Predictive value of left atrial function for latent paroxysmal atrial fibrillation as the cause of embolic stroke of undetermined source.

BACKGROUND: In patients with embolic stroke of undetermined source (ESUS), paroxysmal atrial fibrillation (AF) is often diagnosed, however, the risk of paroxysmal AF in ESUS has not been well described. Several studies have suggested a linkage between left atrial (LA) functional parameters and risk of AF in stroke patients. The aim of this study was to assess the role of LA functional parameters as predictors of latent paroxysmal AF in ESUS on admission. METHODS: Between January 2015 and December 2019, consecutive stroke patients with suspected ESUS at admission were prospectively included in

Transesophageal echocardiography measures left atrial appendage volume and function and predicts recurrence of paroxysmal atrial fibrillation after radiofrequency catheter ablation.

AIMS: Paroxysmal atrial fibrillation (PAF) commonly recurs after radiofrequency catheter ablation (RFCA). This study aimed to assess left atrial appendage (LAA) volume and function by transesophageal echocardiography (TEE) and to explore its value in predicting PAF recurrence after RFCA. METHODS: Eighty patients with PAF were recruited. The left atrial (LA) and LAA volume and function were measured by transthoracic echocardiography (TTE) and TEE before ablation. Patients were followed up for 12 months after RFCA, and recurrence was recorded. Odds ratios of candidate risk indicators were deter

Normal left-atrial structure and function despite concentric left-ventricular remodelling in a cohort of patients with Anderson-Fabry disease.

AIMS: Anderson-Fabry Disease (AFD) is an important cause of cardiomyopathy characterized by concentric left-ventricular hypertrophy (LVH). We evaluated the extent of left-atrial (LA) structural and functional remodelling in this group of patients given that LA remodelling is a marker of adverse outcomes in the presence of LVH. METHODS AND RESULTS: Clinical profiles were obtained and cardiac MRI was performed in cohorts of patients with AFD (n = 31), healthy controls (n = 23), and a positive control cohort with known concentric remodelling and LVH (CR/H, n = 21). Of patients with AFD, 58% were

The effects of percutaneous mitral balloon valvuloplasty on the left atrial appendage function in patients with sinus rhythm and atrial fibrillation.

INTRODUCTION: Mitral stenosis (MS) causes structural and functional abnormalities of the left atrium (LA) and left atrial appendage (LAA), and studies show that LAA performance improves within a short time after percutaneous transvenous mitral commissurotomy (PTMC). This study aimed to investigate the effects of PTMC on left atrial function by transesophageal echocardiography (TEE). METHODS: We enrolled 56 patients with severe mitral stenosis (valve area less than 1.5 CM(2)). All participants underwent mitral valvuloplasty; they also underwent transesophageal echocardiography before and at le

[The rate of recovery of left atrial contractile function after the transition to sinus rhythm in patients with paroxysmal atrial fibrillation].

In the examination of 120 patients without valvular heart disease suffering from atrial fibrillation paroxysms mechanical function of the left atrium (LA) was assessed using Doppler echocardiography. The majority of the examinees had dilated LA which is thought to be arrhythmogenic. The recovery of the sinus rhythm is accompanied by electromechanical dissociation the longest duration of which took place in subjects exposed to electrical defibrillation. An inverse relationship was established between the time of the last fibrillation and recovery of LA mechanical function. The duration of the e

Mechanistic insights into atrial functional mitral regurgitation: Far more complicated than just left atrial remodeling.

Recent studies lend credibility to the notion that lone atrial fibrillation (AF) can cause functional mitral regurgitation (MR), commonly referred to as atrial functional MR (AF-MR). The conventional view holds that left atrial enlargement associated with AF causes annular dilatation which gradually moves the mitral valve leaflets apart resulting in inadequate coaptation and regurgitation. Recent findings, however, suggest that AF-MR is not solely related to left atrial remodeling, but that important structural and functional abnormalities of the left ventricle also play a role in its pathogen

Effects of Atrial Fibrillation Cardioversion after Percutaneous Mitral Balloon Valvuloplasty on Echocardiographic Left and Right Atrial Functions.

Amongst patients with mitral stenosis (MS), the most common complication is AF.Our study aimed at evaluating the effect of AF cardioversion after Percutaneous Mitral Balloon Valvuloplasty (PMBV) on echocardiographic atrial functions. The study included 34 patients with MS and AF, presenting to Ain-shams University hospitals, who underwent successful PMBV then randomized into 2 different groups according to AF management strategy. Group-I patients (n=16) received DC cardioversion after amiodarone infusion (within 24 hours post-PMBV) in addition to anticoagulation. Group-II patients (n= 18) were

Left atrial reduction and mitral valve surgery: the "functional-anatomic unit" concept.

The desired outcome for patients undergoing mitral valve surgery includes both good function of the mitral valve, and preservation and restoration of sinus rhythm. To achieve such an outcome, we evolved the concept of the left atrium and mitral valve as a "functional anatomic unit." In this report, we describe a technique for reduction in left atrial size, isolation of the pulmonary veins, and amputation of the left atrial appendage in combination with mitral valve repair. We performed such a procedure in 4 patients, with rheumatic mitral valve disease and chronic atrial fibrillation, with res

Galectin-3 plasma levels are associated with left atrial contractile function in long-distance runners.

High-intensity endurance exercise induces myocardial remodelling as an adaptive response to sustained volume overload, but excessive cumulative training might also favour deleterious cardiac remodelling, including atrial fibrosis. Whether acute marathon running engages circulating fibrosis-related biomarkers and whether these biomarkers track with atrial function in a training volume-dependent manner remains unclear. This study, with within-subject pre- and post-marathon comparisons, evaluated plasma galectin-3 and amino-terminal type III procollagen propeptide (PIIINP) in 36 male long-distanc

Prognostic implications of impaired three-dimensional left atrial function and stiffness in primary mitral regurgitation.

AIMS: In patients with mitral regurgitation (MR), cardiac remodeling by two-dimensional (2D) echocardiography is variable and less suitable for individualized risk assessment. We evaluated whether three-dimensional (3D) peak LA reservoir strain (LASr) and stiffness improve risk prediction in primary MR. METHODS: In the prospective 3D Echocardiography and Cardiovascular Prognosis in Mitral Regurgitation (3D-PRIME) study, 110 patients with moderate or greater primary MR underwent 2D/3D echocardiographic assessment of LASr and LA stiffness (i.e. (mitral E-wave/ annular e’ velocity)/ LASr). Left atrial function and fibrosis in lifelong endurance athletes: a cardiac magnetic resonance imaging study.

Participating in lifelong endurance sports is associated with an increased risk of atrial fibrillation (AF), potentially mediated through the process of left atrial (LA) remodeling which includes the progression of atrial fibrosis. This cross-sectional cohort study aimed to investigate LA remodeling including the presence and localization of LA fibrosis in lifelong endurance athletes. 42 master endurance athletes (76% male, age 54 ± 9 years), participating in Ironman races, (ultra) marathons and the Cape Epic Mountain Bike races, underwent clinical assessment with questionnaires, physical exam

Left Atrial Structure and Function Following Ethanol Infusion into Vein of Marshall (MR-SHALL Study).

BACKGROUND: Ethanol infusion into the vein of Marshall (EI-VOM) is an adjunctive therapy to pulmonary vein isolation (PVI), which improves the efficacy of persistent atrial fibrillation (AF) ablation procedures. It is unknown how EI-VOM scar formation impacts left atrium (LA) structure and function. OBJECTIVE: To characterize scar formation after EI-VOM, and its impact on LA function, by cardiac magnetic resonance (CMR). METHODS: Consecutive patients referred for persistent AF ablation, or any repeat procedure, were included. Ablation strategy included PVI, EI-VOM and linear lesions when dee
