Endovascular thoracic aortic aneurysm repair with concomitant myocardial and carotid revascularization.

A 73-year-old woman presented with a large saccular aneurysm involving the distal aortic arch. Preoperative aortography and cardiac catheterization revealed left main coronary artery and left common carotid artery stenoses. Concomitant coronary artery bypass grafting to the left anterior descending and first diagonal arteries, ascending aorta-to-left common carotid artery bypass grafting, and endovascular thoracic aortic aneurysm repair with antegrade stent-graft deployment and intentional left subclavian artery coverage were performed.

Iatrogenic aortic dissection during right transradial intervention in a patient with aberrant right subclavian artery: A case report.

BACKGROUND: Aberrant right subclavian artery (ARSA) is the most common congenital anomaly of the aortic arch. When patients having such anomalies receive transradial intervention (TRI), aortic dissection (AD) may occur. Herein, we discuss a case of iatrogenic type B AD occurring during right TRI in an ARSA patient, that was later salvaged by percutaneous angioplasty. CASE SUMMARY: A 73-year-old man presented to our hospital with intermittent chest pain. Coronary computed tomography (CT) angiography revealed significant stenosis in the left anterior descending artery. Diagnostic coronary angio

[Aortobronchial fistula developing in a pseudoaneurysm of the horizontal aorta].

Amongst the rare causes of haemoptysis rupture of the great vessels is always a possibility. An aorto-bronchial fistula is a rare complication of thoracic aneurysms of the aorta. A fistula starting from a false aneurysm is exceptionally rare. Our observation concerns a patient of 61 with previous vascular problems who was discovered to have a mediastinal tumour following dysphonia and haemoptysis. Aortography and surgical intervention revealed that this was a false aneurysm of the horizontal part of the aorta which had developed a fistula in the bronchus.

Use of pre-intervention imaging with a novel image fusion software for guidance of cardiac catheterisation in a patient with pulmonary atresia and major aortopulmonary collaterals.

We present a 3.5-year-old patient with tetralogy of Fallot, pulmonary atresia, and major aortopulmonary collaterals, after repeat implantation of a central shunt, in whom we successfully re-purposed previous imaging scans with a novel image fusion software to guide diagnostic heart catheterisation. The pre-registered CT scan was segmented before the procedure and subsequently manually fused with two-dimensional fluoroscopy images. The overlaid roadmap enhanced selective cannulation of all major vessels originating from the aorta, making aortography unnecessary.

Retrograde aortic dissection after bilateral iliac artery stenting: a case report and review of the literature.

We report a case of retrograde aortic dissection after bilateral iliac artery stenting. A 63-year-old black woman underwent aortography with balloon angioplasty of bilateral common iliac artery lesions and subsequent vascular stent placement. The patient developed an acute aortic dissection from the level of the aortic bifurcation to the left subclavian artery. This case is presented to call attention to the previously unreported complication of retrograde aortic dissection after bilateral iliac artery angioplasty and stent placement presenting as acute chest pain.

A rare anomaly of the aortic arch: aberrant right subclavian artery associated with common carotid trunk.

With increasing use of transradial access (TRA) to perform coronary angiography, arterial anomalies of the upper extremity are more frequently encountered. We describe a patient with an aberrant right subclavian artery (RSCA) originating as the last major branch of the aortic arch, which eventually led to failure of the transradial approach. Subsequent arch aortography revealed an associated common carotid trunk. The combination of both anomalies is a rare finding and may be associated with a number of other arterial and/or mediastinal organ anomalies.

Delayed presentation of posttraumatic aortic false aneurysm with chronic cough.

A chronic posttraumatic false aneurysm of descending aorta in a 19-year-old man who had been involved in a car accident 8 months earlier is described. He presented with chronic cough, and on chest roentgenogram, a mass in upper left mediastinum was noticed. Aortography and computed tomography scan revealed a false aneurysm of the descending aorta. The patient underwent aneurysm resection and graft replacement. Cough resolved after surgery and at 5 years follow-up the patient remained well and symptom-free.

Dissecting aortic aneurysm involving an anomalous right subclavian artery and isolated left vertebral artery: case report and review of the literature.

A 54-year-old hypertensive woman was admitted with severe interscapular back pain. A chest radiograph showed marked widening of the mediastinum. Aortography demonstrated a DeBakey type III, a thoracic aortic dissection and an anomalous right subclavian artery which was associated with an isolated left vertebral artery. The patient underwent aortic arch replacement with 5 branches and made an uneventful recovery. As far as we can determine, this is the first reported occurrence of these anomalies together with acquired disease of the aorta.

Aortopulmonary fistula: an uncommon complication in dystrophic aortic aneurysm.

Wall dissection is a typical complication in the evolution of Marfan aortic aneurysm and usually is associated with valve regurgitation. Formation of a fistula with adjacent structures is very uncommon. We report the case of a 32-year-old man who presented with the typical features of Marfan's syndrome, with chronic aneurysm of the ascending aorta and acute aortopulmonary fistula. Diagnosis was made preoperatively by aortography; operation was performed successfully. A review of the literature only shows a few cases of aortopulmonary fistula in atherosclerotic, syphilitic, or postendocarditis

Circumferential dissection of the ascending aorta with intimal intussusception. Case report and review of the literature.

The present report describes an unusual case (apparently the 10th in the world literature) of a type-A aortic dissection with full circumferential detachment of the ascending aortic intima and intussusception thereof into the aortic arch and descending aorta, partly occluding the arch vessels. Computed tomographic scanning and 2-dimensional echocardiography failed to detect an intimal flap and a false lumen in the ascending aorta. Aortic dissection was visualized by aortography. The ascending aorta was surgically repaired and the aortic valve resuspended. The pertinent literature is reviewed.

[Celiac artery compression syndrome].

External compression and kinking of the celiac axis is caused by the enlarged and fibrous median arcuate ligament of the diaphragm. The pathogenetic cause may be a functional ischemia or an irritation of the squeezed celiac ganglion. This painful syndrome is an unusual condition, which occurs at a maximum age of about 40 years and mostly in women. The diagnosis is established by exclusion of all other abdominal sources of pain and lateral aortography. The therapy can often be limited to longitudinal incision of the ligament; reconstructive procedures of the coeliac artery are sometimes necessa

Surgery of the horseshoe kidney--an experience of 24 patients.

A decade (1973-83) of surgical experience in the management of the diseases of horseshoe kidney is presented. Calculus disease was the major pathological entity (80%) followed by ureteropelvic junction obstruction, nephrocarcinoma and polycystic disease, respectively. Three patients with calculi had, in addition, pyonephrosis and squamous cell carcinoma. Aortography offers maximum information about abnormal vasculature and other anatomical details in proper planning of surgical intervention. It is felt that a symphysiotomy and nephropexy may be considered when the horseshoe kidneys are being o

[Rupture of the aorta complicating multiple injuries (author's transl)].

A case of subtotal circumferential rupture of the aorta is described which involved a woman who had sustained multiple injuries in a traffic accident. The rupture was discovered only during autopsy. As this type of injury is generally not diagnosed until after death, knowledge of some of the characteristics signs of rupture of the aorta is important, especially to the physician in charge of an intensive care unit. If there are suspicious signs aortography is essential. The only treatment likely to succeed is surgery, but postoperative mortality is still as high as 20--40 per cent.

The results of surgical treatment of coarctation of the thoracic aorta.

A retrospective study of the results of surgical treatment in 124 cases of coarctation of the thoracic aorta is presented. The diagnosis was based on clinical findings alone in 90% of cases. Aortography and/or cardiac catheterization were performed if the diagnosis was in doubt or if other cardiac abnormalities were suspected. Surgical treatment involved either resection with end-to-end anastomosis (114) or some form of grafting procedure (10). The results of treatment have been analysed in terms of operative mortality, late mortality, relief of hypertension, and the incidence of re-stenosis.

Thymic cyst simulating aortic aneurysm.

Non-infectious, non-neoplastic cysts of the thymus are rare. Most of them give no symptoms, are located in the anterior mediastinum and are seen in young people. This report deals with a 39-year-old man who had had attacks of precordial pain for 7 years. On admission to hospital he had intense pain in the chest which was attributed to an aortic aneurysm. Retrograde aortography was normal, however. At operation a multilobular thymic cyst was found. The cyst was mostly lined with squamous epithelium.

[Original techniques for biliary surgery (author's transl)].

1. Aspiration of intrahepatic stones. This uses a catheter designed for selective aortography with a special probe, short and provided with a soft cap. The instrument is inserted in the hepatic canal to the contact of the stone, under TV guidance. Strong suction allows extraction. 2. Hepato-gastric prosthesis as palliative. Treatment for cancer of the biliary tract. A. Pezzer sound is placed in a dilated hepatic canal at one end, the other being fixed in the stomach. This is an ultimate solution for far advanced cases, that can only suppress the itching.

[Congestive heart failure and continuous murmur in patient after lumbar disc surgery--a case report].

We present a case of a 61-year-old female who was admitted to the hospital with symptoms of congestive heart failure. Diagnosis of arteriovenous fistula was suggested by the echocardiographic signs of high-output state and a continuous murmur heard especially close to the surgical scar from an intervention on the L4-L5 disc that the patient had undergone eight months before. Aortography confirmed arteriovenous fistula between the right common iliac artery and inferior vena cava. After surgical closure of the fistula, normal cardiac function was restored.

Two cases of quadricuspid aortic valve: preoperative diagnosis and surgical treatment.

Two cases of severe aortic regurgitation were treated surgically. Both patients had a quadricuspid aortic valve, which was diagnosed preoperatively by aortography and/or transesophageal echocardiography (TEE). In the first patient, both aortic and mitral valve replacement were performed with an ATS prosthetic valve. In the second patient, only aortic valve replacement was performed. The postoperative course was uneventful in both cases. Quadricuspid aortic valve is relatively rare. We discuss the preoperative diagnosis of quadricuspid aortic valve, as well as reviewing the relevant literature.
