Surgical treatment of dissecting aortic aneurysm. Twenty years' experience.

Early and late results of 40 cases of dissecting aortic aneurysm were reported. 1. Result of 6 cases with acute dissection was as follows: two of three surgical patients survived, whereas all three with drug therapy died. Surgical treatment is preferred for the dissection of the ascending aorta or the arch. Some problems of the postoperative morbidity were discussed: 2. Whether medically treated or surgically, chronic dissection of the descending aorta or the abdominal aorta had better prognosis than that of the other site. Chronic dissection with no saccular enlargement had good prognosis. Mo

Risk of aortic dissection after aortic valve replacement.

Little information is available regarding the incidence of aortic dissection after previous aortic valve replacement (AVR), and factors associated with its development. Therefore, a meta-analysis of the literature was performed, and a patient population was studied retrospectively. Data from published studies showed that 87% of 31 patients were men (mean age 60 years), and 68% were known to have systemic hypertension. A dilated ascending aorta was observed at the time of AVR in 88% of patients. AVR was performed because of pure aortic regurgitation in 55% of patients, and combined aortic steno

Investigation of Type A Aortic Dissection Using Computational Modelling.

Aortic dissection is a catastrophic failure of the endothelial wall that could lead to malperfusion or rupture. Computational modelling tools may help detect arterial damage. Technological advancements have led to more sophisticated forms of modelling being made available to low-grade computers. These devices can create 3D models with clinical data, where the clinical blood pressure waveforms' model can be used to form boundary conditions for assessing hemodynamic parameters, modelling blood flow propagation along the aorta to predict the development of cardiovascular disease. Acute non-A-non-B aortic dissection: surgical or conservative approach?

OBJECTIVES: The indications for surgical approach in both type A and type B acute aortic dissections are widely recognized and accepted, but little is known about non-A-non-B dissection in which the aortic arch dissection is not accompanied by the involvement of the ascending aorta. METHODS: Between July 2002 and August 2014, all patients referred to our clinic with acute aortic dissection (n=281) were classified prospectively, taking into consideration the extent of dissection and the location of the intimal tear in three main segments of the aorta: the ascending aorta including the root, th

Combined Cardiovascular Syphilis and Type A Acute Aortic Dissection.

The occurrence of acute aortic dissection with the initiating tear in the ascending aorta superimposed on cardiovascular syphilis is an exceedingly rare occurrence. Such was the case, however, in a recently seen patient who presented with typical features of acute dissection (type A). Operative repair yielded the entire ascending aorta to examine both grossly and histologically and classic features of both conditions were observed.

Hybrid repair of type A acute aortic dissections with the Lupiae technique: ten-year results.

OBJECTIVES: Replacing the ascending aorta and the arch in patients with type A acute aortic dissection achieves good short-term results, but several patients are left with distal intimal tears or a patent false lumen in the descending aorta. In this series, we report the 10-year experience with the Lupiae technique, a hybrid aortic repair technique for patients with type A acute aortic dissection. METHODS: From 2003 to 2013, 89 patients with type A acute aortic dissections underwent replacement of the ascending aorta, the arch, and the rerouting of the neck vessels on the ascending aorta, cre

Biomechanics of the Aortic Root and Ascending Aorta in Patients With Marfan Syndrome.

OBJECTIVES: How Marfan syndrome (MFS) translates to alterations in ex vivo biomechanical properties of aortic tissue is not well defined. We aimed to characterize the biomechanical properties of the aortic root and ascending aorta in MFS and compare them to properties of normal, dissected, and non-MFS aneurysmal aortas. METHODS: Biaxial tensile and delamination testing were performed on aortic tissue from patients with MFS (root = 10, ascending = 6), aneurysms with bicuspid aortic valves (BAVs) (root = 19, ascending = 88), aneurysms with tricuspid aortic valves (root = 25, ascending = 90), ty

Left main coronary artery ostial repair with autologous ring-shaped aortic patch for iatrogenic aortic dissection.

Iatrogenic left main coronary artery and aortic root dissection are rare complications during coronary catheterization. A case of catheter-induced left main coronary artery dissection with subsequent retrograde progression into the ascending aorta wall is reported. The patient was surgically managed with an ascending aorta replacement and left coronary ostial repair with an autologuos aortic wall ring-shaped patch in the paraostial position in order to seal the intimal tear.

Stanford type A aortic dissection in a hypertensive patient with atherosclerosis of aorta and aortitis.

Dissection of aorta is a serious condition; the main factors are hypertension and diseases of the connective tissue or of collagen. Aortitis syndrome in combination with hypertension and atherosclerosis in association with ascending aortic dissection is rarely seen. We present the case of a 53-year-old hypertensive patient whose ascending aortic dissection was associated with pericardial effusion without rupture of the aorta and with pleural effusion. Several unusual aspects of transesophageal echocardiography are described. The intraoperative biopsy revealed inflammatory aortitis with mural h

Relationship Between the Primary Entry Tear Locations and Surgical Outcomes of Acute Type A Aortic Dissection: A Single-Center Experience.

PURPOSE: The primary entry tear location affects the prognosis and treatment strategies in aortic dissection. This study aimed to investigate the relationship between entry tear location and surgical outcomes in patients with acute type A aortic dissection (TAAD). MATERIALS AND METHODS: We retrospectively reviewed 89 patients with acute TAAD who underwent surgery between January 2021 and December 2022 at a single center in Vietnam. Patients were categorized into three groups according to entry tear location: ascending aorta, arch, and descending aorta. RESULTS: Primary entry tears were locat

Emergency and Compassionate Use of a Novel Ascending Endograft for Ascending and Arch Aortic Pathology.

PURPOSE: Patients with complicated ascending aortic pathology, including patients with acute type A aortic dissection may be at extreme risk for open repair. Thoracic endovascular aortic repair (TEVAR), infrequently used for the ascending aorta, may be considered an alternative in this setting. We describe early results for emergency and compassionate (E&C) use of a novel endograft, specifically designed for use to treat pathology of the ascending aorta. MATERIALS AND METHODS: This case series evaluated 19 patients (mean age, 68.84±13.12 years; 57.9% female) treated with ascending TEVAR for a

Surgical treatment of acute ascending aortic dissection.

Since adopting a policy of immediate operation on patients with acute dissection of the ascending aorta, 42 men and 6 women (ages 18-67 years) have been managed surgically. Thirty-two patients had graft replacement of the ascending aorta and resuspension of the incompetent aortic valve. One of these had a coronary graft. There were five deaths in this group. Eight patients required aortic valve replacement because of a diseased aortic valve as well as grafting of the ascending aorta, with one death. Three patients had resuspension of the aortic valve and primary repair of their dissection with

Uncommon presentation of Behçet's syndrome: aortic aneurysm and dissection due to Behçet's syndrome.

Aneurysm and dissections localized to the ascending aorta are usually encountered in patients with atherosclerosis and hypertension. Behçet's syndrome is a rare vasculitic syndrome occasionally responsible for inflammation in small to large vessels. Aneurysm formation and dissection in the ascending aorta due to Behcet's disease is an even rarer presentation of this vasculitis that will be discussed in conjunction with a case presentation.

[Removal of ascending aortic dissection and residual blood flow after transcatheter isolation of descending aortic dissection].

Presented herein is a clinical case report concerning successful simultaneous surgical treatment of a female patient with a complication after transcatheter treatment for chronic dissection of the descending portion of the thoracic aorta. The woman was subjected to elective transcatheter isolation of chronic dissection of the descending thoracic aorta using a stent graft with complete coverage of the zone of the origin of the left subclavian artery. Repeat control imaging studies several months after the intervention revealed residual blood flow through the false channel, directed retrogradely

[Retrograde Stanford type A aortic dissection treated by thoracic endo-vascular aortic repair (TEVAR) after ascending aortic replacement].

A 77-year-old woman was admitted to a hospital with complaints of back pain, nausea and vomiting for 4 days. She underwent enhanced aortic computed tomography (CT) and was diagnosed with Stanford type A acute aortic dissection. Then she was transferred to our hospital. CT showed aortic dissection spreading from the ascending aorta up to the abdominal aorta and an intimal tear located at the descending thoracic aorta. Although the false lumen of the ascending aorta was thrombosed, emergency ascending aorta replacement and aortic valve replacement were performed, because pericardial effusion and

[A case of successful operation for chronic aortic dissection (DeBakey 1 type) complicated with cerebral infarction].

We report a successful operation for a patient of chronic aortic dissection complicated with cerebral infarction. A 60-year-old woman has been treated suffering from cerebral infarction for 7 years. She was referred to our hospital for abnormal chest X-ray and complaint of dry cough. The enhanced CT scan and aortogram demonstrated chronic aortic dissection (DeBakey I) extending from ascending aorta to the abdominal aortic bifurcation with occlusion of right common carotid artery and dilated ascending aorta and aortic arch. The onset of aortic dissection seemed to be, from a clinical point of v

Spontaneous rupture of the ascending aorta: case report and review.

Spontaneous nontraumatic rupture of the ascending aorta occurred in a hypertensive patient. The clinical findings suggested acute aortic dissection, and echocardiography showed a large pericardial effusion. Computed tomography scanning did not indicate aortic dissection, but aortography in 3 projections revealed an area of intimal disruption similar to the niche of an ulcer. The patient underwent replacement of the ascending aorta and proximal aortic arch, and the postoperative course was uneventful.

[Total Arch Replacement with Concomitant Thoracic Endovascular Aortic Repair Via the Ascending Aorta for Extended Thoracic Aneurysm;Report of a Case].

The patient was a 66 year-old male. Computed tomography (CT) angiography showed a huge aneurysm(120 mm) in the aortic arch and chronic type B aortic dissection(45 mm) in the descending aorta. Echocardiography showed patent ductus arteriosus( PDA). Because of pulmonary hypertension due to PDA, it was considered unacceptable to put him under general anesthesia twice. We performed thoracic endovascular aortic repair (TEVAR) via the ascending aorta and total arch replacement (TAR) simultaneously to prevent paraplegia. After establishment of cardiopulmonary bypass( CPB), a stent graft was inserted
