BACKGROUND: Free nipple grafting (FNG) has been long reported as a viable alternative for aesthetic results and patient satisfaction in the setting of reduction mammaplasty in severe macromastia. This study demonstrates that FNG concepts used for reduction mammaplasty can be applied to autologous reconstruction following mastectomies in patients who do not qualify for traditional nipple sparing mastectomies.
METHODS: A retrospective chart review was performed at two institutions for patients who underwent immediate autologous breast reconstruction with simultaneous FNG by the senior author (K

Possible ductal spread to the nipple base on imaging generally leads breast surgeons to perform either partial mastectomy, including nipple resection, or total mastectomy in breast cancer patients. Our nipple base resection techniques, however, can provide safe nipple preservation for many breast cancer patients, even when imaging suggests ductal spread to the nipple base. An 86-year-old woman with breast cancer near the nipple-areolar complex showed suspected ductal spread to the nipple base on preoperative imaging. Due to the patient's strong preference for nipple preservation, she underwent

INTRODUCTION: Nipple sparing mastectomy (NSM) can improve domains of body image and sexuality, but preservation of the nipple-areola complex (NAC) does not guarantee sensory maintenance. NAC neurotization/reinnervation techniques have been proposed to restore sensitivity and optimize quality of life. The aim was to synthesize evidence on ACN reinnervation after NSM with immediate reconstruction, including techniques, objective measures, PROs, and complications.
METHODS: This is a systematic review according to PRISMA (CRD420261331412). We searched PubMed/MEDLINE, Embase, and Cochrane, without

Ptosis can be a relative contraindication to nipple sparing mastectomy (NSM). Although a mastopexy-type approach can be performed during mastectomy, this requires very well perfused skin flaps and possibly intraoperative adjustments with the oncological breast surgeon. The purpose of this study is to assess risk factors that result in the nipple surgically raised secondarily in standard NSM with prepectoral tissue expander (TE) reconstruction to help guide surgical decision-making and patient counseling. Patients at our center who underwent NSM and immediate prepectoral TE reconstruction were

Background: The insertion of chest drains (CD) in trauma patients is a lifesaving, albeit high-risk intervention. Safe insertion of CD in settings where aids like ultrasound are not available relies on a landmark technique defining the so-called triangle of safety. The inferior margin of this triangle is identified by nipple height, which is thought to correspond to the fifth intercostal space (ICS). Training manikins are modeled after a lean male body and oftentimes insinuating jokes fuel uncertainty about the height of the nipple as a reliable landmark in female trauma patients. This study a

BACKGROUND: Nipple-sparing mastectomy (NSM) is increasingly used in breast cancer patients, as it offers better cosmetic outcomes and improves quality of life. Nipple-areola complex (NAC) involvement must be accurately determined to identify which patients may be candidates for NSM. We aimed to identify the predictors of NAC involvement and develop a clinical predictive model to determine the patients for whom NAC preservation may be considered.
PATIENTS AND METHODS: Patients (n = 168) with primary operable breast cancer who underwent subcutaneous mastectomy for breast reconstruction at Saita

BACKGROUND: Although the free-nipple breast-reduction technique is essentially an amputation, achieving aesthetic results still is important. The authors present their technique for free nipple-areola complex (NAC) transfer over the superomedial or superior pedicle full-thickness flaps in patients for whom a free-nipple technique is inevitable due to certain risk factors.
METHODS: The study included 25 patients who underwent surgery with the aforementioned method for addressing severe gigantomastia. The patients had a mean age of 43 years (range 34-59 years) and a mean body mass index (BMI) o

BACKGROUND: Milk flow rate may play an important role in an infant's ability to safely and efficiently coordinate sucking, swallowing, and breathing during feeding.
PURPOSE: To test milk flow rates from bottle nipples used in the hospital and after discharge.
METHODS: Bottle nipples used in hospitals (10 unique types) and available nationwide at major retailers (15 unique types) were identified. For each of the 25 nipple types, 15 nipples of that type were tested by measuring the amount of infant formula extracted in 1 minute by a breast pump. Mean milk flow rate (mL/min) and coefficient of

Pontocerebellar hypoplasia (PCH) can occur as an isolated entity or part of a syndrome. PCH has been reported with facial dysmorphism, ocular anomalies, and genital anomalies, but the co-occurrence of all four has not been previously described. We report on four patients, born to two consanguineous families that are not related to one another, with distinctive facial features (short forehead, laterally extended, medially flared eyebrows), corneal dystrophy, underdevelopment of labioscrotal folds, and nonprogressive PCH. In addition, the patients show hair extruding from the lactiferous ducts,

BACKGROUND: For patients who are unable to undergo nipple-sparing mastectomy, reconstruction of the nipple-areola complex has been shown to promote greater satisfaction in cosmetic outcome, body image, and sexual relationships. Although a variety of techniques have been developed to optimize the shape, size, and mechanical properties of the reconstructed nipple-areola complex, maintenance of sustained nipple projection over time remains a challenge for plastic surgeons.
METHODS: Three-dimensionally printed poly-4-hydroxybutyrate (P4HB) scaffolds were designed and fabricated filled with either

PURPOSE: Single-use, laser-cut, slow-flow nipples were evaluated for their effect on respiration and milk ingestion in 13 healthy preterm infants (32.7-37.1 weeks postmenstrual age) under nonlaboratory, clinical conditions.
METHOD: The primary outcomes of minute ventilation and overall milk transfer were measured by using integrated nasal airflow and volume-calibrated bottles during suck bursts and suck burst breaks during slow-flow and standard-flow nipple bottle feedings. Wilcoxon signed-ranks tests were used to test the effect of nipple type on both outcomes.
RESULTS: Prefeeding minute ve

BACKGROUND: Nipple-sparing mastectomy (NSM) after nipple-repositioning surgery (mastopexy or breast reduction) is increasingly used in patients with large or ptotic breasts to improve vascularity to the nipple-areolar complex (NAC) and decrease the risk of necrosis. This systematic review aimed to evaluate the safety and optimal timing of this approach.
METHODS: Preferred reporting items of systematic reviews and meta-analyses guidelines were followed. PubMed, Scopus, and Web of Science databases were searched on February 24, 2024. All observational studies reporting complication outcomes and

Patients with central breast cancers historically underwent mastectomy, as the aesthetic implications of removing the nipple-areola complex and central breast tissue left disfiguring defects after breast conservation. The introduction of oncoplastic techniques allowed for central lumpectomies in ptotic patients, as the excess skin and gland could be mobilized centrally to fill the defect and even immediately reconstruct a nipple and areola. These reconstructions used excess skin on the Wise pattern vertical limbs to create a nipple, or on a "neopedicle" where both the areola and nipple were re

BACKGROUND: In ptotic/macromastic breasts, nipple-sparing mastectomy (NSM) risks ischemic complications. Staging with nipple repositioning (mastopexy or breast reduction) may improve perfusion, but uncertainty remains regarding safety and optimal timing.
METHODS: PRISMA guidelines will be followed. The database search will be conducted using MEDLINE (PubMed), Scopus, Web of Science, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, and ClinicalTrials.org targeting studies from inception to search date. BACKGROUND: Zuska's disease, a rare chronic inflammatory condition of the subareolar breast tissue, is characterized by recurrent non-puerperal abscesses and lactiferous duct fistulas. This condition poses significant therapeutic challenges because of its high recurrence rates. This study aimed to assess the effectiveness of a novel surgical approach integrating complete nipple core excision and inverted nipple correction in decreasing recurrence rates among patients with Zuska's disease, in comparison with conventional treatments.
METHODS: A retrospective analysis was performed on 109 patien

BACKGROUND: Single-port endoscopic nipple-sparing mastectomy (E-NSM) with implant-based breast reconstruction (IBBR) represents a promising surgical technique. However, the surgical procedures are complex and lack standardized and systematic surgical protocols. The seven-step method was introduced as a means of simplifying the complex surgical procedures, with detailed descriptions provided of the key points and critical aspects of the surgery.
METHODS: The medical records of patients who received single-port E-NSM with IBBR guided by the seven-step method from January 2022 to March 2024 were
