Sectional impression tray and sectional denture for a microstomia patient.

Microstomia presents a unique challenge to the patient. Patients with microstomia who must wear removable dental prostheses often face the difficulty of being unable to insert or remove the prosthesis because of the constricted opening of the oral cavity. A completely edentulous patient, who developed microstomia along with Raynaud's phenomenon induced by scleroderma, is presented. This clinical report describes a quick and easy method for fabrication of a sectional custom impression tray connected by press button and a sectional complete denture retained by magnets. A sectional denture that p

A screwless and cementless technique for the restoration of single-tooth implants: a retrospective cohort study.

PURPOSE: The Integrated Abutment Crown (IAC) is a technique for the fabrication of single-tooth implant-supported crowns where the abutment and the crown are one unit. The abutment-crown complex is connected to the implant with a locking taper. This technique does not use cement to retain the crown or screws to retain the abutment. The purpose of this study was to evaluate the clinical outcome of screwless, cementless single implant-supported crowns (IACs) placed in a general dental practice. MATERIALS AND METHODS: A retrospective cohort study was conducted between July 2001 and August 2003.

Prosthetic rehabilitation of orofacial donor site fistula following surgical reconstruction: a clinical report.

Orofacial defects can be either congenital or acquired. Rehabilitation of these patients can be done using a surgical and/or a prosthetic approach. In situations where surgical reconstruction is not possible, prosthetic management becomes the only option. This clinical report describes a simple, economical, and effective technique for the prosthetic rehabilitation of a patient with oro-cutaneous fistula due to donor site dehiscence following tumor defect reconstruction.

Surgical stent fabrication for unilateral nasal obstruction of the anterior portion of the nasal airway.

A description is given of the indication and technical steps for fabricating a unilateral nasal stent to maintain patency of the nasal passage after surgical opening of an obstruction in a pediatric case. The methodology uses a two-step impression of the contralateral unobstructed naris and exterior valve region to generate a two-piece injection mold. The mold is used to fabricate a soft silicone-based anatomical stent. It supports an intranasal skin graft that was placed to reduce the risk of granulation.

Biofilms in the edentulous oral cavity.

PURPOSE: The oral cavity presents numerous surfaces for microbial colonization. These surfaces produce biofilms of differing complexities unique to each individual. Several studies have looked at biofilms in dentate patients. There has been limited research regarding biofilms on dentures or soft tissues of edentulous patients. The purpose of the present investigation was to provide meaningful data describing microbial ecological relationships in the oral cavity of edentulous patients and to evaluate the microbiota on hard and soft tissue surfaces and saliva in edentulous patients wearing compl

Placement and restoration of dental implants in a patient with Paget's disease in remission: literature review and clinical report.

Paget's disease is a disorder of bone that results in the replacement of normal bone with highly vascularized, low density bone. The diminished bone quality associated with Paget's disease is a relative contraindication to the use of dental implants, as it interferes with achievement of osseointegration. There is no current literature on the success of dental implants in patients with Paget's disease. Furthermore, there is no current literature on dental implant success in patients with remissive Paget's disease or in bone that appears to be unaffected by the disease in patients with Paget's.

Acellular dermal allograft for vestibuloplasty--an alternative to autogenous soft tissue grafts in preprosthetic surgical procedures: a clinical report.

Various vestibular extension procedures have been described in the literature over the past 6 decades, including the use of free gingival grafts. An acellular dermal allograft has been used as a substitute for autogenous soft tissue grafts in root coverage procedures. This clinical report describes the use of such an allograft as a substitute for palatal donor tissue in the vestibular extension of an edentulous maxillary arch with multiple frenum attachments before fabrication of a complete denture. In this patient, healing was uneventful, and 6-month clinical observations demonstrated an apic

Rehabilitation of a bulimic patient using endosteal implants.

This article describes the dental rehabilitation of a bulimic patient using endosteal implants. Although the patient, a 31-year-old woman with a long history of bulimia nervosa, had been receiving medical and psychological treatment, the condition was not completely controlled. Clinical examination revealed multiple crowns with extensive cervical caries. The prognosis for all remaining teeth was poor. After extractions, implant therapy was implemented to provide support for fixed prostheses. After the implants were uncovered and during provisional therapy, the peri-implant tissue exhibited inf

Oral rehabilitation of a child with primary failure of tooth eruption.

This article describes the prosthetic evaluation and treatment of a 10-year-old female patient with primary failure of tooth eruption. This case is unique in that the failure of eruption affected both the deciduous and permanent dentition in all quadrants, in the absence of any other systemic abnormalities. After diagnostic articulation and a thorough discussion of the treatment with the patient and her parents, complete maxillary and mandibular overdentures were fabricated. The process followed with this preadolescent patient to ensure maximum compliance, success, and encouragement toward a p

From Osteoclast Differentiation to Osteonecrosis of the Jaw: Molecular and Clinical Insights.

Bone physiology relies on the delicate balance between resorption and formation of its tissue. Bone resorption depends on a process called osteoclastogenesis in which bone-resorbing cells, i.e., osteoclasts, are produced by the differentiation of more undifferentiated progenitors and precursors. This process is governed by two main factors, monocyte colony-stimulating factor (M-CSF) and receptor activator of NFκB ligand (RANKL). While the former exerts a proliferating effect on progenitors/precursors, the latter triggers a differentiation effect on more mature cells of the same lineage. Bone h
