CT of a patient with conductive hearing loss showed a small vascular structure leaving the carotid canal and passing through the stapes to join the facial nerve canal. The small bony channel paralleled the anterior tympanic segment of the facial nerve, continuing beyond the geniculate ganglion into the middle cranial fossa. Otoendoscopic photography documented the presence of a persistent stapedial artery.

It is known that the middle ear acoustic reflex (AR) alters the transmission characteristics of the auditory system; however, disagreement exists on how these changes may influence the ears; transfer of speech stimuli. The objective of the present work was to study the specific endoscopic anatomical features of the middle ear using the dissected temporal bones with the intact tympanic membrane. The hearing and middle ear function in 68 patients with rheumatoid arthritis were tested. Reduced middle ear compliance, presumably due to stiffening of the tympanossicular chain, was noted in 48 patients (70.50%). We assessed the hearing of 25 patients with X-linked hypophosphataemic osteomalacia. Twelve patients had subjective hearing loss and 2 had had episodic tinnitus, deafness, and vertigo similar to that in Ménière's disease. Hearing function was tested in 20 patients affected by rheumatoid arthritis. Audiological examination was performed by pure tone audiometry thresholds, tympanometry, a stapedial reflex threshold test and auditory brainstem responses (ABR). A series of experiments on guinea pigs was conducted to determine the prognostic dependability of the stapedial reflex measurement in Bell's palsy. INTRODUCTION AND OBJECTIVE: Stapes replacement surgery performed in cases of otosclerosis alters various anatomical (ossicular, ligament, and tendon) elements of the middle ear affecting their physical properties. Both MEM and MOC systems are sound-evoked reflexes to the auditory periphery which can be elicited by sound in either ear.
