OBJECTIVE: In this video we will demonstrate endo-ureterotomy using a cutting balloon for vesico-ureteric junction (VUJ) dilatation and stenting of the primary obstructive megaureter.
METHOD: For the technique, a 0.014 inch guide-wire is endoscopically inserted through the VUJ and allowed to curl in the megaureter. A 3 mm atherotome-bladed cutting balloon is inflated with iohexol contrast solution. Under fluoroscopy or cystoscopically, the stenotic VUJ segment is observed to open and post-dilated with a 4 mm simple balloon before JJ stent placement for six weeks.

Water-soluble nonionic x-ray contrast media have greatly improved the quality and safety of myelography. Toxic side effects are still observed however. The side effects are generally worse with the first nonionic agent, metrizamide, which has a glucoselike side group. Kounis Syndrome is an acute coronary syndrome in the setting of mast cell activation. Mast cell activation can be due to anaphylaxis, anaphylactoid reaction, allergies, or hypersensitivities. Three reported variants of Kounis Syndrome include: vasospastic allergic angina, allergic myocardial infarction, and stent thrombosis. Transient global amnesia (TGA) is an uncommon syndrome of recent memory deficit and inability to learn new data, usually resolving within 24 h. Two cases following use of non-ionic contrast media in cerebral angiography are described. The neuroanatomy of memory is reviewed. Possible aetiologies of TGA in relation to cerebral angiography include ischaemia (embolic, arterial spasm), epilepsy that may be primary or tumour-related and direct toxic effects of contrast media.

OBJECTIVE: The disadvantages of water-soluble gastrointestinal contrast agents include high osmolality, contrast dilution, and severe toxicity if aspirated. Perfluorocarbons are nontoxic in the lung and peritoneal cavity. Because perfluorocarbons are immiscible with water, they have no osmotic effect and cannot be diluted. Because these properties offer theoretical advantages over traditional gastrointestinal contrast agents, we compared two perfluorocarbons with barium and ionic and nonionic iodinated contrast material in a rat model of small-bowel obstruction.

Contrast-induced encephalopathy (CIE) following coronary angiography (CAG) is a very rare complication.
