Stridor in asphyxiated neonates undergoing therapeutic hypothermia.

Therapeutic hypothermia is an established standard of care in the treatment of hypoxic-ischemic encephalopathy. Application of therapeutic hypothermia in the clinical setting may reveal a wider spectrum of adverse events than previously reported. We report 5 cases of transient respiratory stridor in 51 infants, occurring at different time points in the cooling process, which appeared to be unrelated to the intubation procedure. Therapeutic hypothermia was associated with transient stridor in this case series. Formal laryngoscopy is required to determine the underlying pathologic etiology.

Management of renal dysfunction following term perinatal hypoxia-ischaemia.

UNLABELLED: Acute kidney injury frequently develops following the term perinatal hypoxia-ischaemia. Quantifying the degree of acute kidney injury is difficult, however, as the methods currently in use are suboptimal. Acute kidney injury management is largely supportive with little evidence basis for many interventions. This review discusses management strategies and novel biomarkers that may improve diagnosis and management of renal injury following perinatal hypoxia-ischaemia. CONCLUSION: Following perinatal hypoxia-ischaemia, acute kidney injury is common. Management of neonatal acute kidne

Impact of a nationwide training program for neonatal resuscitation in China.

BACKGROUND: Seventeen million births occur in China each year. Neonatal mortality is the leading cause of under 5-year-old child deaths, and intrapartum-related injury accounts for much of mental retardation in young children. The Chinese Ministry of Health sought to improve infant and child survival through a nationwide initiative to have at least one person trained in neonatal resuscitation at every birth. The aim of the current study was to evaluate the impact of China Neonatal Resuscitation Program (NRP) on policy and infrastructure changes and its effectiveness in decreasing the incidence

[Prenatal risk factors for neonatal asphyxia: how risk for each?].

OBJECTIVE: Neonatal asphyxia is the third leading cause of neonatal death and main cause of long-term neurodevelopmental handicap throughout the world. Prevention is more important than treatment. Most previous reports are limited to retrospective investigations of the relationships between some prenatal risk factors and low Apgar scores. This study was designed to prospectively investigate the relationship between prenatal risk factors and neonatal asphyxia and the influence of single or multiple risk factors on the incidence of neonatal asphyxia, and examine significant risk factors for neon

[The clinical features of the perinatal hypocsic damage of central nervous system in infants].

The aim of the given work was the establishment of biochemical and immunological disorders in infants with the complicated pneumonia on a background of hypocsic damage of central nervous system. In the study 120 infants from 1 month to 6 months old, have been surveyed. Infants have been divided into 2 groups. The first group was composed of 90 infants with a pneumonia, which was subdivided into two subgroups: subgroup A--60 infants with pneumonia on a background of hypocsic damage of central nervous system, and subgroup B--30 infants with actual pneumonia. The second group consisted of 30 infa

Resuscitation of newborn infants with 21% or 100% oxygen: an updated systematic review and meta-analysis.

BACKGROUND: The issue of whether 21% O(2) is more effective than 100% O(2) for resuscitation of newborn infants remains controversial. OBJECTIVES: We have updated the systematic review and meta-analysis including all studies reporting resuscitation of newborn infants with 21 or 100% O(2). METHODS: Randomized or quasi-randomized studies of depressed newborn infants resuscitated with 21 or 100% O(2) with or without masking of treatment were considered for inclusion. The outcomes of interest included neonatal mortality and hypoxic ischemic encephalopathy. RESULTS: Ten studies fulfilled the inc

Anticonvulsants for preventing mortality and morbidity in full term newborns with perinatal asphyxia.

BACKGROUND: Seizures are common following perinatal asphyxia and may exacerbate secondary neuronal injury by increasing cerebral metabolic demand, causing fluctuations in oxygenation and perfusion, and triggering the release of excitatory neurotransmitters. Anticonvulsant therapy has been used in infants with perinatal asphyxia in order to prevent seizures. However, long term anticonvulsant therapy may lead to inhibition of brain development. Therefore, the routine use of anticonvulsant therapy to prevent seizures following perinatal asphyxia needs to be evaluated. OBJECTIVES: To assess the e

Pathological findings in the temporal bone of newborn infants with neonatal asphyxia.

Hypoxia in adults frequently causes damage to the brain, but not the inner ear. However, hypoxia in neonates with inadequate blood-inner ear barrier function causes damage to the inner ear, leading to hearing loss and equilibration disorder. Clinically, asphyxiated neonates sometimes have the complication of sensorineural hearing impairment at high frequencies. Many reports suggest that this problem is caused by hypoxic encephalopathy. However, it is not clear whether inner ear disorders are present. In this report we examined the inner ear pathology of four asphyxiated neonates using serial s

[Medical-legal aspects: the pediatrician as a defendant or as an expert].

Claims concerning management of term intrapartum fetal asphyxia account for nearly 45% of all claims filed against pediatricians practicing in the perinatal setting. We recall here briefly the legal and judiciary context under French law, calling upon ten years experience as a medical expert to describe the principal circumstances and clinical forms of acute intrapartum fetal distress which have lead to claims. A few precautionary and preventive measures which should be taken for the clinical forms of intrapartum fetal asphyxia which carry the greatest risk of claims are presented. Finally, pr

[Laryngeal atresia in an infant with chondrodysplasia punctata, rhizomelic type].

Laryngeal atresia had to be unexpectedly detected and treated in a newborn baby. The outer appearance with symmetric shortening of humeri and femora and radiological features with epiphysial and extraepiphysial calcifications led to the diagnosis of rhizomelic chondrodysplasia punctata. This disease is described as a peroxisomal disorder with a poor prognosis on the basis of pathophysiological and diagnostic features.

Defective regulation of cerebral oxygen transport after severe birth asphyxia.

Cerebral artery Doppler ultrasonography was used to study the cerebral blood-flow velocity and cerebral oxygen transport of infants requiring a blood transfusion to correct anaemia. Mean flow velocity, pulsatility index and haemoglobin concentration were determined before and after transfusion. 11 stable preterm infants demonstrated an inverse relationship between haemoglobin concentration (or estimated arterial oxygen content) and mean flow velocity, indicating the presence of a homeostatic mechanism keeping brain oxygen transport within certain limits. Three infants with severe post-asphyxia

Post-asphyxial encephalopathy in a preterm infant.

An infant born at 31 weeks gestation had abnormalities consistent with post-asphyxial encephalopathy, including decelerated fetal heart-rate, cord-blood acidosis and depressed Apgar scores. Clinical signs included respiratory depression, hypotonia and severe seizures. When seen at six months corrected age, however, he had no abnormal neurological signs. The authors conclude that asphyxia in preterm infants may result in clinical abnormalities similar to those in mature infants, but that the abnormalities are separate from haemorrhage and ischaemia.

[Lactic acidosis in recovered asphyctic newborn infants].

Thirty newborn babies delivered by elective caesarean section were studied. They were randomly placed in two groups. The first group consisted of newborns who scored 3 or less on the Apgar scale at one minute and 8 or more after five minutes. The other group included newborns which scored 8 or higher at one minute and 9 at five minutes. Blood samples were obtained from the umbilical cord which allowed for the measuring of the infants' pH, serum gases and pyruvic lactic acid levels. An increased level lactate was found in asphyctic infants. Five cases presented lactic acidosis which continued t
