Endoscopic endonasal transmaxillary ligation of a feeding artery and coblation plasma technology enables en bloc resection of advanced juvenile nasopharyngeal angiofibroma without preoperative embolization.

Juvenile nasopharyngeal angiofibroma (JNA) is a hypervascular tumor and uncontrolled hemorrhage makes its removal very difficult. Although preoperative intravascular embolization of a feeding artery is recommended, serious complications such as iatrogenic thrombosis in the brain and insufficient decrease in blood flow to the tumor are concerns. Recently, coblation plasma technology has been reported to be useful for tumor removal with minimum hemorrhage under a clear surgical field. Here we report successful removal of advanced JNA without preoperative embolization, using intraoperative ligati

Bilateral pterygomeningeal arteries penetrating the skull anterior to the foramen ovale.

PURPOSE: The pterygomeningeal (accessory meningeal) artery is an inconstant branch of the middle meningeal or maxillary artery. A variant that penetrates the skull anterior to the foramen ovale is rare, and bilateral coexistence has not been reported. A cadaver with bilateral variants of this type is described. METHODS: The head of a 79-year-old man was examined during a gross anatomy course. Origin, course, and relations of the variant vessels were recorded on both sides. RESULTS: On the left, an accessory middle meningeal branch of the middle meningeal artery penetrated the greater wing ap

Buccal transposition flap for closure of maxillary lip defects in 5 dogs.

OBJECTIVE: To describe the technique and outcomes of the closure of maxillary lip defects using a buccal transposition flap and to identify potential routes of vascular supply to the flap. ANIMALS: Five dogs treated clinically and 1 cadaveric dog head. STUDY DESIGN: Short case series and cadaveric study. METHODS: A left maxillary labial defect and a buccal transposition flap were created on a cadaver head. Iodinated contrast was injected into the left common carotid artery and computed tomography was performed to assess the vascular supply. Medical records were reviewed for all dogs that un

Evaluation of the Posterior Superior Alveolar Artery Using Cone Beam Computed Tomography.

BACKGROUND: Maxillary posterior tooth region is an important area with respect to periapical surgery, implant placement, and sinus lifts. Posterior superior alveolar artery (PSAA) is located on the lateral wall of maxillary sinus and may become injured during such surgical procedures. Therefore, knowledge of the subject is essential to a clinician. The goal was to determine the anatomical relationship of posterior superior alveolar artery to the floor of maxillary sinus and alveolar crest. In our study we attempted to present the locations and course of posterior superior alveolar artery (PSAA

Direct superficial temporal artery access for middle meningeal artery embolization.

Middle meningeal artery embolization has become an important option in the management of subdural hemorrhages with multiple prospective studies demonstrating efficacy and randomized controlled trial data on the way. Access to the middle meningeal artery is usually achieved via the external carotid artery to the internal maxillary artery, then the middle meningeal artery. We report a case where a patient with symptomatic left-sided chronic subdural hemorrhage also had an external carotid artery occlusion. Direct puncture of the superficial temporal artery allowed retrograde access to the intern

[Endonasal coagulation of the sphenopalatine artery in severe posterior epistaxis].

BACKGROUND: Until a few years ago the surgical method of choice in treating uncontrollable nosebleeds from the posterior part of the nose was the transantral ligation of the maxillary artery as described by Seiffert (Caldwell-Luc approach). We introduce a surgical method to expose and coagulate the sphenopalatine artery through an endonasal approach. METHOD: The middle meatus of the nose is exposed with a self supporting nasal speculum under the microscope (focus: 300 mm) and the maxillary sinus is opened through the posterior fontanelle. The medial wall of the maxillary sinus is removed from

Use of percutaneous arterial embolization for treatment of intractable epistaxis in three dogs.

Intractable epistaxis can be a life-threatening condition. Surgical options are presently limited to ligation of the carotid artery. Extensive collateralization develops after occlusion of the carotid artery, so the benefits are likely only transient and the procedure cannot be repeated. In humans, endovascular treatment avoids many of the complications associated with surgery and has a lower recurrence rate than that associated with ligation of the internal maxillary artery. In 3 dogs with intractable epistaxis, embolization of the terminal portion of the maxillary artery was performed with p

Traumatic aneurysms of the face and temple: a patient report and literature review, 1644 to 1998.

The branches of the external carotid artery are protected from injury in most locations by an adequate buffer of soft tissue. On occasion, the vessels approach the surface to cross bone structures, and in these key areas they become vulnerable to blunt trauma. The facial, superficial temporal, and terminal branches of the internal maxillary arteries are the branches most often affected via this mechanism of injury. In addition, damage to deeper branches of the internal maxillary artery and to the subparotid portion of the superficial temporal artery has been reported secondary to maxillary fra

Importance of anatomy in dental implant surgery.

BACKGROUND: The knowledge of nerves and vessels in the maxillofacial region, particularly the anatomical structures in the maxilla, mandible, tongue muscles, and salivary glands, is essential for dental surgeons. In addition, the structures in the mandibular canal, palate, and maxillary sinus should be understood well. HIGHLIGHT: The arteries and nerves in the maxillofacial region were observed in this study. Some variations in the origin of the inferior alveolar artery were found. Notably, the variations in the origin of the inferior alveolar artery from that of the external carotid artery a

Simulation of suprazygomatic and infrazygomatic approaches for maxillary nerve block in children.

PURPOSE: To compare the feasibility and risk of arterial injury associated with suprazygomatic and infrazygomatic maxillary nerve block approaches in children using 3D virtual reality. METHODS: Suprazygomatic, anterior and posterior infrazygomatic approaches were simulated bilaterally using computed-tomography angiography datasets of 89 children (0-18 years). Feasibility and vascular safety were assessed by positioning trajectories from the simulated needle end-point to the skin in respect to bones and maxillary artery, respectively. Solid angles and skin entry areas were quantified, correlat

Maxillary sinusitis complicated by stroke.

INTRODUCTION: Neurological complications of acute sinusitis are exceptional, but potentially serious. CASE REPORT: The authors report the case of a 6-year-old diabetic girl who presented with middle cerebral artery ischemic stroke secondary to inflammatory arteritis of the left internal carotid artery in a context of bilateral acute maxillary sinusitis. MRI confirmed ischaemic stroke associated with carotid arteritis and complete obstruction of the maxillary sinuses. A favourable outcome was observed after endoscopic drainage of the sinuses associated with broad-spectrum antibiotic therapy.

Major vascular complications of orthognathic surgery: hemorrhage associated with Le Fort I osteotomies.

Major intraoperative or postoperative bleeding associated with Le Fort I osteotomies can be venous and/or arterial in nature. Arterial hemorrhage generally involves the maxillary artery and its terminal branches. Arterial hemorrhage tends to be more persistent and can be recurrent, which makes it more difficult to manage. Postoperative bleeding following Le Fort I osteotomies generally presents as epistaxis and usually occurs initially within the first 2 weeks following surgery. Treatment modalities that have been used to successfully arrest postoperative hemorrhage include anterior and/or pos

Bleeding from posterior superior alveolar artery in Le Fort I fracture.

This article describes a patient in whom significant oronasal bleeding developed after an injury to posterior superior alveolar artery of the maxillary artery in Le Fort I fracture.A 34-year-old man had a facial injury after an explosion of a furnace. Computed tomography demonstrated Le Fort I fracture, right open zygomatic tripod fracture, and open nasal bone fracture. Blood pressure, hemoglobin, and hematocrit levels had fallen to 110/60 mm Hg, 5.7 mg/dL, and 16.1%, respectively, 10 hours after injury despite continuous blood transfusion. Selective digital angiography confirmed an injury to

Revisiting major anatomical risk factors of maxillary sinus lift and soft tissue graft harvesting for dental implant surgeons.

The anatomical variations of the maxillary sinus septa, greater palatine artery, and posterior superior alveolar arteries might cause unexpected complications when they are damaged. Dentists who know these structures well might hope to learn more practical knowledge to avoid and assess injury preoperatively. Therefore, this review paper aimed to review the reported anatomy and variations of the maxillary sinus septa, greater palatine artery/nerve, and posterior superior alveolar artery, and to discuss what has to be assessed preoperatively to avoid iatrogenic injury. To assess the risk of inju

Embolization in the head and neck region.

With our results we want to demonstrate the usefulness of the presurgical embolization of tumours in areas where the operation of the highly vascularized tumours is very difficult or even impossible. In certain cases of angiomatous lesions, better results can be achieved by means of embolization than by surgery. For a safe and successful intravascular embolization an optimal technique with DSA, an experienced team and a cooperative patient are absolute prerequisites for interventional procedures in head and neck regions. CT in axial and coronal scans are important to outline the lesion, especi

Arterial branches to the temporal muscle.

UNLABELLED: The temporal muscle is greatly used for forming myofascial flaps and transposition in cases of reconstructive and reparative plastic surgery of the face and neck. Despite this important application, lack of success has been reported, probably because of lack of knowledge of the anatomical characteristics of this muscle, such as variations in its blood supply. OBJECTIVES: To investigate the blood supply of the temporal muscle in order to provide an anatomical basis for reconstructive and reparative surgery of the face using flaps from the temporal muscle. This study was conducted b

Intraosseous anastomosis in the maxillary sinus.

AIM: During sinus-lift surgery, certain intra-osseous vessels may be accidentally cut and this cause bleeding complications in approximately 20% of osteotomies. Therefore, understanding vascular details of the maxilla is very important for the surgeon. Here, we have given an anatomical overview of maxillary sinus vascularization through anatomical dissection. We have analyzed the distribution, localization and distance from the alveolar ridge of intraosseous branches of the maxillary artery found during sinus lift surgery. METHODS: Fifty-six maxillary bone doors were made bilaterally in twent
