A previously healthy girl died suddenly and unexpectedly at three months of age in her sleep and an autopsy failed to reveal an adequate cause of death. As the father was known to have myophosphorylase (PPL) deficiency (McArdle's disease), we performed molecular genetic analysis of the PPL gene in autopsy muscle of the proposita. The girl was homozygous for the nonsense mutation at codon 49 most commonly associated with typical McArdle's disease. This report suggests that among children presenting as Sudden Infant Death Syndrome (SIDS) there may be cases associated with myophosphorylase defici

A review of recent literature helps to clarify normal variations in the physical examination of children who are thought to have been sexually abused. In many instances, no abnormal physical findings are discovered. Clinicians must continue to pay careful attention to the history and work with other professionals to implement appropriate management, despite the lack of physical findings. Guidelines for evaluating sudden and unexpected infant deaths are reviewed. The current recommendations of the American Academy of Pediatrics for infant sleep positions are discussed in light of epidemiologic

Although Sudden Infant Death Syndrome (SIDS) is the major cause of death in infants aged between 1 week and 1 year in Western countries, it is one of the most enigmatic conditions encountered in paediatric forensic practice. SIDS has been recognized since Biblical times, and yet the definition continues to be debated and the aetiology remains obscure. In addition, there are no accepted pathognomonic features at post-mortem and the diagnosis is still one of exclusion. Emery once asked whether the term 'SIDS' is in reality more of a 'diagnostic dustbin' into which are placed a variety of unrelat

The effects of bilateral vagotomy and of right, left, and bilateral stellectomy on sinus node and on ventricular fibrillation threshold (VFT) were assessed in three groups of puppies (1, 3, and 5 wk old) and one group of adult dogs. Heart rate was increased by vagotomy and reduced by right stellectomy in all groups. After vagotomy, VFT did not change in the first week, while it decreased in the third week (-21%, P < 0.0001), in the fifth week (-20%, P < 0.001) and in the adults dogs (-18%, P < 0.005). VFT was not modified by right stellectomy in the first week and in the fifth week (0%, NS), w

Microassay techniques and monospecific antibodies were used to study the hepatic glucose-6-phosphatase system in liver samples from 55 infants who had died suddenly and unexpectedly, including 38 victims of sudden infant death syndrome (SIDS). Raised hepatic glycogen was found in 10, all of whom had a diagnosis of SIDS, and in 1 other infant who was already known to have type 1b glycogen storage disease (deficiency of transport protein T1). Of the 10 infants with raised hepatic glycogen who had a diagnosis of SIDS, 8 had glucose-6-phosphatase deficiency (type 1a glycogen storage disease), and

The question was posed, whether a relationship could be established between prenatal and perinatal events and subsequent sudden infant death. The birth records of eighty mothers, whose infants died of sudden infant death syndrome (SIDS) between 1982 and 1987, were reviewed for obstetric complications. The records immediately following the respective SIDS cases served as controls. A family history of SIDS was found significantly more often in the SIDS group than in the control group (p less than 0.001). Oxytocin was administered significantly more often in the SIDS group than in the control gro

A previous report has suggested an increased incidence of chorioamnionitis in the placentae of sudden infant death syndrome (SIDS) victims. To further evaluate placental pathology in SIDS, 27 cases were identified from coroner's records and matched to two control groups. Both control groups were matched for birthweight, gestational age, and season, with the second also matched for maternal race, infant sex, blood type B, and maternal parity. No significant increase in chorioamnionitis or other placental abnormalities were found in SIDS victims when controlling for prematurity, which is a facto

Victims of sudden infant death syndrome (SIDS) have occasionally been reported to have had prolonged apnea or an increased frequency of short apnea prior to their deaths. To examine the extent of these abnormalities, we compared pneumograms obtained in 17 infants who subsequently died of SIDS (10 with history of apnea) with those of 34 age- and sex-matched controls. The recordings were analyzed by a computer program that avoided observer bias. SIDS infants had significantly greater mean heart rate (P less than 0.05) and periodic breathing during quiet time (P less than 0.003) than control infa

Carotid bodies from infants dying of sudden infant death syndrome contained significantly higher concentrations of dopamine (10-fold) and noradrenaline (3-fold) than those from age-matched control infants. Administration of dopamine inhibits respiration by direct action on the carotid body, and it is suggested that the elevated levels of endogenous catecholamines found in victims of sudden infant death syndrome may compromise the normal function of the carotid body, particularly the ventilatory response to hypoxia.

The microscopic distribution of thoracic visceral petechiae in 100 consecutive victims of sudden infant death syndrome (SIDS) was retrospectively analyzed. Thymic and cardiac petechiae were more numerous near their surfaces than in the central parenchyma, suggesting that abnormal intrathoracic pressure changes were important in their pathogenesis. Low intrathoracic pressures may have preferentially distended, to the point of rupture, the outer rather than central microvasculature of these two organs. The distribution of pulmonary petechiae in association with lung congestion and edema indicate

The carotid bodies of four infants who died of sudden infant death syndrome (SIDS) were compared, using electron microscopic techniques, with the carotid bodies of various control subjects. In the SIDS patients, there was a marked reduction or absence of the dense cytoplasmic granules of the carotid chemorecptor cells, as well as a reduction in cell number and size. These ultrastructural abnormalities may be pathophysiologically related to SIDS. A defect in this respiratory control organ could block normal stimulation of respiration during the periods of hypoxia that occur during episodes of s

Pregnancy intervals were calculated for 54,369 later-born singletons delivered during 1969 in the State of North Carolina. Subsequent infant deaths among this cohort were categorized into probable sudden infant death syndrome (SIDSp), other postneonatal home deaths, all postneonatal hospital deaths, all neonatal deaths, and all deaths due to congenital malformations. Each death was matched with a control drawn from the population of infants surviving the first year of life. The matching variables were maternal age, race, mother's education and number of previous live births. All categories of

OBJECTIVE: Sigh, defined as an isolated breath with an increased tidal volume, can be associated with abrupt changes in heart rate (HR) or blood oxygenation. Sigh may be followed by a central apnea. As impairment of autonomic control was postulated in future SIDS victims, we hypothesized that their autonomic responses to sighs were different from those of healthy control infants.
METHODS: Sighs followed by central apnea were studied in the sleep recordings of 18 infants who eventually died of SIDS and of 18 control infants. The infants of the two groups were matched for sex, gestational age,

This study examined fetal heart rate (FHR) variables during maternal sleep in subsequent siblings of infants who died of sudden infant death syndrome (SIDS) and who are thus at increased statistical risk for SIDS, and controls. Four findings differentiated the tracings of subsequent siblings of SIDS infants and controls: 1) FHR variability in subsequent siblings of SIDS infants ranged between 11 and 25 beats/min, compared with 6 to 10 beats/min in controls (P less than .001). 2) Bradycardia was more frequent in subsequent siblings of SIDS infants. 3) FHR accelerations (more than 150 beats/min)

Within sudden infant death syndrome (SIDS) reside several primary phenomena. These include a state of immune immaturity, susceptibility to infection, and an inflammatory state. Most SIDS risk factors increase the risk of infection (prematurity, lack of breastfeeding, low or absent transplacental antibody, ethnicity, genetics, specific gene polymorphisms associated with susceptibility to infection, poverty, etc.). Most SIDS cases display evidence of an inflammatory state (raised inflammatory markers and inflammatory cytokines). The pattern of inflammation is very similar to that observed follow

BACKGROUND AND OBJECTIVES: Maternal substance use during pregnancy heightens risk of sudden unexpected infant death (SUID), including through unsafe sleep practices. Families impacted by substance use frequently experience disproportionate social drivers of poor health and family vulnerability likely contributory to fatality risk. Characteristics of sleep-related SUID among infants born prenatally substance exposed versus nonexposed were compared to identify targeted prevention opportunities.
METHODS: Using the Sudden Death in the Young Registry, we examined SUID with sleep-related death betw

We aimed to investigate the potential added value of postmortem MRI (PMMRI) in sudden unexpected infant death (SUID) cases referred to our center between September 2020 and June 2023. Ultimately, 19 SUID cases underwent PMMRI alongside standard autopsy procedures, which included technical examinations such as postmortem CT (PMCT). Four radiologists, two with prior PMMRI experience, provided structured reports following consensus. For each case, the responsible forensic medicine specialist documented the cause of death before and after reviewing the PMMRI report. Additionally, they assessed the
