International Collaboration to Develop and Harmonize Drug Interaction Guidance for Nirmatrelvir/Ritonavir During COVID-19: Lessons Learned for Future Pandemic Preparedness.

In December 2021 and January 2022, the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) antiviral drug, nirmatrelvir/ritonavir, was authorized in the United States, Canada, and Europe. However, ritonavir has significant drug-drug interaction (DDI) potential, and information resources were incomplete or provided conflicting advice for certain DDIs. Within the challenging pandemic setting, nirmatrelvir/ritonavir was being prescribed largely by clinicians unfamiliar with ritonavir. Coronavirus disease 2019 (COVID-19) affects people with comorbid conditions and polypharmacy who could b

School and preparedness officials' perspectives on social distancing practices to reduce influenza transmission during a pandemic: Considerations to guide future work.

The objective of this qualitative study was to explore the perspectives of school and preparedness officials on the feasibility of implementing a range of social distancing practices to reduce influenza transmission during a pandemic. In the summer of 2017, we conducted 36 focus groups by teleconference and webinar lasting 90 min with school and preparedness stakeholders from across the United States. We identified and characterized 11 themes arising from the focus group protocol's domains as well as unanticipated emergent themes. These themes were: the need for effective stakeholder communica

Research and community engagement during the COVID- 19 pandemic in a resource-limited setting: a mixed methods study with epidemic preparedness implications.

BACKGROUND: When WHO declared the SARS-CoV- 2 respiratory virus pandemic in 2020, Uganda was unprepared to prevent and control its spread or manage the COVID- 19 and non-COVID patients. A community trial to assess the efficacy of an intensive community engagement and risk communication (CERC) was contemplated. However, a baseline assessment of the existing CERC status and the challenges of conducting community research in the early months of the pandemic was necessary. METHODS: A community baseline qualitative research (in-depth interviews, key informant interviews, focus group discussions, a

Preparedness for maternal and offspring health surveillance in pandemics: Lessons learned from studies on COVID-19 using Scandinavian registry data.

When the COVID-19 pandemic emerged, pregnant women were quickly identified as a high-risk group. It became a priority to clarify any risks associated with COVID-19 infection, and later vaccination, to pregnant women and their offspring. The Nordic registries constituted unique resources to quickly answer these questions. Our findings from the Nordic registries demonstrated that pregnant women with COVID-19 face a higher risk of hospitalization compared to non-pregnant women of reproductive age, and a higher risk of venous thromboembolism and stillbirth. However, the majority of studies proved

Toolkit for Quickly Generating and Characterizing Molecular Probes Specific for SARS-CoV-2 Nucleocapsid as a Primer for Future Coronavirus Pandemic Preparedness.

Generating and characterizing immunoreagents to enable studies of novel emerging viruses is an area where ensembles of synthetic genes, recombinant antibody pipelines, and modular antibody-reporter fusion proteins can respond rapidly. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) continues to spread through the global population causing widespread morbidity, mortality, and socioeconomic chaos. Using SARS-CoV-2 as our model and starting with a gBlocks encoded nucleocapsid (N) gene, we purified recombinant protein from E. coli, to serve as bait for selecting semisynthetic nanobodi

Avian influenza virus cross-infections as test case for pandemic preparedness: From epidemiological hazard models to sequence-based early viral warning systems.

Pandemic preparedness starts with an early warning system of viruses with a pandemic potential. Based on information collected in a multitude of surveys, hazard models were developed identifying influenza viruses presenting a pandemic threat. Scores are attributed for 10 viral traits by expert panels which identified avian influenza viruses (AIV) belonging to subtypes H7N9 and H5N1 as representing the greatest pandemic risk. In 2013, more than 100 human cases infected with AIV H7N9 were observed in China. Case fatality rate (CFR) was high (27%), but the human-to-human transmission rate was low

Design and computational evaluation of a novel multi-epitope hybrid vaccine against monkeypox virus: Potential targets and immunogenicity assessment for pandemic preparedness.

Monkeypox is a type of DNA-enveloped virus that belongs to the orthopoxvirus family, closely related to the smallpox virus. It can cause an infectious disease in humans known as monkeypox disease. Although there are multiple drugs and vaccines designed to combat orthopoxvirus infections, with a primary focus on smallpox, the recent spread of the monkeypox virus to over 50 countries have ignited a mounting global concern. This unchecked viral proliferation has raised apprehensions about the potential for a pandemic corresponding to the catastrophic impact of COVID-19. This investigation explore

Despite good intentions, the regulation on in vitro diagnostic medical devices (IVDR) in Europe could impact negatively on preparedness and response for the next pandemic.

Currently, there is concern and uncertainty in the European and North American markets for in vitro diagnostics regarding the regulation of in vitro diagnostic tests. In the European Union, starting from May 2022, the regulation on vitro diagnostic medical devices (IVDR) has replaced the directive on in vitro diagnostic medical devices (IVDD). The IVDR, while written with the good intentions to ensure patient safety and health while supporting innovation and transparency, has resulted in uncertainty, instances of disruption of diagnostic development, and concerns related to pandemic preparedne

Assessing rapid response mobile laboratory (RRML) capacities in Europe and Africa to improve One Health disease surveillance activities and pandemic preparedness across two continents.

BACKGROUND: Europe and Africa are increasingly affected by (re-)emerging risk group 3 and 4, zoonotic viral disease epidemics, which not only require diagnostic BSL-3/4 laboratory capacity but also a One Health-based control strategy for efficient outbreak containment. In many European and African countries such laboratory capacity is often not readily available, and rapid response mobile laboratories (RRMLs) can play important, complementary roles in outbreak responses and pandemic preparedness activities on national, regional and international level. MAIN BODY: The aim of the present review

Rapid response infrastructure for pandemic preparedness in a tertiary care hospital: lessons learned from the COVID-19 outbreak in Cologne, Germany, February to March 2020.

The coronavirus disease (COVID-19) pandemic has caused tremendous pressure on hospital infrastructures such as emergency rooms (ER) and outpatient departments. To avoid malfunctioning of critical services because of large numbers of potentially infected patients seeking consultation, we established a COVID-19 rapid response infrastructure (CRRI), which instantly restored ER functionality. The CRRI was also used for testing of hospital personnel, provided epidemiological data and was a highly effective response to increasing numbers of suspected COVID-19 cases.
