Formal paraphasias: a single case study.

A case study is reported of an aphasic patient, RB, who showed frequent form-related whole-word substitutions in oral naming, writing to dictation, and reading aloud. In both written language tasks, the abstractness of the targets influenced the number of formal errors. In oral naming, a high proportion of formal paraphasias was related to the intended words in both form and meaning. A comparison between targets and formal paraphasias indicated a high agreement both in word class, number of syllables, stress pattern, and in basic (stressed) vowels. The agreement in consonants (including word-i

Ideational apraxia: a deficit in tool selection and use.

We report a 67-year-old left-handed man who exhibited an ideational apraxia in both clinical and nonclinical natural settings following a right hemisphere infarction. His inability to use tools could not be explained by a motor production deficit (ideomotor apraxia), because he made content errors and could not match tools with objects. His deficit could not be attributed to an agnosia or language comprehension deficit, because he could name tools and point to tools on command. Based on our testing, it appeared that this patient had a loss of knowledge related to tool use.

Altered patterns of word associations in dementia and aphasia.

The word associations of 38 demented, 17 aphasic, and 22 normal subjects were studied. Both normal and brain-injured subjects appear to make judgments about the grammatical class of the word stimulus. Certain stimulus words (especially nouns and adjectives) elicit paradigmatic responses whereas other words (especially verbs and adverbs) elicit syntagmatic responses. The mechanism producing syntagmatic responses seems relatively resistant to deterioration in dementia or aphasia. However, in dementia the mechanism that generates paradigmatic responses becomes progressively less efficient (possib

Whole network, temporal and parietal lobe contributions to the earliest phases of language production.

We investigated whether it is possible to study the network dynamics and the anatomical regions involved in the earliest moments of picture naming by using invasive electroencephalogram (EEG) traces to predict naming errors. Four right-handed participants with focal epilepsy explored with extensive stereotactic implant montages that recorded temporal, parietal and occipital regions -in two patients of both hemispheres-named a total of 228 black and white pictures in three different sessions recorded in different days. The subjects made errors that involved anomia and semantic dysphasia, which

Prelexical representations and processes in reading: evidence from acquired dyslexia.

We report a detailed and extensive single-case study of an acquired dyslexic patient, L.H.D., who suffered a left-hemisphere lesion as a result of a ruptured aneurysm. We present evidence that L.H.D.'s reading errors stem from a deficit in visual letter identification, and we use her deficit as a basis for exploring a variety of issues concerning prelexical representations and processes in reading. First, building on the work of other researchers, we present evidence that the prelexical reading system includes an allograph level of representation that represents each distinct visual shape of a

Word finding difficulty as a post-polio sequelae.

OBJECTIVE: Seventy-nine percent of respondents to the 1990 National Post-Polio Survey reported difficulty "thinking of words I want to say," with 37% reporting frequent, moderate-to-severe word finding difficulty. This study was undertaken to objectively document polio survivors' word finding difficulty and to identify its relationship to fatigue, neuropsychologic processes requiring cortical activation, and a peripheral marker for brain dopamine secretion. DESIGN: In this study, 33 polio survivors were administered the Post-Polio Fatigue Questionnaire, Animal Naming and FAS Tests, and tests

The relationship between limb apraxia and the spontaneous use of communicative gesture in aphasia.

This study investigated the relationship between limb apraxia, as assessed by a formal clinical test, and the production of spontaneous communicative gesture, as measured by a newly designed rating scale--the Nonvocal Communication Scale (NCS). Subjects were aphasic adult males with cerebrovascular lesions of the left hemisphere. The performance of aphasic patients on the praxis test and the NCS was independent of demographic, neuroanatomic, linguistic, or cognitive variables, except for global aphasics who were low-scoring across the board. There was a significant positive correlation, howeve

Color "amnesia" without aphasia.

Following an apparent left parietal CVA, a patient developed a severe and nearly complete color amnesia which was not associated with any disturbance in color vision or color perception. Like all previously reported cases with color amnesia, this patient was alexic but, unlike most previously reported cases, he was not aphasic.

Static Visual Agnosia Following Awake Resection of a Left Frontal Low-Grade Glioma: A Case Report of Ventral Stream Network Disruption ("Astatopsia").

Background and Clinical Significance: Visual agnosia and speech production deficits are well-described sequelae of neurosurgical interventions, but their selective dissociation remains rare. This report presents an unusual combination of postoperative deficits following awake resection of a left frontal low-grade glioma. Case Presentation: We present the case of a right-handed female with left hemisphere language dominance who had a left frontal low-grade glioma. Preoperatively, she exhibited anomia and dysexecutive syndrome, including difficulty completing everyday goal-directed tasks such as

Four cases of natalizumab-related PML: a less severe course in extended interval dosing?

BACKGROUND: Progressive multifocal leukoencephalopathy (PML) is a severe adverse event of natalizumab (NTZ). The administration of NTZ with extended interval dosing (EID) has been proposed as a strategy to potentially reduce the incidence of PML while maintaining its therapeutic efficacy. METHODS: In the current paper, we describe 4 cases of NTZ-PML in EID included in the Italian PML cohort. RESULTS: The patients developed PML after at least 38 NTZ infusions. Their John Cunningham virus (JCv) index was > 1.5, and patients had not previously used immunosuppressant. Two patients were asymptoma

Surface dyslexia in a Japanese patient with semantic dementia: evidence for similarity-based orthography-to-phonology translation.

We studied the reading performance of a Japanese-speaking patient, TI, with bilateral but asymmetrical (left more than right) temporal-lobe atrophy, severe anomia, and poor word comprehension. Most Japanese kanji characters correspond to several different legitimate pronunciations in different contexts, with varying degrees of correspondence consistency. TI made many errors in reading aloud words that violate statistically typical character-sound correspondences, especially for less common words, but had relatively preserved ability to read aloud strings in which the assignment of the typical

Atypical Frontotemporal Dementia Associated With SQSTM1 Gene Mutation: A Clinicopathological Case.

A 78-year-old man presented with a six-year history of progressive memory decline, initially manifesting as recent memory impairment and mild anomia, which gradually evolved into motor clumsiness, gait disturbances, language difficulties, behavioral changes, and late-onset parkinsonism. He had been diagnosed with Paget disease of bone (PDB) at the age of 45. Brain MRI revealed asymmetric left anterior temporal atrophy, while [18F]-Fluorodeoxyglucose (FDG) PET demonstrated frontotemporal hypometabolism, predominantly on the left side, with marked involvement of both temporal poles and greater h

Persistent Cognitive Dysfunction in a Non-Hospitalized COVID-19 Long-Hauler Patient Responding to Cognitive Rehabilitation and Citicoline Treatment.

The Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) infection is characterized by severe flu-like symptoms, which can progress to life-threatening systemic inflammation and multiorgan dysfunction. The nervous system is involved in over one-third of patients, and the most common neurological manifestations concern the central nervous system, such as headache, fatigue, and brain fog. The activation of innate, humoral, and cellular immune responses, resulting in a cytokine storm and endothelial and mitochondrial dysfunctions, are the main pathophysiological mechanisms of SARS-CoV-2 i

Primary progressive aphasia.

Primary progressive aphasia (PPA) is a progressive loss of specific language functions with relative sparing of other cognitive domains at least for the first few years of the illness. Based on the constellation of symptoms, PPA has been recently classified into a nonfluent, semantic, or logopenic variant. Nonfluent variant PPA is characterized by dysfluent and effortful speech, often combined with agrammatism. Also, some patients have initially predominant apraxia of speech. The neuroimaging findings in nfvPPA are in most cases progressive atrophy within the left inferior, opercular, and insu

Alexia due to ischemic stroke of the visual word form area.

The visual word form area (VWFA) is a region in the posterior left occipitotemporal cortex adjacent to the fusiform gyrus hypothesized to mediate word recognition. Evidence supporting the role of this area in reading comes from neuroimaging studies of normal subjects, case-controlled lesion studies, and studies of patients with surgical resection of the VWFA for tumors or epilepsy. Based on these prior reports, a small discrete lesion to the VWFA would be expected to cause alexia in a literate person without prior brain process, but such a case has not previously been reported to our knowledge

Verbal and non-verbal semantic impairment: From fluent primary progressive aphasia to semantic dementia.

UNLABELLED: Selective disturbances of semantic memory have attracted the interest of many investigators and the question of the existence of single or multiple semantic systems remains a very controversial theme in the literature. OBJECTIVES: To discuss the question of multiple semantic systems based on a longitudinal study of a patient who presented semantic dementia from fluent primary progressive aphasia. METHODS: A 66 year-old woman with selective impairment of semantic memory was examined on two occasions, undergoing neuropsychological and language evaluations, the results of which were

Associative agnosia and optic aphasia: qualitative or quantitative difference?

We report a patient who, following the partial removal of a tentorial meningioma, suffered a hematoma in the left occipital lobe, which was resected. He showed severe anomia for visually presented stimuli, while naming was normal when they were presented in the tactile, auditory and verbal modality. His performance on visuo-perceptual tests, not requiring meaning identification, provided evidence that he was able to access the stored representations of stimuli. When recognition was assessed with non-verbal tasks, a mixed pattern of findings emerged and the patient showed features of both assoc

Semantic dementia. Progressive fluent aphasia with temporal lobe atrophy.

We report five patients with a stereotyped clinical syndrome characterized by fluent dysphasia with severe anomia, reduced vocabulary and prominent impairment of single-word comprehension, progressing to a stage of virtually complete dissolution of the semantic components of language. A marked reduction in the ability to generate exemplars from restricted semantic categories (e.g. animals, vehicles, etc.) was a consistent and early feature. Tests of semantic memory demonstrated a radically impoverished knowledge about a range of living and man-made items. In contrast, phonology and grammar of

Visual object naming in patients with small lesions centered at the left temporopolar region.

Naming is considered a left hemisphere function that operates according to a posterior-anterior specificity gradient, with more fine-grained information processed in most anterior regions of the temporal lobe (ATL), including the temporal pole (TP). Word finding difficulties are typically assessed using visual confrontation naming tasks, and have been associated with selective damage to ATL resulting from different aetiologies. Nonetheless, the role of the ATL and, more specifically, of the TP in the naming network is not completely established. Most of the accumulated evidence is based on stu
