Half of all pregnancies in the United States are unintended. Contraceptive methods are effective in reducing unintended pregnancy burden. The shared decision-making model serves as best practice when choosing among healthcare options, and may be appropriate when considering contraceptive options. This study examined women's experiences with shared decision-making in contraceptive use dynamics. Researchers analyzed 38 interviews with women of reproductive age (range: 19-50 years) living in South Carolina (May-November 2016). Researchers completed a constant comparative method of data analysis u

Gastroesophageal reflux disease (GERD) is a common upper esophageal condition and typical symptoms can include heartburn and sensation of regurgitation while atypical symptoms include chronic cough, asthma, hoarseness, dyspepsia and nausea. Typically, diagnosis is presumptive given the presence of typical and atypical symptoms and is an indication for empiric therapy. Treatment management can include lifestyle modifications and/or medication therapy with proton pump inhibitor (PPI) class being the preferred and most effective. Complete symptom resolution is not always achieved and long-term PP

OBJECTIVE: Describe cardiovascular clinicians' perceptions of Shared Decision Making following use of a decision aid (DA) for stable coronary artery disease (CAD) "PCI Choice", in a randomized controlled trial.
METHODS: We conducted a semi-structured qualitative interview study with cardiologists and physician extenders (n=13) after using PCI Choice in practice. Interviews were transcribed then coded. Codes were organized into salient themes. Final themes were determined by consensus with all authors.
RESULTS: Most clinicians (70%) had no prior knowledge of SDM or DAs. Mixed views about the

OBJECTIVE: To compare the effectiveness of shared decision making with usual care in choice of admission for observation and further cardiac testing or for referral for outpatient evaluation in patients with possible acute coronary syndrome.
DESIGN: Multicenter pragmatic parallel randomized controlled trial.
SETTING: Six emergency departments in the United States.
PARTICIPANTS: 898 adults (aged >17 years) with a primary complaint of chest pain who were being considered for admission to an observation unit for cardiac testing (451 were allocated to the decision aid and 447 to usual care), an

BACKGROUND: Shared decision making (SDM) and patient self-management support are key components of US and UK policy for chronic disease management, whereby SDM is seen as enhancing physician-patient negotiation around self-management. The WISE trial is implementing training in self-management support for primary care physicians in one UK region. This article describes preintervention levels of patient-reported SDM and explores how this varies with patient and practice characteristics.
METHODS: We analyzed baseline data from a cluster randomized controlled trial for 2965 patients with diabetes

The pathological eating behaviors in Anorexia Nervosa (AN), Bulimia Nervosa (BN), and obesity are characterized by a preference for high immediate reward, despite higher future losses in terms of both physical and psychological outcomes. The present study compared the decision making profile of females with a diagnosis of AN (n = 22), BN (n = 17), obesity (n = 18), and a healthy weight comparison group (n = 20) using a standardized neuropsychological test, the Iowa Gambling Task (IGT). The three clinical groups (AN, BN, obesity) were significantly impaired on the IGT compared with the comparis

INTRODUCTION: Socioeconomic disparities for breast cancer surgical care exist. Although the aetiology of the observed socioeconomic disparities is likely multifactorial, patient engagement during the surgical consult is critical. Shared decision-making may reduce health disparities by addressing barriers to patient engagement in decision-making that disproportionately impact socioeconomically disadvantaged patients. In this trial, we test the impact of a decision aid on increasing socioeconomically disadvantaged patients' engagement in breast cancer surgery decision-making.

BACKGROUND: In burn care, one of the most debated topics is the optimal treatment of patients with deep partial-thickness burns. With these patients, the decision must be made to perform early surgery or to wait and potentially limit, or even avoid, surgery. Both options are available in Dutch burn care, and the best treatment option is decided on clinical outcomes as well as patients' preferences. This complexity highlights the need for shared decision-making (SDM) and a decision aid (DA) to facilitate this process.
OBJECTIVE: This study aimed to support patients and health care professional

BACKGROUND: Taiwan has a high national caesarean rate coupled with a low vaginal birth after caesarean (VBAC) rate. Studies suggest that women do not receive sufficient information about birth choices after caesarean in Taiwan and shared decision making (SDM) is not an expectation. This pilot study aimed to test the feasibility of using a birth choices decision aid to improve women's opportunity for engagement in SDM about birth after cesarean.
METHODS: A two-phase sequential mixed methods pilot study was conducted in a regional hospital in northern Taiwan. Phase I involved a randomized pre-t

BACKGROUND: Conventional treatments have been frequently reported to offer partial relief for some individuals managing arthritis pain and related symptoms, leading many to consider alternative options, such as cannabis. Informed decision-making about cannabis use requires patients to weigh potential benefits and risks in light of their personal values and preferences.
OBJECTIVE: This study aims to systematically develop a theory-driven, evidence-based, user-centered decision aid (DA) for older adults with arthritis, considering medical cannabis.

BACKGROUND: there is little research on how people with dementia are involved in treatment decisions at diagnosis.
OBJECTIVE: to measure shared decision making when starting cholinesterase inhibitors, investigate associations with contextual factors and explore satisfaction and experience of the diagnostic meeting.
SETTING: nine UK memory clinics in two geographical locations.
SUBJECTS: 74 people receiving dementia diagnoses (with 69 companions) and 21 doctors.
METHODS: we video-recorded 74 memory clinic consultations and rated doctor-shared decision making behaviours using the Observing P

BACKGROUND AND AIMS: Shared decision-making (SDM) is increasingly recognized as a core element of patient-centered dental care, promoting collaboration between clinicians and patients to ensure that treatment decisions reflect both clinical evidence and individual preferences. However, few patient decision aids (PDAs) tailored to dental contexts have been systematically developed or evaluated. This study aimed to develop a dental-specific PDA and explore patients' perceptions of SDM following PDA-assisted consultations.
METHODS: A mixed-methods design was employed in Taiwan, combining a quant

BACKGROUND: Physicians and patients apply different but overlapped knowledge in their contexts to define the divergent problems to solve during cancer treatment.
OBJECTIVES: This study aims to identify the knowledge sources of problem defining between physicians and patients in shared decision-making on cancer treatment.
METHODS: This study adopted a divided narrative approach and interviewed 32 cancer patients and their paired 16 physicians from two top hospitals in northwestern China.
RESULTS: Physicians defined problems by five types of knowledge sources structured in the social system a

BACKGROUND AND OBJECTIVES: Shared decision making (SDM) before nonurgent tracheostomy in a child with chronic respiratory failure (CRF) is often recommended, but has proven challenging to implement in practice. We hypothesize that utilization of the microsystem model for analysis of the complex ecosystem in which SDM occurs will yield insights that enable formation of a reproducible, measurable SDM process.
METHODS: Retrospective chart review of a case series of children with CRF in whom a SDM process was pursued. The process included a palliative care consult, a validated decision aid and 12

OBJECTIVE: Limited research has studied the involvement of children in medical decision-making. The aim of the study was to understand the involvement of adolescents with cleft lip and/or palate (CL/P) in decisions about elective surgeries and treatments.
DESIGN: Parents and professionals completed mixed-methods questionnaires about the degree to which children had been involved in choices about elective treatments. Data were analyzed using content analysis. Young people aged 12 to 25 years were asked to take part in semistructured interviews. The data were analyzed using inductive thematic a

Prevention of sudden cardiac arrest/death and disease progression has traditionally been the primary basis for determining exercise recommendations for those with underlying cardiovascular disease. Potential harms of exercise restriction, including physical inactivity's impact on obesity, chronic disease, and negative psychological and emotional consequences, have been considered ancillary, if considered at all. Accumulating evidence suggests the relative safety of continued sport and exercise participation in the vast majority of those with a variety of cardiac conditions that were previously

The Rothman Index (RI) gives a visual picture of patient's condition and progress for the physician and family to view together. This case demonstrates how the RI graph facilitates physician-family communication. An 85-year-old man with normal pressure hydrocephalus and ventriculoperitoneal shunt presented with a subdural haematoma. He required a temporoparietal craniotomy and evacuation of left subdural haematoma, followed by care in an intensive inpatient rehabilitation unit. His course was complicated by aspiration pneumonia, dehydration, renal failure and phenytoin toxicity. During hospita

AIMS: Problem gambling has been proposed to represent a 'behavioural addiction' that may provide key insights into vulnerability mechanisms underlying addiction in brains that are not affected by the damaging effects of drugs. Our aim was to investigate the neurocognitive profile of problem gambling in comparison with alcohol dependence. We reasoned that shared deficits across the two conditions may reflect underlying vulnerability mechanisms, whereas impairments specific to alcohol dependence may reflect cumulative effects of alcohol consumption.
DESIGN: Cross-sectional study.
