Injuries of the fingers and thumb in the athlete are common. Mallet finger, jersey finger, boutonniere deformity, Bennett and Rolando fractures, and gamekeeper's thumb are just a few of the injuries that can occur in athletes. A thorough understanding of the mechanism of injury, osseous and soft tissue abnormalities, imaging features, and treatment is important in the care of athletes. Prompt and accurate diagnosis is important and may help minimize outcomes of malunion, posttraumatic arthritis, and debility.

The thumb frequently is involved in rheumatoid arthritis and often is a source of significant functional loss, pain, and deformity. Surgical intervention in patients with rheumatoid arthritis of the thumb should be based on the degree of radiological destruction according to Larsen, the natural course of the rheumatoid hand, the nature and stage of deformity as well as the status of tendons, ligaments, and adjacent joints. The goals of surgery are to relieve pain, increase motion, and restore thumb function. The timing for shoulder surgery should be early in the course of the disease, since it

We report three patients who presented 3 to 8 months after sustaining a closed injury to the dorsoradial aspect of the metacarpophalangeal joint of the thumb. All three patients had an extensor lag of the metacarpophalangeal joint and paradoxical hyperextension of the interphalangeal joint. There were no collateral ligament injuries. The patients required surgical treatment which included advancement and reattachment of the extensor pollicis brevis insertion and imbrication of the dorsoradial capsule to restore the anatomical alignment of the extensor pollicis longus. Surgical treatment of dor

Thumb pain secondary to arthritis at the basal joint of the thumb is a common condition, especially in women, and can be quite disabling. An accurate diagnosis can be readily made from the history and examination. Radiographs are used to stage the severity of the arthritis. Splinting is the mainstay of conservative care. Reconstructive procedures for each stage of the disease are aimed at restoring thumb motion and strength. Partial or complete trapeziectomy with tendon interposition and ligament reconstruction to stabilize the metacarpal base is used for advanced disease. Secondary metacarpop

Black fungi are a group of fungi that are characterized by the development of a pale brown to black color in the cell walls of their vegetative cells, conidia, or both. A mycotic infection caused by a member of black fungi can be subdivided into three clinical entities: phaeohyphomycosis, chromoblastomycosis, and mycetoma. Phaeohyphomycosis is distinguished from mycetoma by the absence of grain (organized, interwoven mycelial aggregates) formation, and from chromoblastomycosis by the absence of sclerotic bodies (thick-walled muriform cells). Phaeohyphomycosis is a rare disease and has been spo

We present an unusual case of polydactyly of the thumb. The patient, despite having a fully developed nail bed, had a duplication at the metacarpophalangeal level consisting of a single bony phalanx. The Wassel classification of polydactyly, which is the most commonly cited classification scheme, does not include this particular anomaly. In addition, there has been no reference to this type of polydactyly in the literature.

Bone metastasis in the hand is rare. The etiology is quite different from that of metastasis to other bones; bronchogenic carcinoma is by far the most frequent case. Distal phalanges are mainly involved with irregular osteolysis and cortical destruction. Differential diagnosis of phalangeal metastasis includes osteomyelitis, rheumatoid arthritis and gout. The prognosis is always that of metastatic bronchial cancer with an average survival of three months. Treatment may involve distal digital amputation or antalgic radiotherapy. A case of bronchogenic carcinoma with metastasis to the thumb is p

Fracture of thumb sesamoids is a rare injury. We are reporting four cases. In two cases, the fracture involved the ulnar sesamoid and in two others, the radial sesamoid. The right thumb was affected in two patients and the left thumb in the other two. In one patient, arthrography of the metacarpophalangeal joint of the thumb demonstrated contrast leakage from the volar aspect of the joint and confirmed volar plate rupture. All patients were treated conservatively with immobilization. Normal hand function was restored in all four patients. Sesamoid fractures may accompany severe injuries of the

Hand function in patients with congenitally deficient thumbs can usually be improved by operative procedures. The specific operation depends on the type of hypoplasia present. It is important for the surgeon to determine whether the thumb can be retained and made more functional by various reconstructive procedures, or whether it should be ablated in favor of a single-stage index finger pollicization. This decision is particularly difficult in patients with type III hypoplastic thumbs, in whom both the osseous and musculotendinous structures may be significantly affected. It is nearly impossib

The two important fractures of the trapeziometacarpal joint are Bennett's fracture and Rolando's fracture. Bennett's fracture is a fracture-dislocation of the joint. The preferred treatment is closed reduction and percutaneous pinning of the fracture with additional fixation through the trapeziometacarpal joint verified by anteroposterior (AP) and lateral roentgenograms. If the fracture is not reducible, and open reduction through the thenar approach with Herbert screw fixation is recommended. Rolando's fracture is a comminuted T- or Y-shaped fracture through the base of the first metacarpal.

We clinically examined the hands of twenty-three patients and experimented with amputation specimens to clarify the characteristic features and pathomechanism of locking of the metacarpophalangeal joint of the thumb. In all of the patients the locking had resulted from forced hyperextension of the thumb. Every injured thumb showed a mild hyperextension deformity of the metacarpophalangeal joint, with the joint having become fixed. Manual reduction was successful in seven patients, but the other sixteen required open reduction, during which we found that the proximal palmar ligament was rupture

With the use of a stereoscopic microscope, observations were made on the course of the proper digital nerve of the thumb on ten upper limbs in adult cadavers. By observation from the dorsal side after removal of the bone, it was possible to clearly see the ramification of the digital nerve from the subcutaneous tissue to the dermis. Ramification from the palm to the level of the interphalangeal joint was segmental. The digital nerve of the thumb trifurcates from the proximal site of the distal palmar thumb crease into the main, medial, and lateral branches. The main branch is thick and extends

Prompt recognition of the magnitude of articular damage is essential for optimal management of digital joint injuries. Basic to this concept is the need to differentiate injuries that can be successfully treated by closed techniques from those that require surgery. Following an accurate restoration of the damaged joint, recovery of function depends on careful immobilization in a protective position and, whenever possible, early joint mobilization. Precise primary care is the key to the prevention of disabling joint contractures.

Dermatoglyphics of six patients with absence of thumb are described. Two specific abnormal configurations were seen in the palmar area. In the first, there was no axial triradius and the course of the ridges in the proximal part of the palm was transversal. In the second, a peculiar distal loop on the radial border was present with a palmar triradius. These abnormal patterns are probably due to the absence of a thenar volar pad or the presence of an abnormal one when the ridges are formed.
