CORONIS - International study of caesarean section surgical techniques: the follow-up study.

BACKGROUND: The CORONIS Trial was a 2×2×2×2×2 non-regular, fractional, factorial trial of five pairs of alternative caesarean section surgical techniques on a range of short-term outcomes, the primary outcome being a composite of maternal death or infectious morbidity. The consequences of different surgical techniques on longer term outcomes have not been well assessed in previous studies. Such outcomes include those related to subsequent pregnancy: mode of delivery; abnormal placentation (e.g. accreta); postpartum hysterectomy, as well as longer term pelvic problems: pain, urinary problems, i

Bladder neck contracture after retropubic radical prostatectomy: incidence and risk factors from a large single-surgeon experience.

OBJECTIVE: To examine a large, single-surgeon series of patients with prostate cancer who underwent retropubic radical prostatectomy (RRP) for men with postoperative bladder neck contractures (BNCs). PATIENTS AND METHODS: From 1983 to 2007, 4132 men underwent RRP for prostate cancer by one surgeon. All patients had BN reconstruction with mucosal eversion. The bladder to membranous urethral anastomosis was made using six 2/0 chromic catgut sutures over an 18 F Foley catheter. The catheter was left in place for 10 days. Data from these men is stored in a prospective database, which was reviewed

Novel Coating of Surgical Suture Confers Antimicrobial Activity Against Porphyromonas gingivalis and Enterococcus faecalis.

BACKGROUND: The oral cavity is colonized by >10(9) bacteria, many of which can increase heart disease risk when seeded into the bloodstream. Most dentoalveolar surgeries require the use of surgical sutures. Suture placement and removal can increase the risk of postoperative infection and bacteremia. The aim of this study is to evaluate the antimicrobial activity of a novel quaternary ammonium compound, K21, when coated on different suture materials. METHODS: The periodontal pathogen Porphyromonas gingivalis and the endodontic species Enterococcus faecalis were grown to early log phase and ino

From tradition to healing: the promise of acupuncture in managing chronic fatigue syndrome.

Chronic fatigue syndrome (CFS) is a global public health problem affecting more than 65 million patients worldwide. The combined prevalence rate of CFS was 45.2% after 4 weeks in patients with novel coronavirus. Women, people over 40 years of age, and low-income people are susceptible groups, which have a significant impact on immune, nervous, endocrine, and other system functions. First, from the perspective of epidemiology, this paper reviews the global epidemic trend of CFS, the differences in incidence and prevalence in different regions and populations, and risk factors such as heredity,

Variant CJD (vCJD) and bovine spongiform encephalopathy (BSE): 10 and 20 years on: part 2.

Up until February 2006, variant CJD (vCJD), the human disease associated with transmission of BSE from cattle, has been confirmed in 160 patients resident in the UK and 28 elsewhere, some of whom have never visited the UK. Cases have been reported in France (16 cases), Ireland (3), USA (2), Canada, Italy, Japan, The Netherlands, Portugal, Saudi Arabia and Spain (1 each). The presumed main period of hazard for ingestion of the BSE agent in bovine products in the UK is 1984-89, or perhaps up to 1995-6 but at a reduced level. Debated incubation periods for vCJD are discussed, with special referen

The CORONIS Trial. International study of caesarean section surgical techniques: a randomised fractional, factorial trial.

BACKGROUND: Caesarean section is one of the most commonly performed operations on women throughout the world. Rates have increased in recent years - about 20-25% in many developed countries. Rates in other parts of the world vary widely.A variety of surgical techniques for all elements of the caesarean section operation are in use. Many have not yet been rigorously evaluated in randomised controlled trials, and it is not known whether any are associated with better outcomes for women and babies. Because huge numbers of women undergo caesarean section, even small differences in post-operative m

Current state of research on acupuncture for acne: a scoping review.

OBJECTIVE: Acne vulgaris is recognized as one of the top eight most disabling dermatological diseases globally. Acupuncture has emerged as a clinically valuable and widely practiced intervention for acne, with the World Health Organization endorsing it as an effective non-pharmacological treatment. While existing evidence demonstrates acupuncture's ability to significantly improve acne symptoms, the research remains scattered and lacks comprehensive synthesis. This scoping review systematically maps the current clinical research on acupuncture for acne treatment to identify knowledge gaps and

Comparative effectiveness of different acupuncture therapies for neck pain.

BACKGROUND: Neck pain is a common musculoskeletal symptom that has negative effects on quality of life and work productivity. Acupuncture has been widely used for neck pain, and a number of randomized controlled trials (RCTs) and systematic reviews (SRs) have evaluated its effectiveness. However, previous studies have obtained inconsistent results regarding the effects of acupuncture for neck pain, and there is no SR for the comparative efficacy and safety of various types of acupuncture. Therefore, we herein conducted a SR and network meta-analysis to compare and rank different types of acupu

[Perforation of the ear drum. On the history of paracentesis and grommet insertion].

As early as 1649, Jean Riolan the Younger pierced an ear drum, after which the patient's hearing improved. This occurred as a result of an accidental ear drum injury while cleaning an ear canal with an ear-spoon. In 17th and 18th centuries, several pioneers in medicine (Thomas Willis, Antonio Mario Valsalva, William Cheselden) conducted experiments in an effort to ascertain the function of the ear drum in hearing. At the end of the 18th century, ear drum perforation, like perforation of a cataract, was indiscriminately performed by itinerent quacks and "physicians" in England, France, and Germ

Suture materials and other factors associated with tissue reactivity, infection, and wound dehiscence among plastic surgery outpatients.

The most common complications in plastic surgery are tissue reactivity, infections, and wound dehiscence. In the literature, there are only a few studies with sample sizes large enough and methods of statistical analysis appropriate for evaluating the role of suture materials in inducing such complications. In the 1000 plastic surgery outpatients in this study, the association of different suture materials, individual patient characteristics, surgeon skill, and wound site and length with postoperative wound complications (i.e., tissue reactivity, infection rate, and wound dehiscence) were inve

Randomised controlled study of two different techniques of skin suture in endoscopic release of carpal tunnel.

In a prospective, randomised trial of 54 hands in 47 patients incisions were randomised to be closed by either absorbable subcuticular (polyglytone 6211, Caprosyn), or non-absorbable interrupted (polybutester, Novafil), sutures after single-portal endoscopic release of the carpal tunnel. There was a significant reduction in pain scores on days 1 and 2 in the patients treated with an absorbable continuous subcuticular suture, and no difference in inflammation or infection. There was no difference in the cosmetic appearance between the two groups after three months.
