Drug-resistant tuberculosis (DR-TB) is an emerging health problem, challenging the effective control of global TB. Current treatment of DR-TB includes administration of multiple anti-TB drugs via oral and parenteral routes for a duration of 20-28 months. High systemic exposure, side effects and lengthy treatment time are problems affecting current treatment. The success rate of current lengthy treatment regimens is generally <50%. Bedaquiline, a new anti-TB drug is synergistic with pyrazinamide and in combination with moxifloxacin accelerates sputum-culture conversion. Therefore, a triple comb

BACKGROUND: Pharmacokinetic data on the first-line antituberculosis drugs using the World Health Organization (WHO) revised dosages for children are limited. We investigated the pharmacokinetics of these drugs in children who were mostly treated with revised dosages.
METHODS: Children with tuberculosis on first-line therapy for at least 4 weeks had blood samples collected at predose, 1, 2, 4, and 8 hours postdose. Drug concentrations were determined by validated liquid chromatography mass spectrometry methods, and pharmacokinetic parameters were calculated using noncompartmental analysis. Fac

The effect of five antimycobacterial drugs isoniazid, rifampicin, streptomycin, pyrazinamide and ethambutol on the generation of reactive oxidants by polymorphonuclear leucocytes was investigated in vitro, using N-formyl-L-methionyl-L-leucyl-phenylalanine (FMLP) activated and spontaneous luminol-enhanced chemiluminescence and myeloperoxidase-mediated iodination. Streptomycin, pyrazinamide and ethambutol had no effect on the assays at the concentrations investigated. Isoniazid as concentrations of 1.25 and 5 micrograms/ml and rifampicin at 100 micrograms/ml significantly inhibited iodination. R

BackgroundLong-term combination anti-tuberculosis regimens may lead to various types and severities of adverse drug reactions, potentially resulting in treatment discontinuation, interruption, or prolonged therapy.AimConsidering the lengthy duration of anti-tuberculosis combination regimens, it is crucial to monitor adverse drug reactions occurring during such treatment. This is the first hospital-based study conducted in western China to report adverse drug reactions and assess their prevalence and impact among children with tuberculosis receiving first-line anti-tuberculosis therapy.MethodsA

Rationale: Pretomanid is a new nitroimidazole with proven treatment-shortening efficacy in drug-resistant tuberculosis. Pretomanid-rifamycin-pyrazinamide combinations are potent in mice but have not been tested clinically. Rifampicin, but not rifabutin, reduces pretomanid exposures. Objectives: To evaluate the safety and efficacy of regimens containing pretomanid-rifamycin-pyrazinamide among participants with drug-sensitive pulmonary tuberculosis. Methods: A phase 2, 12-week, open-label randomized trial was conducted of isoniazid and pyrazinamide plus 1) pretomanid and rifampicin (arm 1), 2) p

Tuberculosis is one of the leading infectious causes of death and as such represents a major global health problem. Infants may develop congenital tuberculosis from an infectious mother or, most commonly, they may acquire postnatal disease by contact with an infectious adult source. Important epidemiologic, pathogenetic, and clinical data regarding the management of infantile disease are reviewed. Diagnostic evaluation includes tuberculin skin tests, chest radiography and other imaging studies, smears and cultures, examination of the cerebrospinal fluid, and polymerase chain reaction, as well

BACKGROUND: The REMEMBER (A5274) study found that the four-drug TB preventive regimen did not reduce mortality compared to isoniazid-only, raising adherence concerns. Using drug measurements and pharmacometrics, we assessed adherence in the four-drug arm by comparing participants who developed TB (cases) to those who did not (controls).
METHODS: Using a 1:4 matched case-control design, we analysed stored blood samples at weeks 2, 4, and 8 since treatment start. Rifampicin and pyrazinamide were measured, and adherence was assessed using two thresholds: i) lower limit of quantification (LLOQ) a

First line Anti-TB therapy with rifampicin, isoniazid, pyrazinamide, and ethambutol/streptomycin is very effective. However, major adverse reactions to antituberculous drugs can cause significant morbidity and mortality. Cutaneous adverse drug reaction (CADR) is one of the commonly observed major adverse events. This retrospective study looked at the cases of TB treated in Respiratory Unit, Penang Hospital from January 2004 to December 2005. Of 820 patients treated for active TB, 47 patients (25 females; 22 males) developed CADR (5.7%). CADRs observed include morbiliform rash (72.3%), erythema

A 73-year-old man from the Philippines with Type 2 diabetes mellitus and end-stage renal disease on peritoneal dialysis developed peritonitis. The infection was refractory to empiric therapy, and the pathogen was identified as Mycobacterium tuberculosis, when 4 weeks after collection, a preliminary positive AFB culture underwent PCR testing. He did not initially tolerate a regimen of isoniazid, rifampin, pyrazinamide, and ethambutol but temporarily tolerated rifabutin, levofloxacin, and ethambutol and ultimately tolerated a regimen of levofloxacin, ethambutol, and pyrazinamide. This case highl

Multidrug-resistant M. tuberculosis in HIV-infected people has not yet been reported in Switzerland, and there have been no nosocomial epidemics as they have recently occurred in the USA. We present the case of a 38-ear-old HIV-infected man who developed disseminated tuberculosis as AIDS-defining disease. Initially he was treated with isoniazid, pyrazinamide and rifampin. Due to the emergence of resistance to isoniazid and streptomycin, ethambutol was added for one month. Later the therapy was changed back to the initial three drugs. The patient responded well to this therapy, but five months

PURPOSE: Tuberculosis remains a major global public health problem. Adherence to antituberculous treatment is essential to prevent disease transmission and the emergence of multidrug-resistant tuberculosis. This study aims to evaluate adherence to first-line antituberculous medications in the United States.
METHODS: A retrospective cohort study was conducted using data compiled in the Merative MarketScan commercial and Medicaid claims databases from 2016 to 2022. High adherence was defined as a proportion of days covered of ≥80% over a 6-month follow-up period. The study included patients wit

OBJECTIVES: This study aimed to examine the efficacy of whole genome sequencing (WGS) in accurately predicting susceptibility profiles, potentially eliminating the need for conventional phenotypic drug susceptibility testing (pDST) for first-line antituberculosis drugs in routine tuberculosis diagnosis.
METHODS: Over the period of 2017 to 2020, 1114 Mycobacterium tuberculosis complex isolates were collected with drug susceptibility testing conducted using the MGIT960 system and WGS performed for predicting drug resistance profiles. In addition, we implemented a new algorithm with an updated W

Tuberculosis stands as a prominent cause of mortality in developing countries. The treatment of tuberculosis involves a complex procedure requiring the administration of a panel of at least four antimicrobial drugs for the duration of six months. The occurrence of treatment failure after the completion of a standard treatment course presents a serious medical problem. The purpose of this study was to evaluate antimicrobial drug resistant features of Mycobacterium tuberculosis associated with treatment failure. Additionally, it aimed to evaluate the effectiveness of second line drugs such as am

A direct in-line pre-column extraction technique in which guanidinium and ammonium sulfate are used, followed by column switching, was employed to analyze serum, plasma and cerebrospinal fluid samples of patients treated for tuberculous meningitis. Resolution of a wide range of polar to non-polar xenobiotics was obtained on a C8 silica column by using a linear gradient from a binary system consisting of solvent A (0.05 M KH2PO4) and solvent B (acetonitrile-isopropanol, 4:1, v/v). Apart from the antituberculosis drugs (isoniazid, pyrazinamide, ethionamide and rifampicin) the patients received u

The mechanisms responsible for the increased renal clearance of uric acid in the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) are not fully clarified. Studies using either pyrazinamide or probenecid, or both drugs but at an interval of several days, could not undoubtedly distinguish the 'hypersecretory theory' from the one favoring a defect in either post- or presecretory reabsorption. We decided to do a combined pyrazinamide and probenecid test in 5 patients with hyponatremia due to SIADH in order to evaluate more clearly the respective importance of these different pat

In diabetes, the serum uric acid level is low due to increased urate clearance; however, its mechanism remains unknown. We examined seven maturity-onset diabetic patients with rarely reported hypouricemia due to renal tubular abnormality to better understand the renal handling of urate by the diabetic kidney. The results of studies indicate that the increase in urate clearance was entirely accounted for by increased pyrazinamide-suppressible urate clearance. The maximal uricosuric response to probenecid administration was exaggerated. The effects of drugs on urate clearance were similar to tho

Tuberculous meningitis is a devastating manifestation of tuberculosis, disproportionately affecting children and persons with HIV. Despite decades of clinical experience, diagnosis is often delayed and outcomes remain poor. At the US-Mexico border, management is further complicated by the high prevalence of Mycobacterium bovis, a member of the Mycobacterium tuberculosis complex intrinsically resistant to pyrazinamide, one of the few first-line agents with reliable central nervous system penetration. We present a case of M. bovis meningitis in a patient residing near the US-Mexico border, highl

Treatment of latent tuberculosis infection (LTBI) is considered an essential component of TB control programs in several industrialized countries and is being used in selected situations in developing countries. In 2000, the American Thoracic Society (ATS) and Centers for Disease Control and Prevention (CDC) published new recommendations on the diagnosis and treatment of LTBI. These guidelines emphasized targeted tuberculin testing as a means of diagnosis in populations at risk, and expanded the choice of drug regimens to be used for treatment. After the implementation of the recommendations,

The relationships between antituberculosis drug exposure and treatment effects on humans receiving multidrug therapy are complex and nonlinear. In patients on treatment, an analysis of the rate of decline in the sputum bacillary burden reveals two slopes. The first is the α-slope, which is thought to reflect bactericidal effect, followed by a β-slope, which is thought to reflect sterilizing activity. We sought to characterize the effects of standard first-line treatment on sterilizing activity. Fifty-four patients receiving combination therapy for pulmonary tuberculosis in a clinical trial had

BACKGROUND: Tuberculosis is the world's leading infectious disease killer. We aimed to identify shorter, safer drug regimens for the treatment of tuberculosis.
METHODS: We did a randomised controlled, open-label trial with a multi-arm, multi-stage design. The trial was done in seven sites in South Africa and Tanzania, including hospitals, health centres, and clinical trial centres. Patients with newly diagnosed, rifampicin-sensitive, previously untreated pulmonary tuberculosis were randomly assigned in a 1:1:1:1:2 ratio to receive (all orally) either 35 mg/kg rifampicin per day with 15-20 mg/
