Superioralization of the Inferior Alveolar Nerve and Roofing for Extreme Atrophic Posterior Mandibular Ridges with Dental Implants.

INTRODUCTION: Posterior mandibular ridges with extreme atrophy are usually combined with superficial location of the mental nerve and inferior alveolar nerves (IAN) and with a short residual mandibular ridge. As a result, dental implants cannot be placed in conjunction with IAN transposition alone. The aim of this paper is to introduce a new treatment approach to treat those patients. PATIENTS AND METHODS: Eleven patients with 18 extreme atrophic posterior mandibular ridges characterized by superficial location of the IAN and short residual ridge had been treated during a 4-year period. The t

Suprazygomatic Access for Continuous Bilateral Mandibular Nerve Block for Pain and Trismus Relief in the Tetraplegic Patient.

PURPOSE: Extraoral mandibular nerve block (MNB) is used in oropharyngeal surgery for analgesia and anesthesia. Repeated or continuous MNB has been used successfully as treatment for uncontrollable pain, masseter spasticity, and airway assessment. The usual technique involves transcutaneous infrazygomatic access. However, in some specific settings, this approach is not always feasible. MATERIALS AND METHODS: A continuous bilateral MNB with a suprazygomatic approach to the pterygomandibular space was used to resolve a case of refractory and painful trismus in a patient with tetraplegia. Rare involvement of herpes zoster in the mandibular branch of the trigeminal nerve: A case report and review of the literature.

Herpes zoster (HZ) infections rarely affect the mandibular branches of the trigeminal nerve. When the mandibular branches are involved, lesions may appear on the face, in the mouth, in the eye, or on the tongue. Additionally, this condition may be associated with devitalized teeth, internal resorption and spontaneous exfoliation of the teeth, and osteomyelitis of the alveolar bone. In this paper, the treatment of a case HZ of the mandibular branch of the trigeminal nerve is reported, and 22 articles on HZ cases with involvement of the mandibular branch are reviewed. This is the first literatur

Mandibular fracture as a complication of inferior alveolar nerve transposition and placement of endosseous implants: a case report.

Placement of endosseous implants and inferior alveolar nerve transposition is a treatment option for patients with an edentulous posterior mandible with inadequate bone height superior to the inferior alveolar canal. Complications associated with these procedures include infection, prolonged neurosensory disturbances, and/or pathologic fracture. This report presents the surgical management of a patient with a mandible fracture after inferior alveolar nerve transposition with concurrent placement of two endosseous implants.

Sensory impairment of the lingual and inferior alveolar nerves following removal of impacted mandibular third molars.

In a prospective study 1,106 impacted mandibular third molars were removed from 687 patients. Clinical, radiographic, and surgical factors were recorded. Postoperatively, we examined the modalities of common sensation in order to assess sensory deficit. The patients were followed up, until complete restitution occurred, or, if the sensibility failed to recover, for at least 6 months. A total of 3.6% of the operated sides demonstrated impairment of labial sensation, and 2.1% of lingual sensation. The vast majority of these disturbances subsided completely during the follow-up period. The incide

Clinical evaluation of inferior alveolar nerve block by injection into the pterygomandibular space anterior to the mandibular foramen.

The conventional inferior alveolar nerve block (conventional technique) has potential risks of neural and vascular injuries. We studied a method of inferior alveolar nerve block by injecting a local anesthetic solution into the pterygomandibular space anterior to the mandibular foramen (anterior technique) with the purpose of avoiding such complications. The insertion angle of the anterior technique and the estimation of anesthesia in the anterior technique were examined. The predicted insertion angle measured on computed tomographic images was 60.1 +/- 7.1 degrees from the median, with the sy

Aetiological aspects of mandibular tooth agenesis--focusing on the role of nerve, oral mucosa, and supporting tissues.

The purpose of the present investigation was to consider possible aetiological aspects of mandibular tooth agenesis by comparing the pattern of mandibular molar and premolar agenesis with radiological evidence of the mandibular canal. Orthopantomograms from 33 children affected by agenesis of at least four premolar and/or molar teeth in the mandible were investigated. Three children were affected by ectodermal dysplasia. Two anthropological mandibles with absence of mandibular canals were included in the study for comparison. According to the investigation, agenesis should be divided into thre

Mandibular malignant peripheral nerve sheath tumour in a patient with a history of radiotherapy for cerebellopontine angle ganglioneuroma.

Malignant peripheral nerve sheath tumours (MPNSTs) are rare, aggressive sarcomas. There are only a few reports of their occurrence in the mandible involving patients with a prior history of radiation therapy. This case report describes a mandibular MPNST occurring after subtotal resection and radiotherapy for a cerebellopontine angle region ganglioneuroma, highlighting the diagnostic considerations regarding secondary versus de-novo disease, and the importance of close clinical and radiologic follow-up in patients with high-risk ganglioneuroma, particularly after incomplete resection and radio

Does the application of counter-clockwise rotation in mandibular advancement increase the stress on the inferior alveolar nerve?

Sagittal split ramus osteotomy (SSRO) is a reliable surgical procedure for the correction of maxillofacial deformities. However, the most common complication that occurs after the procedure is a neurosensorial disturbance of the inferior alveolar nerve (IAN). Especially in individuals with retrognathic mandible or sleep apnoea, it may be necessary to perform a large amount of mandibular advancement and counter-clockwise (CCW) rotation depending on the angle of the mandibular plane. The likelihood of prolonged neurosensorial disturbance is increased in such patients. The objective of this exper

The Mandibular Canal: A Study to Determine If Cortical Bone Exists as a Protective Roof for the Inferior Alveolar Nerve.

Placement of dental implants has many inherent risks, of which all clinicians must be aware. One of the most important concepts in dental implantology is avoidance of the vital structures present in the human jaws, which can have both immediate as well as long-standing implications. The inferior alveolar nerve (IAN) is one of these vital structures located in the posterior body of the mandible. Invasion of this sensory nerve with a dental implant can result in transient or permanent paresthesia, anesthesia, or dysesthesia. Radiographic imaging is done routinely order to assess the bone volume

[Coronectomy - A treatment option for mandibular third molars with a close anatomical relationship to the inferior alveolar nerve.].

Extractions of mandibular third molars are a common procedure in dental practice. In cases where a close anatomical relationship of the root and the inferior alveolar nerve exists, the risk of transient or even permanent nerve injury increases. Nerve injury is difficult to manage and symptoms may cause severe patient discomfort, i.e. lifelong dysesthesia, i.e. painful sensation of the lower lip and chin area on the affected side. An alternative surgical procedure is coronectomy, which has first been described 25 year ago. Instead of complete tooth removal only the crown is extracted and the ro

Facial nerve paralysis and frey syndrome in an infant following removal of an internal mandibular distraction device.

Mandibular distraction using an implantable device has become a widely accepted and utilized procedure for the treatment of retrognathia. Although excellent results have been reported and observed with distraction osteogenesis, complications such as facial nerve injury have been previously reported. Often, this injury is usually temporary and corrects over the course of time. Frey syndrome has been classically described as an injury or severance of the auricotemporal branch of the trigeminal nerve. It is commonly seen as a complication of parotid surgery and has never been reported in associat
