BACKGROUND: Pediatric upper extremity fractures are common and frequently require surgical intervention. Traditional fixation methods using metallic implants, such as Kirschner wires and elastic stable intramedullary nails, offer reliable mechanical stability but necessitate secondary procedures for hardware removal. Polymer-based resorbable implants offer an advantage by eliminating the need for secondary procedures for hardware removal while maintaining efficacy. This systematic review evaluates the safety, efficacy, and clinical outcomes of resorbable compared to non-resorbable implants in

BACKGROUND: We have proposed that a positive tendon irritation test is suggestive of flexor pollicis longus (FPL) tendon damage that can lead to tendon rupture after volar plate fixation for distal radius fractures. We investigated cases of postsurgical hardware removal and validated the tendon irritation test as a way to elicit a sign of FPL tendon irritation.
PATIENTS AND METHODS: We performed hardware removal from 30 wrists in 28 consecutive patients after volar plate fixation. Subjects included 9 men and 19 women with an average age of 58.8 years. The duration of internal fixation average

BACKGROUND: Distal fibular fractures are extremely common, yet there remains controversy about which type of plating technique is the most appropriate. We aimed to compare clinical and biomechanical outcomes following posterior antiglide plating and lateral neutralization plating for Weber B distal fibular fractures.
METHODS: A systematic review and meta-analysis of the literature was conducted by two independent reviewers according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We included all comparative studies of distal fibular fracture fixation with

Soft tissue reconstruction of the distal third of the lower limb with exposure of the internal hardware is a challenging problem with several potential complications, such as exposure of the fracture line, fracture instability and bacterial contamination. The treatment of these lesions usually consists of substitution of the internal hardware with external fixation devices and further flap coverage. We propose a different reconstructive approach, characterized by harvesting a sural fasciomusculocutaneous flap on the exposed internal hardware once a sterile ground has been obtained. Four patien

OBJECTIVE: Spinal cord stimulation has been shown to improve pain relief and reduce narcotic analgesic use in cases of complex refractory pain syndromes. However, a subset of patients ultimately undergoes removal of the spinal cord stimulator (SCS) system, presumably because of surgical complications or poor efficacy. This retrospective study addresses the paucity of evidence regarding risk factors and underlying causes of spinal cord stimulation failures that necessitate this explantation.
METHODS: In this retrospective single-center review, 129 patients underwent explantation of SCS hardwar

INTRODUCTION: Olecranon fractures are common injuries, especially in the elderly population, in whom conventional osteosynthesis with tension band wiring (TBW) or plating (PF) is associated with high reoperation rates for hardware removal due to intolerance (up to 80% of surgically treated patients in the TBW group and 25% in those treated with PF). High-strength transosseous suture (TBS) has been proposed as an alternative to reduce these complications associated with conventional techniques, although few studies with long-term follow-up exist. The aim of this study was to assess, in the long

Medical tourism is a rapidly growing sector of economic growth and diversification. However, data on the demographics and characteristics of the traveling patients are sparse. In this study, we analyzed the common demographic properties and characteristics of the inbound medical tourists seeking orthopedic medical care in Germany for the years 2010 to 2019 compared to a domestic group. At the same time, we examined how the COVID-19 pandemic outbreak of 2020 changed the field of medical tourism in Germany. Calculations were performed using administrative hospital data provided by the Federal St

Distal clavicle fractures and acromioclavicular injuries are common shoulder injuries. Despite the frequency of these injuries, there is no gold standard for treatment. Recent suspensory fixation techniques have minimized the amount of hardware required for surgical treatment of both acromioclavicular separations and distal clavicle fractures. Advantages include reconstruction of ligamentous anatomy, minimal hardware irritation, earlier return to activity, and decreased need for hardware removal. These reconstructions are classically done in the beach chair position in an open fashion. Our pre

BACKGROUND: Cardiac implantable electronic devices (CIEDs) have now become common therapeutic adjuncts for patients prior to orthotopic heart transplantation (OHT). Removal of the generator and the intracardiac components occurs at time of transplantation but removal of the intravascular portion of leads may be unsuccessful without specialized extraction equipment.
METHODS: We performed a retrospective audit of chest radiographs and clinical records of patients undergoing OHT at Green Lane and Auckland City Hospitals between 2002 and 2012.
RESULTS: At the time of transplant surgery, 56 of 10

INTRODUCTION: In recent years, anatomically contoured locking plates have been introduced as an alternative fixation method for patellar fractures. Biomechanical studies have demonstrated superior mechanical stability and clinical studies further suggest that plating reduces complication and revision rates, especially in comminuted fracture patterns. However, there is uncertainty regarding the role of plating in less complex fracture types. Following an institutional shift to routine plating of all surgically treated patellar fractures, this study compares outcomes and costs of plate versus no

A posterior approach to the elbow utilizing an olecranon osteotomy has been shown to provide excellent visualization of the distal humerus articular surface. However, many bony stabilization and fixation methods for the olecranon osteotomy are usually prominent, frequently symptomatic, and often require a second operation for removal. This paper evaluates the use of an innovative device, the olecranon sled, in fixation of olecranon osteotomies for exposure of intra-articular distal humerus fractures and provides follow-up results. A retrospective review of all patients with intra-articular dis

PURPOSE: Tension band wiring (TBW) or plating may be used for fixation with similar clinical outcomes for adults with displaced Mayo 2A olecranon fractures. The primary hypothesis is that total direct costs (TDCs) for surgery are lower for TBW than plating. Our secondary hypothesis is that combined surgical TDCs are lower for TBW even with a 100% rate of subsequent tension band hardware removal and a 0% rate of plate removal.
METHODS: Patients who underwent TBW or plating of an isolated unilateral Mayo 2A olecranon fracture between July 2011 and January 2020 at a single academic medical cente

OBJECTIVES: Objectives: Titanium plates may offer advantages in fracture healing, yet concerns remain regarding hardware removal due to risks such as screw stripping and cold welding. Whether titanium removal is more difficult than stainless steel remains uncertain. This study compared the ease of implant removal, focusing on distal femur fractures treated with titanium versus stainless-steel plates.
METHODS: We conducted a retrospective cohort study of patients with distal femur fractures who underwent implant removal after fixation with either stainless-steel or titanium plates. The primary

Two elderly female patients with Kellgren-Lawrence grade 4 knee osteoarthritis (KOA) having varus knee deformity and retained hardware for internal fixation in either femur or tibia underwent total knee arthroplasty (TKA) using 3D templating and patient-specific instrumentation (PSI). TKA was performed to treat KOA by minimally removing or without removing the retained hardware for the internal fixation of distal femoral or tibial diaphyseal fractures. Throughout a 2-year follow-up period, no superficial or deep infection was observed. In additions, no radiological symptoms of suspected compon

We report a case of a 63-year-old man who presented an end stage hip osteoarthritis 42 years after a proximal femoral osteotomy performed for the treatment of an intracapsular femoral neck fracture. A cemented total hip arthroplasty was implanted using a standard posterior approach. The osteotomy hardware was totally covered with a new cortical bone formation. A layer of bone tissue was removed, the screw heads were broken and the nail plate was also removed. The screw threaded portions were retained because they did not interfere with cemented femoral stem implantation. Fourteen years after t

OBJECTIVE: To evaluate the effectiveness of a replating technique having a less-invasive stabilization system (LISS) for femoral shaft fractures due to LISS failure in adults.
PATIENTS AND METHODS: There were 11 patients with hardware failure of LISS for femoral shaft fractures, on an average of 50 days after the primary operation. The failed implants were removed, and the fractures were replated with a LISS following the rationale of biological osteosynthesis. Radiological fracture union and incidence of postoperative complications were employed to evaluate the effectiveness of this replatin

Posterior vault distraction osteogenesis traditionally relies on the use of cranial distractors affixed with titanium screws to the posterior cranial vault transport segment to allow for expansion of intracranial volume. The authors describe a novel technique using the resorbable fixation Synthes Rapidsorb Injectable Polymer System for stable fixation of the distraction hardware for posterior vault distraction osteogenesis. In a retrospective review of their experience with the system, there was no instance of hardware or fixation failure. This off-label use of Rapidsorb system resulted in a s
