Anatomical Feasibility Study of the Digastric Branch of the Facial Nerve: A Potential Donor for Facial Nerve Reanimation.

PURPOSE: The digastric branch is the second branch of the facial nerve after emerging from the stylomastoid foramen. To the best of our knowledge, few detailed anatomic studies of the digastric branch have been performed. Moreover, the use of the digastric branch for facial nerve reanimation has not been explored. MATERIALS AND METHODS: Nine sides of 5 fresh frozen cadavers were dissected for anatomic observation of the digastric branch and exploration of the feasibility of the digastric branch as a donor for facial nerve reanimation. The facial nerve trunk (FNT) and digastric branch were rea

Anatomical assessment of the digastric branch of the facial nerve as a landmark to localize the extratemporal facial nerve trunk.

PURPOSE: Localization of the facial nerve trunk (FNT) [i.e., the portion of the facial nerve between the stylomastoid foramen (SMF) and pes anserinus] may be required during various surgical interventions such as parotidectomy and hypoglossal-facial anastomosis. Several landmarks have been proposed for efficient identification of the FNT. We sought to assess the anatomical features of the digastric branch of the facial nerve (DBFN) and its potential as a landmark to identify FNT. METHODS: Fifteen sides of eight cadaveric heads were dissected to find the DBFN. Anatomic features of DBFN includi

Facial nerve hemangiomas at geniculate ganglion: preservation of nerve integrity is correlated with duration of facial palsy.

OBJECTIVE: To study preservation of nerve integrity in 16 cases with facial nerve hemangiomas at geniculate ganglion (GG). METHODS: 16 cases with facial nerve hemangiomas at GG, who presented with facial palsy, were included in the study. Preservation of nerve integrity was attempted by the same surgeon during surgical removal, and those who failed to preserve nerve integrity underwent nerve grafting. The patients were divided into longer duration group (>12months) and shorter duration group (≤12months) according to duration of facial palsy, and preservation of nerve integrity in the couple o

Electrophysiology of Cranial Nerve Testing: Trigeminal and Facial Nerves.

The clinical examination of the trigeminal and facial nerves provides significant diagnostic value, especially in the localization of lesions in disorders affecting the central and/or peripheral nervous system. The electrodiagnostic evaluation of these nerves and their pathways adds further accuracy and reliability to the diagnostic investigation and the localization process, especially when different testing methods are combined based on the clinical presentation and the electrophysiological findings. The diagnostic uniqueness of the trigeminal and facial nerves is their connectivity and thei

Nerve suture and grafting to restore the extratemporal facial nerve.

A concept for dealing with extratemporal lesions of the facial nerve is based on cadaver studies performed by Meissl. In spite of plexiform arrangement and fiber exchange, areas and fascicle groups can be differentiated within the cross section of the facial nerve which contain a great percentage of the fibers supplying a certain muscle group. An attempt should be made to unite the corresponding parts by direct coaptation, or, if nerve defect exists, by nerve grafts.

Facial nerve: paralyses, surgical approaches, rehabilitation.

The seventh cranial nerve (also called the facial nerve) is a neuroanatomical structure that arises from the brainstem near the lower part of the pons, enters the facial canal through the fundus of the internal auditory meatus, and finally emerges from the stylomastoid foramen, giving branches to the facial muscles. It innervates not only these muscles but also provides fibers to glands, including the lacrimal, salivary, and sublingual glands; therefore, malfunction of this nerve may result in dire clinical consequences. Because it is located mainly within bone, morphological variations may re

Relative exophthalmos in facial nerve palsy.

OBJECTIVE: While ocular manifestations of facial nerve palsy, such as lagophthalmos are well known, there is limited knowledge on whether CN 7 palsy impacts globe positioning and its implications on symptoms and management. We hypothesize that CN 7 palsy is associated with ipsilateral exophthalmos. METHODS: A retrospective analysis of 500 patients diagnosed with facial nerve disorders, identified by ICD codes G51.0-G51.9, was conducted at a tertiary health care facility from January to November 2023. Patients were included if they had a diagnosis of unilateral facial nerve (CN 7) palsy and av

[Neural Reconstruction for Facial Nerve Palsy].

The basic techniques of nerve reconstruction surgery for facial nerve palsy are end-to-end nerve suture and end-to-side nerve suture. The latter has the epineural window method and the partial neurectomy/neurotomy method for the treatment of "side" nerves. Hypoglossal-facial nerve cross-link graft is a method of nerve reconstruction in which the hypoglossal and facial nerves are bridged by autologous nerve transposition with end-to-side nerve suture. It has been shown to be effective in treating the sequelae of established facial nerve palsy, but there are some issues that prevent the achievem

Best Practices in Facial Nerve Monitoring.

OBJECTIVES/HYPOTHESIS: Facial nerve monitoring (FNM) has evolved into a widely used adjunct for many surgical procedures along the course of the facial nerve. Even though majority opinion holds that FNM reduces the incidence of iatrogenic nerve injury, there are few if any studies yielding high-level evidence and no practice guidelines on which clinicians can rely. Instead, a review of the literature and medicolegal cases reveals significant variations in methodology, training, and clinical indications. STUDY DESIGN: Literature review and expert opinion. METHODS: Given the lack of standard r

Experimental Studies on Facial Nerve Regeneration.

Insufficient recovery after injury of a peripheral motor nerve is due to (1) inappropriate pathfinding as a result of axonal regrowth to inappropriate targets, (2) excessive collateral axonal branching at the lesion site, and (3) polyinnervation of the neuromuscular junctions (NMJs). The rat facial nerve model is often used because of its simple and reliable readout to measure recovery of function (vibrissal whisking). Over the last decades scientists have concentrated their efforts to combat mostly NMJ polyinnervation, because it turned out to be very difficult to reduce collateral axonal bra

[Ophthalmic manifestations of facial nerve palsy].

Facial nerve palsy is a common pathology that develops due to various diseases, traumas and medical interventions. The most common peripheral type of facial paralysis leads to acute unilateral involvement of facial muscles along with the development of paralytic lagophthalmos. The absence of adequate and timely medical treatment in patients with paralytic lagophthalmos can lead to severe corneal complications such as cornea perforation causing loss of eye and consequently restricted professional ability, work capacity and significant decrease of patient's life quality. Patients with paralytic

Facial Nerve Laceration and its Repair.

INTRODUCTION: Facial paralysis is a devastating condition with profound functional, aesthetic and psychosocial consequences. Tumors within or outside the skull, Bell's palsy and trauma are the most common causes of facial paralysis in adults. CASE PRESENTATION: Our patient was a 35-year-old man with deep laceration wounds. The patient was taken to the operating room and the nerves were repaired. We observed gradual improvement of muscle performance except branches of the frontal nerve. CONCLUSIONS: Complete rupture of the facial nerve is challenging and the treatment is surgery, which requir

Imaging spectrum of facial nerve lesions.

The facial nerve is affected by a wide variety of pathologies, including congenital, traumatic, inflammatory, and neoplastic conditions. Imaging plays a vital role in the diagnosis of these pathologies. The facial nerve has a complex anatomy and course. A strong grasp of normal facial nerve anatomy is essential for the radiologist to maintain a high level of diagnostic sensitivity. This article details the normal imaging anatomy of the facial nerve and the imaging features of common facial nerve pathologies.

Masseteric nerve use in facial reanimation.

PURPOSE OF REVIEW: To review the growing literature on the use of the masseteric nerve in facial reanimation, from direct neurotization, to 'baby-sitter' techniques, to its use in powering neuromuscular free tissue transfer. We focus on the indications for the different uses based on the timing of the facial paralysis and other patient factors. RECENT FINDINGS: The use of the masseteric nerve in facial reanimation is gaining widespread acceptance for use in an expanding number of clinical scenarios. Surgeon's experience and preference as well as patient selection are very important factors in

Facial nerve hemangioma: a case report.

Although hemangiomas are relatively common in the head and neck, those that originate in the facial nerve are extremely rare. These benign tumors have the potential to compress or invade the adjacent facial nerve and thereby produce facial paralysis and other associated symptoms. We present a case of facial nerve hemangioma in a 15-year-old girl that initially manifested as unilateral facial weakness. We also discuss the diagnostic imaging and management options.

Facial nerve: from anatomy to pathology.

The facial nerve (CN VII) emerges from the facial nerve nucleus in the pons. It is accompanied by CN VIII along its cisternal pathway, as well as at the internal auditory meatus. Its petrous pathway includes a labyrinthine segment, a horizontal tympanic segment and a vertical mastoid segment until the stylomastoid foramen. It then continues to the parotid gland. Pontine impairment is usually associated with other neurological symptoms. Lesions of the cerebellopontine angle (most often meningioma and schwannoma) initially result in impairment of CN VIII. The impairment of CN VII takes second pl
