Quality of Life Changes in Respiratory Infection Trajectory with Point of Care Testing During Clinical Consultation.

BACKGROUND: This study evaluates the impact of a point-of-care (POC) testing strategy and subsequent treatments for respiratory tract infections (RTIs) on patients' health-related quality of life (QoL). MATERIALS AND METHODS: The PRUDENCE trial, with a sample of 2639 patients, showed that a POC testing strategy for RTIs (C-reactive protein, group A streptococcus, and influenza A/B, with optional SARS-CoV-2 testing) did not reduce antibiotic prescribing nor had a measurable impact on time to patient recovery compared to usual care. However, differences in patient-reported QoL may still exist,

Mixed-methods evaluation of point-of-care hepatitis C virus RNA testing in a Scottish prison.

OBJECTIVES: Hepatitis C virus (HCV) poses a global public health threat. Prisons are a focus of prevention efforts due to high infection burdens. Expedition of treatment for incarcerated people is critical, as many are short-term sentenced. We evaluated point-of-care (PoC) HCV RNA testing in a maximum-security Scottish prison and assessed its impact on transition to treatment. We also evaluated costs and determinants of implementation. DESIGN: Mixed-methods evaluation of a single-centre care pathway pilot using National Health Service (NHS) data from 2018 to 2021. Descriptive statistics and s

Feasibility and impact of near-point-of-care integrated tuberculosis/HIV testing in Malawi and Zimbabwe.

OBJECTIVES: Near-point-of-care (POC) testing for early infant diagnosis (EID) and viral load expedites clinical action and improves outcomes but requires capital investment. We assessed whether excess capacity on existing near-POC devices used for TB diagnosis could be leveraged to increase near-POC HIV molecular testing, termed integrated testing, without compromising TB services. DESIGN: Preimplementation/postimplementation studies in 10 health facilities in Malawi and 8 in Zimbabwe. METHODS: Timeliness of EID and viral load test results and clinical action were compared between centralize

Detection of antibiotic resistance is essential for gonorrhoea point-of-care testing: a mathematical modelling study.

BACKGROUND: Antibiotic resistance is threatening to make gonorrhoea untreatable. Point-of-care (POC) tests that detect resistance promise individually tailored treatment, but might lead to more treatment and higher levels of resistance. We investigate the impact of POC tests on antibiotic-resistant gonorrhoea. METHODS: We used data about the prevalence and incidence of gonorrhoea in men who have sex with men (MSM) and heterosexual men and women (HMW) to calibrate a mathematical gonorrhoea transmission model. With this model, we simulated four clinical pathways for the diagnosis and treatment

Cellphone-Based Hand-Held Microplate Reader for Point-of-Care Testing of Enzyme-Linked Immunosorbent Assays.

Standard microplate based enzyme-linked immunosorbent assays (ELISA) are widely utilized for various nanomedicine, molecular sensing, and disease screening applications, and this multiwell plate batched analysis dramatically reduces diagnosis costs per patient compared to nonbatched or nonstandard tests. However, their use in resource-limited and field-settings is inhibited by the necessity for relatively large and expensive readout instruments. To mitigate this problem, we created a hand-held and cost-effective cellphone-based colorimetric microplate reader, which uses a 3D-printed opto-mecha

CRISPR/cas-based biosensors for point-of-care testing: a comprehensive review of signal readout strategies.

The CRISPR/Cas system has emerged as a transformative tool for nucleic acid detection, offering significant potential for point-of-care testing (POCT). However, translating CRISPR/Cas-based assays into practical POCT devices critically depends on the development of portable, sensitive, and user-friendly signal readout modalities. This review systematically compares four major readout modalities: fluorescence, electrochemical, colorimetric, and distance‑based readout, analyzing their mechanisms, analytical performance, and practical limitations. Key challenges, including sample preparation, amp

Ultrasensitive portable aptamer-electrochemical sensor for point-of-care testing of Alzheimer's biomarker amyloid-β oligomers.

Early and accessible diagnosis of Alzheimer's disease (AD) is crucial for effective intervention, yet current methods like amyloid-PET(positron emission tomography) imaging remain cost-prohibitive and lack scalability. Here, we present a miniaturized electrochemical platform for ultrasensitive detection of amyloid-β oligomers (AβOs), pathogenic biomarkers implicated in early AD progression. The system employs a disposable screen-printed electrode functionalized with gold nanoparticles (AuNPs) and thiolated aptamers, leveraging AuS bonds for oriented aptamer immobilization and AβO-specific reco

Wave-shaped microfluidic chip assisted point-of-care testing for accurate and rapid diagnosis of infections.

BACKGROUND: Early diagnosis and classification of infections increase the cure rate while decreasing complications, which is significant for severe infections, especially for war surgery. However, traditional methods rely on laborious operations and bulky devices. On the other hand, point-of-care (POC) methods suffer from limited robustness and accuracy. Therefore, it is of urgent demand to develop POC devices for rapid and accurate diagnosis of infections to fulfill on-site militarized requirements. METHODS: We developed a wave-shaped microfluidic chip (WMC) assisted multiplexed detection pl

A centrifugal microfluidic chip for point-of-care testing of staphylococcal enterotoxin B in complex matrices.

Staphylococcal enterotoxin B (SEB) is a typical biological toxin that causes food poisoning. Currently reported SEB detection methods have the drawbacks of sophisticated sample preparation and being time-consuming and labor-intensive. Herein, we propose a strategy based on an immune sandwich structure operating on a centrifugal microfluidic chip for point-of-care testing (POCT) of SEB. The fluorescent microparticle-labeled primary antibody (CM-EUs-Ab1), capture antibody (CAb), and goat anti-mouse IgG antibody (SAb) were modified on the bond area, T-area, and C-area, respectively. When SEB was

Design and Testing of a Single-Element Ultrasound Viscoelastography System for Point-of-Care Edema Quantification.

Management of fluid overload in patients with end-stage renal disease represents a unique challenge to clinical practice because of the lack of accurate and objective measurement methods. Currently, peripheral edema is subjectively assessed by palpation of the patient's extremities, ostensibly a qualitative indication of tissue viscoelastic properties. New robust quantitative estimates of tissue fluid content would allow clinicians to better guide treatment, minimizing reactive treatment decision making. Ultrasound viscoelastography (UVE) can be used to estimate strain in viscoelastic tissue,

Use of the "STANDARD G6PDTM" quantitative point-of-care test in neonates and infants.

Severe neonatal hyperbilirubinaemia represents a considerable cause of mortality and long term-morbidity in neonates born in low resource settings. Early identification of risk factors, such as glucose-6-phosphate dehydrogenase (G6PD) status, has the potential to prevent severe hyperbilirubinaemia and improve the clinical outcomes. The primary aim of the study was to assess equivalency of cord blood and neonatal capillary blood for diagnosis of G6PD deficiency using the quantitative point-of-care "STANDARD G6PDTM" test (SD Biosensor, Korea). The secondary aim was to analyse changes in G6PD act
