Genome Editing in Medicine: A Scoping Review of Ethical, Bioethical, and Medico-Legal Implications.

Genome editing, prominently led by the revolutionary CRISPR-Cas9 technology, is a powerful tool with significant applications in diverse fields, particularly in medicine and agriculture. It empowers scientists with the ability to effect precise genetic modifications, thereby potentially paving the way for advanced treatments for genetic disorders such as Huntington's disease, hemophilia, and cystic fibrosis. Yet, the significant capabilities of this technology also brings to the fore a myriad of intricate bioethical, legal, and regulatory dilemmas. In light of these complexities, this article

Organs and organisations: Situating ethics in organ donation after circulatory death in the UK.

Controlled organ donation after circulatory death (DCD) has recently been revived in the UK, as part of attempts to increase organ donation rates. The re-introduction of DCD has subsequently become the focus of bioethical controversy, since it necessitates intervening in the care of dying patients to obtain quality donor organs. Transplant policy responses to these concerns have generated new legal and ethical guidelines to address uncertainties around DCD, producing claims that the UK has overcome' the ethical challenge of DCD. In contrast, by drawing on Lynch's call to 'respecify' ethics, th

Our Life Depends on This Drug: Competence, Inequity, and Voluntary Consent in Clinical Trials on Supervised Injectable Opioid Assisted Treatment.

Supervised injectable opioid assisted treament (siOAT) prescribes injectable opioids to individuals for whom other forms of addiction treatment have been ineffective. In this article, we examine arguments that opioid-dependent people should be assumed incompetent to voluntarily consent to clinical research on siOAT unless proven otherwise. We agree that concerns about competence and voluntary consent deserve careful attention in this context. But we oppose framing the issue solely as a matter of the competence of opioid-dependent people and emphasize that it should be considered in the context

Individual breast cancer risk assessment in underserved populations: integrating empirical bioethics and health disparities research.

Research suggests that individual breast cancer risk assessment may improve adherence to recommended screening and prevention guidelines, thereby decreasing morbidity and mortality. Further research on the use of risk assessment models in underserved minority populations is critical to informing national public health efforts to eliminate breast cancer disparities. However, implementing individual breast cancer risk assessment in underserved patient populations raises particular ethical issues that require further examination. After reviewing these issues, we will discuss how empirical bioethi

Giving or giving back: new psychosocial insights from sperm donors in France.

Despite the growing importance of the international scientific literature concerning donor insemination, studies of French samples are rare. We recently had the opportunity to conduct a nationwide study on psychosocial issues related to semen donation in France. In this article, we present the main results of an analysis of the narratives of 33 sperm donors. We examine the meaning they attribute to this experience, their motivations, the social ramifications of their action, and their perspective on the principles of sperm donation in France. We highlight our results by comparing them to those

In the wake of progress--ethical problems of renal failure treated by dialysis.

The ability of dialysis in the 1960s to prolong the lives of patients with terminal renal failure created a running and changing debate in bioethics. The first issue was the selection of suitable individuals to be treated in the limited facilities available. Progress in technology was ahead of any medical or ethical consensus. The costs of treatment soon exceeded the will or means of health funders to pay, creating a system of covert rationing and fiscal fudges with which physicians reluctantly colluded. Evidence, epidemiology and public opinion eventually prevailed and the UK now provides a s

An alternative laboratory designed to address ethical concerns associated with traditional TAS2R38 student genotyping.

The TAS2R38 alleles that code for the PAV/AVI T2R38 proteins have long been viewed as benign taste receptor variants. However, recent studies have demonstrated an expanding and medically relevant role for TAS2R38. The AVI variant of T2R38 is associated with an increased risk of both colorectal cancer and Pseudomonas aeruginosa-associated sinus infection and T2R38 variants have been implicated in off-target drug responses. To address ethical concerns associated with continued student TAS2R38 gene testing, we developed an alternative to the traditional laboratory genotyping exercise. Instead of

The limitations of the Dutch concept of euthanasia.

Why has the law on euthanasia in the Netherlands caused such an upheaval both at home and abroad? In this paper I explore some bioethical issues in the decriminalisation of euthanasia in the Netherlands. The regulatory role of legal and state institutions in the process of decision-making by patients, physicians and other people concerned plays a central role in these discussions. I argue, first, that the limited scope of the Dutch legislation on euthanasia cannot be a solution to end-of-life issues in general, and, second, that it is inadequate as a model for dealing with problems related to

Chapitre 4. Autonomy of minors questioned: Russian case with the loss of medical secrecy.

What is the value of confidentiality? Russian society faced this issue in 2020, when minors aged 15 to 18 lost their privacy. The amendment to the Federal Law, caused the situation, was received ambiguously, but quickly ceased to be the subject of public discussion. In my article, I study this event in a bioethical context, considering the topics of privacy, autonomy and relativity. Social discussion was not productive, since the arguments of both sides contained a double-edged argument: depending on existing relations in a family, the amendment will have a positive or negative meaning. By poi

The bioethics movement in India and the legacy of Sunil Pandya.

The passing of Sunil K Pandya (SKP) on December 17, 2024, evoked memories of his remarkable life both as an individual and a medical professional in many physicians, patients and colleagues. In her tribute, Lopa Mehta has described him as "Medical Ethics personified, an astute clinician, a dexterous neurosurgeon, an ardent historian, a philosopher who stood high above worldly gains and fame" [1]. Early in January 2025, we published an anthology of SKP's select articles in IJME as a tribute to his contribution to bioethics and to the journal. The reminiscences of his friends and colleagues in t

A foundation for public health ethics at Tuskegee University in the 21st century.

This commentary is a reflection on Tuskegee University's National Center for Bioethics in Research and Health Care on the health and ethical challenges of the 21st century. The Center has dedicated the last 10 years to addressing the unresolved biomedicine and public health issues and/or the ethical dilemmas that plague the nation's health. The authors believe that health disparities continue to worsen because the approach under-appreciates the ethical dilemma that plagues health policy and health disparities. The authors discuss synergies and the paradigmatic differences between science and m

Global report on preterm birth and stillbirth (6 of 7): ethical considerations.

INTRODUCTION: Despite the substantial global burden of preterm and stillbirth, little attention has been given to the ethical considerations related to research and interventions in the global context. Ethical dilemmas surrounding reproductive decisions and the care of preterm newborns impact the delivery of interventions, and are not well understood in low-resource settings. Issues such as how to address the moral and cultural attitudes surrounding stillbirths, have cross-cutting implications for global visibility of the disease burden. This analysis identifies ethical issues impacting defini

Noninvasive Prenatal Testing: Views of Canadian Pregnant Women and Their Partners Regarding Pressure and Societal Concerns.

BACKGROUND: Noninvasive prenatal testing (NIPT) provides important benefits yet raises ethical concerns. We surveyed Canadian pregnant women and their partners to explore their views regarding pressure to test and terminate a pregnancy, as well as other societal impacts that may result from the routinization of NIPT. METHODS: A questionnaire was offered (March 2015 to July 2016) to pregnant women and their partners at five healthcare facilities in four Canadian provinces. RESULTS: 882 pregnant women and 395 partners completed the survey. 64% of women anticipated feeling no pressure to take t

End-of-life care and dying: issues raised by staff supporting older people with intellectual disability in community living services.

BACKGROUND: The aim of this study was to explore the current status of end-of-life care and dying of people with intellectual disability based on the experiences of staff in community living services. MATERIALS AND METHODS: Focus groups and individual interviews were conducted, guided by grounded theory methodology. RESULTS: The current status of end-of-life care and dying comprised five key 'issues': knowledge of dying, ethical values, the where of caring, the how of caring and post-death caring. These issues occurred in relationship with 'partners', including the dying person, other client

[Some ethical aspects of donation and transplantation].

Ethical and legal consensus in our country bases the practice of donations and transplants on different ethical principles, which are contained in the legislation, closely conforming to the four principles of principialist bioethics: autonomy, beneficence, non-maleficence, and justice. The level of donations achieved in our milieu might, in fact, be related to a strict respect for these principles by the health professionals, as well as to the excellent organisation of the transplant world. Many scientific, technical and ethical challenges have had to be met to reach the present state of the t

An affordable implantable vagus nerve stimulator system for use in animal research.

In this research, a vagus nerve stimulator has been developed and miniaturized for use in epilepsy research. The board contains all the components necessary for its operation during the standard duration of the experiments, being possible to control it once implanted and even being able to reuse it. The VNS system has been designed for rodents since the VNS devices available for human are not only too large for laboratory animals, but also too expensive. With this solution the expenditure on materials made by laboratories is greatly reduced and bioethical considerations were kept in mind. The

Ethics Students Go to the Jail.

This article describes an educational initiative in which clinical ethics students, who were either in a bioethics master's degree program or in the fourth year of medical school, spent two days observing health care in an urban jail. Students submitted reflections about their experience, in which they drew attention to concerns about privacy, physical restriction, due care, drug addiction, mistrust, and the conflicting expectations that arise when incarcerated people become patients. The rotation was of great value to the students both because it exposed them to many of the ethical issues tha

[When the one who heals dies. The physician at the end of life].

A physician's death represents a critical point of tension between the values of modern medicine and the ethical limits of care. This review article analyzes how physicians face their own end of life and the type of care they receive compared with the general population. A narrative review of literature published between 2014 and 2025 was conducted using PubMed, Scopus, and SciELO databases with the MeSH and DeCS descriptors: "Terminal care", "Palliative care", "Attitude to death", "Health personnel", "Right to die". Findings show that although physicians express a preference for a dignified d
