogan's syndrome have been reported in TINU, though the nature of this overlap remains poorly understood. Here, we report a pediatric case of atypical Cogan syndrome to contribute to the growing literature on this unusual clinical association.
MATERIALS AND METHODS: Description of a case report and overview of the current literature.
RESULTS: 12-year-old female with TINU syndrome characterized by bilateral ante

We present a case series consisting of three female patients in their 30s with presumed autoimmune uveitis resembling Cogan's syndrome following caesarean sections (C-sections) with severe intraoperative bleeding and the use of chitosan-tamponade, exhibiting a combination of varying ocular and auditory symptoms postoperatively. Our patients displayed a range of inflammatory ocular changes, including stromal keratitis, panuveitis, retinal infiltrates, haemorrhages, optic disc swelling, and intraretinal and subretinal fluid, along with otalgia and hearing loss, consistent with typical and atypic

Cogan's syndrome (CS) is a rare autoimmune disorder that can cause profound sensorineural hearing loss by affecting the eyes and inner ears. A case report of a 12-year-old patient with CS who underwent successful cochlear implantation is presented. The patient developed bilateral hearing loss after a COVID-19 infection and showed no improvement with multiple medical therapies. Radiological evaluation revealed round window ossification and decreased fluid intensity in the cochlea and vestibule, consistent with inner ear inflammation and early-stage ossification. Following bilateral simultaneous

Cogan's syndrome is a rare autoimmune disease characterized by ocular inflammation and audiovestibular manifestations. Treatment consists of systemic glucocorticoids and other immunosuppressive agents including methotrexate, cyclophosphamide and TNF-α-inhibitors. Due to potential ovarian or fetal toxicity immunosuppressive treatment options are limited during pregnancies. Thus far there is a paucity of reports on pregnancies in Cogan's syndrome. With minimal transplacental transfer, Certolizumab pegol is considered to be safe for the use in pregnant patients with underlying inflammatory diseas

Cogan's syndrome (CS), a rare vasculitis characterized by non-syphilitic, interstitial keratitis and Ménière-like attacks, is classified into "typical" and "atypical" forms, while Takayasu arteritis (TAK) is a rare large-vessel vasculitis associated with human leukocyte antigen (HLA)-B*52. Very few cases meet both the CS and TAK classification criteria. We herein report a 53-year-old woman diagnosed with atypical CS and aortitis similar to TAK. Her 25-year-old daughter manifested TAK without symptoms of CS, and both are HLA-B*52 positive. Our case highlights the difficulties of distinguishing

We present a case of a 49-year-old woman who complained of chest pain after activity for 3 months. She had undergone percutaneous coronary intervention (PCI) on the left main (LM) and right coronary artery (RCA) for acute myocardial infarction (AMI) 1 year ago. At this admission, a coronary angiogram (CAG) revealed stent restenosis. Then, 99m Tc-Sestamibi myocardial perfusion imaging (MPI) and 18 F-FDG PET/CT showed multiple arteritis involving the coronary artery with no obvious myocardial ischemia. This suggests that the patient should be treated with anti-inflammatory measures instead of re

Cogan's syndrome is characterized by interstitial keratitis, vestibular impairment and hearing loss, commonly bilateral. Many patients, especially children, experience a delay in proper diagnosis which may delay treatment and thus impact on prognosis of hearing restoration. Less than 10 pediatric cases of Cogan's syndrome have been reported in literature and only five of them were reported with long-term follow-up. We report an extremely rare pediatric case of Cogan's syndrome typical form with long-term follow-up and evaluation of hearing impairment. Cogan's syndrome must be familiar to otorh

BACKGROUND: Cogan's syndrome is a rare disorder of unknown origin characterized by inflammatory ocular disease and vestibuloauditory symptoms. Systemic vasculitis is found in about 10% of cases.
CASE PRESENTATION: A 46-year-old female with Cogans's syndrome and a history of arterial hypertension presented with severe chest pain caused by an aneurysm of the ascending aorta with a dissection membrane located a few centimeters distal from the aortic root. After surgery, histopathological analysis revealed that vascular matrix integrity and expression of the major matrix molecules was characteriz

Behçet's disease (BD) is an autoimmune vasculitis of unclear etiology characterized by a wide range of systemic manifestations. The nervous system is involved in approximately 10% of patients with BD (neuro-Behçet). Diagnosis of neuro-Behçet is challenging, especially in the case of BD onset with neurological manifestations, and is based on a combination of clinical, laboratory, and neuroimaging data. To verify the diagnosis of neuro-Behçet, a wide range of conditions with similar manifestations must be ruled out: multiple sclerosis, neuromyelitis optica spectrum disorder, neurosarcoidosis, an

We report a case of Cogan's syndrome presenting as fever of unknown origin in a 31-year-old woman who was admitted to the hospital with a 7-week history of fever, night sweats and other constitutional symptoms. The diagnosis remained elusive despite numerous investigations, and the patient subsequently developed rash, episcleritis, dizziness and sensorineural hearing loss. While initially thought to be a postinflammatory response to a previous infection, confirmation of the rash as a vasculitis together with the audiovestibular and ocular involvement led to a clinical diagnosis of Cogan's synd
