Comfort rules for face masks among healthcare workers during COVID-19 spread.

BACKGROUND: The outbreak of Coronavirus disease 2019 (COVID-19) made imperative the use of protective devices as a source control tool. As there is no definite antiviral treatment and effective vaccine, the only efficient means of protecting and mitigating infectious contagion has been the use of personal protective equipment, especially by healthcare workers. However, masks affect the humidification process of inhaled air, possibly leading to a basal inflammatory state of the upper airways. STUDY DESIGN: This is a single-center observational study conducted at the University Hospital of Cata

Infection control for COVID-19 in hospital examination room.

Healthcare providers are vulnerable to infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) because of their close proximity to patients with coronavirus disease 2019. SARS-CoV-2 is mainly transmitted via direct and indirect contact with respiratory droplets, and its airborne transmission has also been identified. However, evidence for environmental factors is scarce, and evidence-based measures to minimize the risk of infection in clinical settings are insufficient. Using computational fluid dynamics, we simulated exhalation of large and small aerosol particles by patie

Impact of CPAP interface and mandibular advancement device on upper airway mechanical properties assessed with phrenic nerve stimulation in sleep apnea patients.

Oronasal mask (ONM) can be used when mouth leaks impair nasal-CPAP effectiveness. However, ONM's constraint on the chin and straps' traction may alter upper airway (UA) mechanical properties. In contrast, mandibular advancement device associated with nasal-CPAP (NM+MAD) may reduce UA resistance. The aim of this exploratory study was to compare the effects of ONM, NM, and NM+MAD on UA mechanical properties. The three interface modalities were assessed in 11 OSAS patients at 6, 8, 10 cmH(2)O CPAP using a phrenic nerve magnetic stimulation (PNMS) protocol. PNMS-twitches' related flow, pharyngeal

Hydrogen sulfide and the probabilities of 'inhalation' through a tympanic membrane defect.

We conclude that workers with tympanic membrane defects (perforated eardrums) should not be excluded from working in atmospheres containing concentrations of hydrogen sulfide (H2S). Several existing requirements and recommendations exclude workers with perforated eardrums from working in or around H2S. Such protective measures stem from the belief that H2S can enter the body through the perforation in sufficient measure to compromise the wearer's respiratory protection. However, based on calculations of anticipated leakage of H2S for a variety of eustachian tube conditions and in the absence o

Intravenous arachidonate-induced respiratory distress: comparative study of three in vivo models for the measurement of antithrombotic activity.

Three different in vivo methods for the study of antithrombotic drugs have been compared. A syndrome of acute respiratory distress has been induced by intravenous injection of arachidonate in three different animal species: mouse, rabbit, and hamster. The intensity of the induced stress was quantified by different procedures: visual observation in the mouse, death or survival in the rabbit, and mechanical recording of the respiration in the hamster. The protective effect of a new antithrombotic agent, triflusal, a salicylic acid derivative, was studied. At the doses used, triflusal has no effe

Examination of factors that impact mask or respirator purchase and usage during the COVID-19 pandemic.

Understanding the motivations and barriers populations face in respiratory protection during a pandemic is key to effective primary prevention. The goal of this study was to identify the impact of various motivational factors on individual choice of masks or respirators during the COVID-19 pandemic. The survey study included a sample of 619 participants between the ages of 18 to 75 years old and English speaking, who were asked about factors that affected their choice of masks or respirators between the months of April 2020 and May 2021. Participants showed a positive association between choic

The five-minute prebreathe in evaluating carbon dioxide absorption in a closed-circuit rebreather: a randomized single-blind study.

INTRODUCTION: Closed-circuit underwater rebreather apparatus (CCR) recycles expired gas through a carbon dioxide (CO₂) 'scrubber'. Prior to diving, users perform a five-minute 'prebreathe' during which they self-check for symptoms of hypercapnia that might indicate a failure in the scrubber. There is doubt that this strategy is valid. METHODS: Thirty divers were block-randomized to breathe for five minutes on a circuit in two of the following three conditions: normal scrubber, partly-failed scrubber, and absent scrubber. Subjects were blind to trial allocation and instructed to terminate the

Effectiveness of three decontamination treatments against influenza virus applied to filtering facepiece respirators.

Filtering facepiece respirators (FFRs) are recommended for use as precautions against airborne pathogenic microorganisms; however, during pandemics demand for FFRs may far exceed availability. Reuse of FFRs following decontamination has been proposed but few reported studies have addressed the feasibility. Concerns regarding biocidal efficacy, respirator performance post decontamination, decontamination cost, and user safety have impeded adoption of reuse measures. This study examined the effectiveness of three energetic decontamination methods [ultraviolet germicidal irradiation (UVGI), micro

[Expert consensus on clinical application of inhaled nitric oxide therapy (2024 edition)].

Inhaled nitric oxide (iNO) is a selective pulmonary vasodilator that has been used in newborns, children, and adults with various cardiopulmonary diseases to reduce pulmonary artery pressure and improve oxygenation. Currently, there are numerous brands of iNO devices on the market with significant differences in performance, as well as variations in connection and setup. Additionally, adverse reactions such as methemoglobinemia may occur during iNO treatment. Therefore, the safety and efficacy of this treatment need to be ensured in clinical practice. To address these concerns and standardize

Development and evaluation of a novel self-contained breathing apparatus pack to reduce musculoskeletal loads and enhance firefighter comfort.

Self‑Contained Breathing Apparatus (SCBA) is an essential piece of personal protective equipment for firefighters, providing essential respiratory support and protection. A major challenge in SCBA research is to reduce musculoskeletal disorders and enhance firefighter comfort while carrying the SCBA. In this study, a novel SCBA pack was designed to reduce SCBA‑related musculoskeletal stress. Ten volunteer firefighters were recruited to participate in the test protocol equipped with a novel SCBA (SCBANovel) and a traditional SCBA (SCBATrad) pack. Surface electromyography and pressure data were

A Novel Inhalation Mask System to Deliver High Concentrations of Nitric Oxide Gas in Spontaneously Breathing Subjects.

Nitric Oxide (NO) is administered as gas for inhalation to induce selective pulmonary vasodilation. It is a safe therapy, with few potential risks even if administered at high concentration. Inhaled NO gas is routinely used to increase systemic oxygenation in different disease conditions. The administration of high concentrations of NO also exerts a virucidal effect in vitro. Owing to its favorable pharmacodynamic and safety profiles, the familiarity in its use by critical care providers, and the potential for a direct virucidal effect, NO is clinically used in patients with coronavirus diseas

Critical care patients' experience of the helmet continuous positive airway pressure.

BACKGROUND: Continuous positive airway pressure (CPAP) is a common treatment modality for acute respiratory failure (ARF) in critical care. Historically, a tight-fitting mask is used to provide respiratory support. This however is not without risks to the patient. The helmet CPAP is a new product that provides the same treatment with a different method of delivery. There is minimal evidence to date explaining the patient's experience of the new helmet modality. AIMS AND OBJECTIVES: The aim of this research study is to explore critical care patient's experience of helmet CPAP. DESIGN: A quali

Effect of temperature on permeation through air supply hoses.

Permeation of chemicals through the walls of air-supply hoses used with respirators is an underrecognized problem in industry. Transport of chemicals through the wall of a hose occurs in the same manner as through gloves and chemical suits, driven by the chemical concentration gradient, but for air-supply hoses the chemical evaporating inside the air-supply hose is inhaled. A simple method based on the mathematical equivalence of filling a homogeneous hose with a chemical, to immersing it in a chemical, has been developed. The method requires a short section of hose to be filled, plugged, and

A case of amyotrophic lateral sclerosis in electronic parts manufacturing worker exposed to lead.

Amyotrophic lateral sclerosis (ALS) is a neurodegenerative disease affecting the motor neurons of the spinal cord and brain. Although the definite etiology of ALS remains unclear, occupational or environmental exposures have been considered as one of the potential causes of ALS. Here, we report the case of a patient with ALS who used to work in a factory manufacturing electronic parts and has evidence of acute exposure to lead in excess. A 39-year-old man visited a neurology clinic with symptoms of progressive limb weakness and spasticity, which began 2 years ago. Upon neurological examination
