BACKGROUND: Language impairment and behavioral symptoms are both common phenomena in dementia patients. In this study, we investigated the behavioral symptoms in dementia patients with different language backgrounds. Through this, we aimed to propose a possible connection between language and delusion.
METHODS: We recruited 21 patients with Alzheimer's disease (AD), according to the DSM-IV and NINCDS-ADRDA criteria, from the memory clinic of the Cardinal Tien Hospital in Taipei, Taiwan. They were classified into two groups: 11 multilinguals who could speak Japanese, Taiwanese and Mandarin Chi

Central to psychotic disorders, delusions are associated with disability and often respond inadequately to pharmacotherapy. Cognitive-behavioral treatments have been developed over the last 20 years that successfully address delusions. However, meta-analyses suggest only a modest improvement in psychotic symptoms. Because delusions share considerable overlap with anxiety, adapting principles and techniques that have demonstrated efficacy in the treatment of anxiety disorders might improve the impact of cognitive-behavioral treatment of delusions. We report a case illustrating a cognitive-behav

Case records of 546 patients referred for the first time to a community psychiatric service were studied. Fifty-six patients revealed delusions of various types. The female/male ratio was 2:1, and the mean age 57 years. The majority of the paranoid patients belonged to the lowest socio-economic groups. Symptomatology, including type of delusion, treatment given and diagnostic classification is presented. The delusions occurred as part of all major psychotic syndromes with a preponderance of organic and affective psychoses. After 5-20 years of observation a full remission without relapse was se

A patient manifesting an acute psychosis after receiving an injection of procaine penicillin is reported. The psychosis began immediately after drug administration and gradually abated over a forty-eight-hour period. The clinical presentation was dominated by paranoid delusions and a Capgras-like syndrome. Sixty-six previously reported cases were identified and reviewed. Patients manifested combinations of fear, auditory hallucinations, somatic hallucinations, visual hallucinations, and paranoid or religious delusions. The syndrome may occur with inadvertent intravenous injection of procaine a

IMPORTANCE: Persecutory delusions are common, distressing, and difficult to treat. Testing computational neuroscience models of delusions can identify new therapeutic targets.
OBJECTIVE: To determine whether change in delusion severity is associated with a corresponding change in volatility priors and brain activation estimated during a belief updating task.
DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial was conducted from April 9, 2021, to December 5, 2023, within the Vanderbilt University Medical Center Psychiatric Hospital and at a community mental health center in Nash

Patients with somatic delusional disorder are firmly convinced that something is wrong with their bodies and are often referred from one specialist to another without receiving a clear diagnosis. Some of these patients may turn to complementary or alternative medicine based on information found through online health searches. However, online health content is not regulated for quality, and much of it is inaccurate or potentially harmful, including fad diets and vitamin supplement advice from celebrity doctors. We present the case of a patient who believed he had Huntington's disease and develo

BACKGROUND: Neuropsychiatric sequelae of COVID-19 have been documented, including delusions, hallucinations, agitation, and disorganized behavior. Although the mechanisms for these symptoms remain unclear, there has been an increasing body of literature suggesting a correlation between COVID-19 infection and psychosis. Here, we illustrate the case of a 34-year-old female with no previous psychiatric history who contracted COVID-19 and subsequently developed severe symptoms of psychosis. After presenting to the emergency department with one month of worsening mood, auditory hallucinations, intr

OBJECTIVE: Conceptualizations of delusion formation implicate deficits in feedforward information updating across the posterior to prefrontal cortices, resulting in dysfunctional integration of new information about contexts in working memory and, ultimately, failure to update overfamiliar prior beliefs. The authors used functional MRI and machine learning models to address individual variability in feedforward parietal-prefrontal information updating in patients with schizophrenia. They examined relationships between feedforward connectivity, and delusional thinking and polygenic risk for sch

BACKGROUND: Several clinical studies point to a high prevalence of psychotic symptoms in frontotemporal dementia associated with C9ORF72 mutations, but clinicopathological studies addressing the association between C9ORF72 mutations and delusions are lacking.
METHOD: Seventeen patients with pathologically proven frontotemporal lobar degeneration (FTLD) associated with C9ORF72 mutations were identified from Neurodegenerative Disease Brain Bank. Of the 17 cases with C9ORF72 mutation, 4 exhibited well-defined delusions. The clinical history, neurological examination, neuropsychological testing,

BACKGROUND: Coping is of substantial relevance in the treatment and course of psychiatric disorders. Standardized instruments to assess coping with psychotic symptoms, particularly delusions, are rare. The aim of this study was to develop and evaluate the psychometric properties of a new instrument to assess coping strategies in the context of delusional experiences: the Heidelberg Coping Scales for Delusions (HCSD).
METHODS: Two hundred and twelve inpatients with schizophrenia spectrum disorders and affective disorders currently experiencing delusions were interviewed with the HCSD and other

Previously we reported a three-factor structure for hallucinations accompanying sleep paralysis (SP). These earlier analyses were, however, based on retrospective accounts. In a prospective study, 383 individuals reported individual episodes online providing further evidence for the three-factor structure as well as clearer conceptually meaningful relations among factors than retrospective studies. In addition, reports of individual episodes permitted a more fine-grained analysis of the internal structure of factors to assess predictions based on the hypothesis that a sensed or felt presence (

BACKGROUND: Delusional misidentification syndromes are rare psychiatric conditions, and subjective double delusion is particularly uncommon in adolescents. Agomelatine, an antidepressant with melatonergic and serotonergic properties, has limited evidence in pediatric populations and may trigger mood instability in vulnerable individuals.
CASE PRESENTATION: We report the case of a 17-year-old male with a history of depressive symptoms who developed a manic episode with psychotic features shortly after initiating agomelatine. The clinical picture was dominated by a subjective double delusion, i

This case describes an instance of delusion of pregnancy in an older male. This patient has a past history of schizophrenia and reported the delusion soon after engaging in a consensual sexual act with another male for the first time. Delusion of pregnancy is a rare disorder, especially among males and older adults. Along with the use of antipsychotic medications, we discuss the approach of using a pregnancy test in a male to aid in reality orientation and dispel the delusion of pregnancy.

Hypopituitarism can cause various psychiatric disorders. We herein report the case of a 60-year-old female with hypopituitarism who presented with severe hyponatremia, impaired consciousness, and delusional disorder. Brain imaging revealed a pituitary macrotumor with hemorrhaging, and the laboratory findings suggested panhypopituitarism. Glucocorticoid replacement therapy improved the hyponatremia, but the delusions persisted. Growth hormone replacement therapy successfully normalized the patient's insulin-like growth factor-1 levels and notably led to the disappearance of delusions. This case

BACKGROUND: Psychotic features have been part of the description of the borderline personality disorder (BPD) ever since the concept "borderline" was introduced. However, there is still much to learn about the presence and characteristics of delusions and about the stability of both hallucinations and delusions in patients with BPD.
METHODS: A follow-up study was conducted in 326 BPD outpatients (median time between baseline and follow-up = 3.16 years). Data were collected via telephone (n = 267) and face-to-face interviews (n = 60) including the Comprehensive Assessment of Symptoms and Histo

Although psychosis is a defining feature of Lewy body disease, psychotic symptoms occur in a subset of patients with every major neurodegenerative disease. Few studies, however, have compared disease-related rates of psychosis prevalence in a large autopsy-based cohort, and it remains unclear how diseases differ with respect to the nature or content of the psychosis. We conducted a retrospective chart review of 372 patients with autopsy-confirmed neurodegenerative pathology: 111 with Alzheimer's disease, 59 with Lewy body disease and concomitant Alzheimer's disease, 133 with frontotemporal lob

In his both highly amusing and thrilling story Roald Dahl describes an adventure of his fictive uncle Oswald Hendryk Cornelius, a snobby Englishman and womanizer, suffering from delusional anxiety of infections and hygienic compulsions. Forced by the breakdown of his car in the Sinai desert, he accepts the invitation of a noble Arab to spend the night in his palace, situated like a mirage in the middle of the desert. Oswald is plagued by erotic obsessions at the sight of the beautiful wife as well as the likewise beautiful daughter. The night rewards him with the desired amorous adventure but

In this paper, early detection of schizophrenia types such as hallucination and delusion propose through the high Q-factor of RADWT in EEG signal acquired during the cognitive task of the patient. The earlier diagnose obtains from the energy distribution of the EEG signal in the high resolution via optimum tuning in dilation factor, which influences the Q-factor, redundancy and ringing in the EEG signal. The early detection of type of schizophrenia prevents the illness progression and lifelong disease. In existing clinical trial, the psych clinician diagnose only the schizophrenia disease throu

Cognitive Neuropsychiatry) provides a robust critique of the two-factor theory of delusions. The heart of his critique is two challenges he derives from a paper by Tranel and Damasio (Tranel, D., & Damasio, H. (1994). Neuroanatomical correlates of electrodermal skin conductance responses. Psychophysiology, 31(5), 427-438), who illuminate the autonomic responses and brain damage of four patients often cited in support of the two-factor theory of Capgras delusion. I d

The interplay between psychiatric and dermatologic conditions has been recognized for decades as evidenced by the widely accepted classification system of psychocutaneous disorders: (1) primary dermatologic disorder with psychiatric sequelae, (2) primary dermatologic disorder exacerbated by stress, (3) primary psychiatric disorder with dermatologic sequelae, and (4) miscellaneous. However, there is minimal literature regarding dermatologic patients who demonstrate a preoccupation with a more severe cutaneous disorder despite evidence confirming a diagnosis of a minor, treatable skin condition.
