A shearing osteochondral fracture of the humeral head following an anterior shoulder dislocation in a child.

A dislocation of the shoulder joint is rare in children with an open physis. The fractures associated with an anterior dislocation generally reported in the literature have been Hill-Sachs lesions, avulsions of the greater tuberosity and glenoid fractures. We present a case of a previously unreported shearing osteochondral fracture, which is distinct from a classic Hill-Sachs lesion of the humeral head, in 12-year-old boy. The patient suffered a traumatic anterior shoulder dislocation with a spontaneous reduction along with this associated fracture. The fracture subsequently healed with no fur

Pressure between the humeral head and the subscapularis tendon after the modified Boytchev procedure.

The pressure between the humeral head and the subscapularis tendon was determined in 32 patients who had recurrent anterior shoulder dislocations. These patients' pressures were measured during a modified Boytchev procedure both before and after transposition of the conjoined tendon, and they were remeasured at the time of screw removal, performed at a mean of 13 months after the initial surgery. The mean clinical follow-up period was 31 months. A micro-tip catheter transducer was inserted into the glenohumeral joint between the humeral head and the subscapularis tendon. Pressures were measure

Cystic changes of the humeral head on MR imaging. Relation to age and cuff-tears.

We obtained MR images of 140 painful shoulders in 134 patients to determine the relationship between cystic changes of the humeral head and integrity of the rotator cuff. Cystic changes were observed in 49 shoulders (35%) and the commonest site was in the bare bone area of the anatomical neck, and the second commonest site was at the attachment of the supraspinatus tendon. Cystic changes in the bare bone area were observed equally often in shoulders with or without rotator cuff tears (27% and 18%, respectively) and were more frequently observed in the elderly. Cystic changes at the attachment

Rotator cuff tearing and inferior subluxation of the humeral head: report of two cases.

We encountered two patients with atraumatic rotator cuff tears who exhibited inferior subluxation of the humeral head. In both patients shoulder motion was limited by pain. Repair of the rotator cuff tear relieved the pain and restored glenohumeral joint motion. Although the cause of this unusual complication of rotator cuff damage remains unclear, the inferior subluxation was associated with reflex muscle hypotonia, which would have reduced the pressure in the subacromial space and thus would have decreased the pain caused by the cuff lesion. No similar case seems to have been documented in t

Posterior shoulder dislocation and humeral head necrosis in a recreational scuba diver with diabetes.

An insulin-dependent diabetic who was previously a recreational scuba diver suffered a posterior shoulder dislocation after a trivial injury and was subsequently found to have local necrosis of the humeral head. The only definite conclusion that can be reached is that this patient should not dive again.

Use of the Humeral Head as a Reference Point to Prevent Axillary Nerve Damage during Proximal Fixation of Humeral Fractures: An Anatomical and Radiographic Study.

INTRODUCTION: Treatment of proximal humeral fractures with plate osteosynthesis or intramedullary nail fixation in humeral shaft fractures with a proximal locking bolt carries the risk of iatrogenic injury of the axillary nerve. The purpose of this anatomical study is to define a more reliable safe zone to prevent iatrogenic axillary nerve injury using the humeral head instead of the acromion as a (radiographic) reference point during operative treatment. MATERIALS AND METHODS: Anatomical dissection and labeling of the axillary nerve and branches was performed on 10 specially embalmed human s

Tendon Cells Root Into (Instead of Attach to) Humeral Bone Head via Fibrocartilage-Enthesis.

Large joints are composed of two closely linked cartilages: articular cartilage (AC; rich in type II collagen, a well-studied tissue) and fibrocartilaginous enthesis (FE; rich in type I collagen, common disorder sites of enthesopathy and sporting injuries, although receiving little attention). For many years, both cartilages were thought to be formed by chondrocytes, whereas tendon, which attaches to the humeral bone head, is primarily considered as a completely different connective tissue. In this study, we raised an unconventional hypothesis: tendon cells directly form FE via cell transdiffe

Management of intraoperative posterior decentering in shoulder arthroplasty using anteriorly eccentric humeral head components.

BACKGROUND: Posterior humeral decentering presents a challenge in glenohumeral arthroplasty. Soft tissue releases and osteophyte resection can lead to intraoperative decentering not evident preoperatively. Inferior outcomes result if decentering is not addressed as a part of the arthroplasty. When there is >50% posterior subluxation of the humeral head on passive elevation of the arm at surgery, we have used an anteriorly eccentric humeral head component to improve centering of the humeral articular surface on the glenoid. METHODS: We reviewed the 2-year outcomes for 33 shoulder arthroplastie

Late-presenting chronic humeral head collapse, Cruess stage V, managed with modular uncemented hemiarthroplasty.

Humeral head osteonecrosis may remain unrecognized until collapse and secondary arthropathy develop. A 44-year-old man presented with progressive right shoulder pain and restricted motion for 1 year. Radiographs and computed tomography showed advanced humeral head collapse with secondary glenohumeral arthropathy, most consistent with Cruess stage V osteonecrosis. Preoperative ultrasound demonstrated preserved rotator cuff continuity. Because joint-preserving procedures were not feasible, modular uncemented shoulder hemiarthroplasty was performed. This case illustrates the rationale for hemiart

Arthroscopic-Assisted Core Decompression of Humeral Head Osteonecrosis With Bone Marrow Aspirate Concentrate Augmentation.

BACKGROUND: The proximal humerus is the second-most common anatomic site for osteonecrosis, a condition of disrupted blood supply leading to death of bone tissue, with resultant shoulder pain and dysfunction. Core decompression is a viable joint-preserving surgical intervention that aims to improve local blood supply and restore bone health prior to bony collapse. INDICATIONS: Humeral head osteonecrosis has numerous underlying causes, most commonly the sequelae of prior trauma, chronic use of systemic corticosteroids, sickle cell disease, and alcohol abuse. Core decompression may be indicated

Sex estimation from humeral and femoral head diameters using CT-scans of living Finns.

This study used 155 computed tomography (CT) scans of Finns (81 males, 74 females) for creating population-specific sectioning points from femoral and humeral head diameters for the purposes of sex estimation. The study included four measurements: vertical and transverse diameter of the femoral head and vertical and transverse diameter of humeral head. All the measured dimensions were sexually dimorphic and the following sectioning points were calculated: Femur: vertical diameter 46.2 mm, transverse diameter 46.3 mm; Humerus: vertical diameter 46.8 mm, transverse diameter 43.6 mm. These sectio

Quantitative SSM-based analysis of humeral head migration in rotator cuff tear arthropathy patients.

Rotator cuff tear arthropathy (RCTA) is characterized by massive rotator cuff tearing combined with humeral head migration (HHM). The aim of this study is to investigate the quantitative characteristics of this migration and its association with glenoid erosions and prearthropathy scapular anatomy. We quantified HHM and prearthropathy scapular anatomy of 64 RCTA patients with statistical shape modeling-based techniques. Glenoid erosion was classified according to Sirveaux et al. A cutoff value for confirming HHM was 5 mm based on a control group of 49 patients. Group 1 (RCTA without HHM) consi

Can Rapid Progression in Nonambulatory Cerebral Palsy Scoliosis Be Predicted Using Humeral Head Ossification?

BACKGROUND: Patients with cerebral palsy scoliosis (CPS) experience higher complication rates compared with idiopathic scoliosis and often present for surgery with larger curves. Prediction of an inflection point for rapid deformity progression has proven difficult. A proximal humerus-based skeletal maturity staging system (HS) has been recently validated and is commonly visible on the posteroanterior radiograph. The authors hypothesize that this system can be used to identify a period at which CPS may progress rapidly, perhaps facilitating discussion of timely surgical intervention. METHODS:

[Research progress of greater tubercle fixation and rotator cuff repair in humeral head replacement].

OBJECTIVE: To summarize the research progress of the greater tubercle fixation and the rotator cuff repair in humeral head replacement. METHODS: The literature about proximal humerus fracture and humeral head replacement in recent years was extensively consulted and analyzed. RESULTS: The greater tubercle fixation and the attached rotator cuff repair have great influence on the function of shoulder joint after humeral head replacement. It is difficult to make an objective comparison because of lack of direct comparison between various methods, unified standards of grading, and limited number

Radiographic humeral head restoration after total shoulder arthroplasty: does the stem make a difference?

BACKGROUND: Humeral stem designs for total shoulder arthroplasty have varied over the years, with a recent trend toward shorter stems. The purpose of this study was to examine the impact of humeral component stem length on the ability to restore the native humeral head anatomy. METHODS: We performed a retrospective review including patients who underwent total shoulder arthroplasty for primary osteoarthritis between 2007 and 2017 with complete operative reports and adequate radiographs. Surgical data including stem design were collected. Preoperative and postoperative radiographic measurement

Glenoid and humeral head bone loss in traumatic anterior glenohumeral instability: a systematic review.

PURPOSE: The aim of this systematic review is to analyze outcomes of surgical procedures for glenoid and/or humeral bony defects, performed singularly or in combination, in patients with traumatic anterior glenohumeral instability. A secondary aim is to establish in clinical settings which percentage of glenoid or humeral bone loss needs to be treated with a bony procedure to avoid recurrence of dislocation. METHODS: A systematic review of the literature according to the PRISMA guidelines was performed. A comprehensive search of PubMed, Medline, CINAHL, Cochrane, Embase and Google Scholar dat

Biomechanical comparison of a proximal humeral locking plate using two methods of head fixation.

HYPOTHESIS: Locking plates have emerged as the implant of choice for stabilization of proximal humeral fractures. The biomechanical properties of a locked plating system using smooth pegs vs threaded screws for fixation of the humeral head were compared to test the hypothesis that there would be no biomechanical difference between pegs and threaded screws. MATERIALS AND METHODS: Sixteen pairs of fresh frozen cadaveric humeri were randomized to have a surgical neck gap osteotomy stabilized with a locked plate using threaded screws (n=8) or smooth pegs (n=8). The intact contralateral humerus se
