Pseudoangiosarcomatous carcinoma of the genitourinary tract.

Two cases of pseudoangiosarcomatous carcinoma of the genitourinary tract, arising in the vulva in one and the bladder in the other, are presented. In case 1, an 84 year old woman, the vulvectomy specimen contained an irregular ulcerated tumour, infiltrating the left labia and extending into the clitoris. In case 2, a 59 year old woman, the excised bladder showed diffuse thickening of its wall by infiltrating haemorrhagic tumour. Both tumours showed focal keratinisation. This, in association with the presence of atypical squamous epithelium, immunohistochemistry and ultrastructural analysis, le

Multifocal malignant melanoma arising in vesicovaginal melanosis.

An 80-year-old white woman developed vulvar melanosis and malignant melanoma of the labia majora and clitoris and underwent a simple vulvectomy. She subsequently developed melanosis of the urinary bladder and presented 3 years later with multifocal malignant melanoma involving the vagina, urethra, and urinary bladder in a background of extensive melanosis with variable degrees of atypia. She underwent radical surgery but died 18 months later with liver metastases and liver failure. Malignant melanoma is uncommon in the vulva and vagina and is rare in the urinary bladder. This case illustrates

Lung Metastasis in a Case of Recurrent Poorly Differentiated Leiomyosarcoma of the Bartholin Gland: A Case Report and Review of the Literature.

Vulvar neoplasms represent four percent of all gynecological cancers. While most cases of vulvar neoplasms are benign, two percent of patients present with malignant disease. We present the case of a 37-year-old premenopausal female who presented to an outside institution with a lump in her left vulva, which had progressively enlarged to the size of an egg. A wide local excision of the left vulva was performed, and the pathology revealed a high-grade sarcoma, not otherwise specified (NOS), with negative margins. Imaging showed enlarged bilateral external iliac lymph nodes, likely metastatic. A

Period prevalence and risk factors for postoperative short-term wound complications in vulvar cancer: a cross-sectional study.

INTRODUCTION: Although clinicians recognize that postoperative wound management in patients with vulvar cancer (VC) is challenging, the prevalence and risk factors for different types of short-term wound complications (WCs) remain unclear. The aims of this study were: (1) to determine the period prevalence of postoperative short-term WCs and (2) to identify risk factors associated with short-term WCs in patients with VC. METHOD: In a cross-sectional study in a Swiss University Hospital, a sample of 108 patients with VC treated surgically (and free of WCs at the time of admission) was included

Vulvar Carcinoma in Pregnancy: A Case Report on Diagnosis, Management, and Surgical Outcomes.

Vulvar cancer is an uncommon malignancy in reproductive-aged women, and its occurrence during pregnancy is rare. This report presents a case of vulvar squamous cell carcinoma (VSCC) diagnosed perioperatively in a 35-year-old pregnant woman. The patient was incidentally found to have lichen sclerosis and a suspicious vulvar lesion during an emergency cesarean section. Biopsy confirmed VSCC, and subsequent management involved a simple vulvectomy, sentinel lymph node biopsy, and local advancement flap reconstruction. Histopathological examination revealed clear margins and no lymph node involveme

Plexiform schwannoma: an unusual clitoral mass.

Acquired clitoral enlargement is a rare condition resulting from a variety of etiologies, including tumors and excess androgens. Few cases of nonmalignant schwannoma, a benign tumor of the peripheral nerve sheath, have been reported in the literature as causes of clitoral enlargement in patients without known neurofibromatosis. These painless, slow-growing tumors rarely recur once excised. We present the initial investigation of a patient with a large clitoral schwannoma and subsequent treatment with partial vulvectomy. The workup, including advanced pelvic imaging for diagnosis and surgical p

Conservative management of extramammary paget disease with imiquimod.

OBJECTIVE: Extramammary Paget disease is a rare cutaneous neoplasm that most frequently affects the vulva. Surgery remains the preferred treatment, despite its association with high recurrence rates. Few reports have described conservative treatments for vulvar Paget disease. Our aim was to evaluate the efficacy of conservative treatment with imiquimod. MATERIALS AND METHODS: We performed a retrospective analysis of 4 patients who were treated with topical imiquimod 5% cream. RESULTS: One patient underwent vulvectomy after imiquimod therapy, and 3 patients experienced extensive recurrent dis

Hypercalcemia associated with early recurrence of vulvar cancer.

INTRODUCTION: Humoral hypercalcemia of malignancy is an important cause of hypercalcemia associated with malignancies and has been rarely reported because of vulvar cancer. CASE: A 65-year-old woman performed modified radical vulvectomy and bilateral lymph node dissection because of vulvar cancer was referred to our clinic with fatigue and somnolence 4 months after her operation. Calcium level was 15.9 mg/dL. There was no bone metastasis, and hypercalcemia was thought as paraneoplastic syndrome because of distant recurrence of the primary vulvar cancer. DISCUSSION: Vulvar cancer and hypercal

[Operative treatment for vulvar cancer by different methods].

A retrospective analysis of 88 patients with vulvar cancer treated by radical vulvectomy and groin lymphadenectomy from July, 1958 to December, 1990 is presented. 88 cases are divided into two groups based on different operative methods: the enbloc group (25 cases) and the non-enbloc group (63 cases). No significant statistic differences are found in the 5-year survival rates (76.0% vs 79.8%) and the complications (erysipelas, lower extremity edema) between the two groups. The wound healing time is 14.4 days shorter in the non-enbloc group than enbloc group. Factors such as clinical staging, m

Vulval dystrophies: new approaches.

In describing vulval skin changes, all terms such as leucoplakia, kraurosis vulvae and lichen sclerosus et atrophicus should be discarded in favour of 'chronic vulval dystrophy'. Initial biopsy is mandatory to establish the precise diagnosis. From a review of the literature and from our own studies we believe that vulval dystrophy is not usually a premalignant condition, a risk of carcinoma exiting only in dystrophic vulvas with associated atypia. For such patients the treatment is local vulvectomy; otherwise, because of the high rate of recurrence of chronic vulval dystrophy following surgery

Ustekinumab for treatment of cutaneous Crohn's disease.

Cutaneous Crohn disease (CD) affecting the vulva, perineum, and perianal skin, is a rare entity, which may accompany or precede gastrointestinal CD. Vulvar involvement, if untreated, may ultimately require extensive surgery including vulvectomy to gain control of the disease. Both gastrointestinal and cutaneous CD respond to biologics, which block TNF. In addition, ustekinumab, which targets both IL-12 and IL-23 cytokines, is effective in patients with gastrointestinal CD who fail TNF blockade. However, it is unclear if ustekinumab is effective for cutaneous CD. Herein we present a patient wit

Adenoid Cystic Carcinoma of the Vulva: A Case Report.

Adenoid cystic carcinoma (ACC) of the vulva represents a highly uncommon type of female malignancy. Due to the absence of specific treatment guidelines, such cases are typically managed by the treatment protocols for vulvar cancer. Here, we report the case of a 52-year-old woman who presented with a painful right vulvar mass, leading to a diagnosis of ACC of the vulva after biopsy and immunohistochemical analysis. She underwent vulvectomy, bilateral inguinal lymphadenectomy, and targeted radiotherapy, and no evidence of recurrence has been found for three years, with ongoing monitoring for pos

Adenoid cystic carcinoma of skene glands: a rare origin in the female genital tract and the characteristic clinical course.

Adenoid cystic carcinoma (ACC) is a relatively uncommon malignancy that most frequently arises in the salivary glands. In the genital tract, approximately 60 cases of ACC that originated from Bartholin glands have been reported to date. In this report, we describe a case of ACC that arose from Skene glands, a very rare origin for this disease. In this patient, the disease had an indolent clinical course, with few symptoms other than localized pain. During the surgical operation, the tumor was found to have invaded more extensively than had been estimated preoperatively, and it required pelvic

Malignant melanoma of the vulva: a review.

Vulvar melanoma is an unusual tumor with a poor prognosis. Most surgeons have abandoned radical vulvectomy as the treatment of choice. The role of elective node dissection is controversial. Currently effective adjuvant therapy is lacking. In this paper 51 sources concerning melanoma and/or melanoma of the vulva were reviewed, 43 of these sources were considered to be pertinent and current enough to include in this review. The objective was to describe current knowledge about the natural history, staging, pathology, and treatment of melanoma of the vulva. It has been found that melanoma continu

Old and new perspectives in the management of high-risk, locally advanced or recurrent, and metastatic vulvar cancer.

During the last decades there has been a continuing evolution in the surgical approach of squamous cell carcinoma of the vulva that has been traditionally treated with radical vulvectomy and bilateral inguinal-femoral lymphadenectomy. Patients with T1 tumour are usually treated with radical local excision, if the lesion is unifocal and the remainder of the vulva is normal. Patients with T1a disease have no risk of groin metastases and do not need lymphadenectomy, whereas those with T1b disease need ipsilateral inguinal-femoral lymphadenectomy if the lesion is lateral, and bilateral lymphadenec

A Case of Chronic and Relapsing Paget Disease of the Vulva.

Extramammary Paget disease is a rare neoplastic condition that more commonly affects postmenopausal Caucasian women. Although the vulvar area is the most frequently affected location, it corresponds solely to 1 to 2% of all vulvar malignancies. A 72-year-old female patient was observed in our outpatient clinic with a 2-year history of an erythematous and pruritic plaque on the vulva. Histopathology and immunohistochemistry studies were compatible with extramammary Paget disease of the vulva. Associated neoplastic conditions were excluded. Due to multiple relapses, the patient was submitted to

Organ-sparing central pelvic compartment resection for the treatment of vulvo-vaginal melanomas.

Vulvo-vaginal melanomas are one of the rarest gynecological oncology diseases with a poor survival compared with other malignancies. The 5-year survival varies from 13% to 32.3%. Vulvo-vaginal melanomas involving the upper 2/3rds of the vagina are usually treated with total pelvic exenteration (TPE). TPE surgery carries a 50% risk of major complications and also morbidity associated with double stomas. Central pelvic compartment resection is a novel organ-sparing surgical approach entailing radical total laparoscopic hysterectomy, bilateral salpingo-oophrectomy, laparoscopic vaginectomy and vu

Human papillomavirus (HPV) and cervical dysplasia in adult female cystic fibrosis (CF) lung transplant recipients.

Human papillomavirus (HPV) is the principal risk factor for cervical cancer. Transplant recipients are at a disproportionate risk of HPV complications. We conducted a single-centre, retrospective study of adult female cystic fibrosis (CF) lung transplant recipients between 2008 and 2021. We observed 12 of 34 (35.3%) with ≥1 abnormal pap smear (median age: 26.7 years). Complications included refractory anogenital warts (n=3), vulvectomy (n=2) and cervical cancer (n=4), with two deaths from metastatic disease. None with HPV morbidity was vaccinated. Lung transplant recipients had greater odds of
